The Strategy to respond to antimalarial drug resistance in Africa is a technical and advocacy document, grounded in the best available evidence to date and aimed at minimizing the threat and impact of antimalarial drug resistance of Plasmodium falciparum parasites in Africa. Its objectives are to: i...) improve the detection of resistance to ensure a timely response; ii) delay the emergence of resistance to artemisinin and artemisinin-based combination therapy (ACT) partner drugs; and iii) limit the selection and spread of resistant parasites where resistance has been confirmed.

La Stratégie de riposte face à la résistance aux antipaludiques en Afrique est un document technique de sensibilisation qui se fonde sur les bases factuelles les plus solides dont on dispose et vise à réduire au maximum la menace et l’impact de la résistance aux antipaludiques de Plasmodium ...falciparum en Afrique. Elle a pour objectifs i) d’améliorer la détection de la résistance et d’assurer une riposte en temps utile ; ii) de retarder l’émergence de la résistance à l’artémisinine et aux médicaments associés utilisés dans les combinaisons thérapeutiques à base d’artémisinine (CTA) ; et iii) de freiner la sélection et la propagation des parasites résistants là où une pharmacorésistance est confirmée. Si la Stratégie cible l’Afrique, il convient de noter que la
menace de la résistance aux antipaludiques concerne tous les pays d’endémie palustre et que les interventions proposées dans le présent document sont applicables dans d’autres régions géographiques à condition d’être adaptées aux spécificités locales.

En este manual se explica cómo ejecutar las medidas con vistas a logra...r la eliminación de la malaria y prevenir su restablecimiento, mediante el aumento de la intensidad y la calidad de las medidas, la reorientación de las iniciativas, la reducción de los retrasos que favorecen la transmisión y un monitoreo adecuado que permita ajustar las intervenciones. En este sentido, se propone como primer paso estratificar el territorio —de modo que se puedan planificar y priorizar las intervenciones y grupos de población— para, a continuación, aplicar una medida de diagnóstico, tratamiento, investigación y respuesta como estrategia clave dirigida a hacer efectiva la eliminación de la enfermedad y a prevenir su restablecimiento en todos los estratos. Además, el documento trata de hacer operativo el concepto de vigilancia, como intervención promovida por la Organización Mundial de la Salud a través de la Estrategia Técnica Mundial contra la Malaria 2016-2030, y destaca la importancia de que las comunidades tengan acceso al diagnóstico y el tratamiento tempranos. All malaria-endemic countries in the Region of the Americas have taken on the challenge to eliminate the disease and to put in place measures to orient their health programs and strategies in that direction. This manual explains how to implement measures to achieve malaria elimination and prevent it...s reestablishment by increasing the intensity and quality of interventions, reorienting initiatives, reducing delays that favor transmission, and ensuring adequate monitoring to adjust interventions. In this regard, the first suggested step is to stratify the territory - so that interventions and population groups can be planned and prioritized - and then implement diagnosis, treatment, investigation and response (DTI-R) actions as a key strategy aimed at effectively eliminating the disease and preventing its reestablishment in all strata. In addition, the document seeks to operationalize the concept of "surveillance as an intervention" promoted by the World Health Organization through the Global Technical Strategy for Malaria 2016-2030 and highlights the importance of communities having access to early diagnosis and treatment. Finally, the manual also includes management and monitoring aspects that must be taken into account to eliminate the foci with transmission.

The Plan of Action for Malaria Elimination 2021-2025 has been developed in consultation with countries and regional partners as a framework of reference to guide the efforts of countries and the contributions of donors and partners towards elimination of the disease in the Americas. The Plan subscri...bes to the goals and pillars of the WHO Global Technical Strategy against Malaria 2016-2030 (GTS), while presenting key elements to address the specific challenges of the Region. The document seeks to guide national plans and promote an inter-programmatic- intersectoral approach, and joint efforts between countries and partners. The Plan is also the reference framework for PAHO technical cooperation in malaria in the period 2021-2025. The goals to be achieved require changes in action against malaria that must occur at the operations level and for which regulatory and policy adjustments are required from the national levels. Thus, the Plan promotes a systematic action of detection, diagnosis and response, which must be massively implemented and monitored programmatically. A main element of change is action aimed at recognizing the need to address key malaria foci in each country with specific, information-based operational solutions. The Plan of Action seeks to promote these changes in malaria programs in the countries and through the interactions among all actors. The model proposed in this plan is based on a cross-functional dialogue and interconnections across the lines of action (SL). Strategic lines 1, 2 and 3, which correspond to the three pillars of the WHO/GTS, complement each other, and are not designed to function independently. The idea of the consolidated supportive elements - strengthened health systems, strategic planning, financing, partnerships, advocacy, and operational research (SL 4) is to provide the platform, operational structures and alliances for the more specific malaria interventions presented in strategic lines 1, 2 and 3.

La thèse de Yobouet Inès Kouakou, soutenue à l’Université Claude Bernard Lyon 1, porte sur le traitement d’urgence du paludisme sévère chez l’enfant à l’aide d’une administration intranasale d’artésunate. L’objectif principal de ce travail est d’explorer, in vitro, la voie na...sale comme alternative à la voie intra-rectale recommandée actuellement en pré-hospitalier. L’étude démontre la non-toxicité de formulations d’artésunate sur un modèle de muqueuse nasale humaine, bien que la perméation observée reste faible. Des pistes sont proposées pour améliorer cette perméation, notamment par l’optimisation galénique. La thèse inclut également une revue de la pharmacocinétique de l’artésunate et le développement de méthodes de dosage simples et peu coûteuses, adaptées aux laboratoires à ressources limitées. Ce travail s’inscrit dans une démarche de lutte contre le paludisme, notamment en zones rurales où l’accès aux soins reste difficile.

The document was developed to address the public health need caused by the evolution and spread of insecticide resistance, particularly to pyrethroids. Such resistance threatens the effectiveness of the current standard of malaria vector control in many countries, namely pyrethroid-only long-lasting insecticidal nets (LLINs).
