Background: MSF -OCB a stimul...é avec succès le lancement des soins du VIH à Maputo (Mavalane) et dans la province de Tete depuis 2001. Au départ, les patients étaient traités dans des hôpitaux de jour centraux, mais, depuis 2007, les traitements sont décentralisés dans des centres médicaux de la province. Depuis 2002, MSF soutient les autorités sanitaires à Tete dans la mise en oeuvre et l'élargissement des soins et du traitement du VIH. L'approvisionnement en ARV a commencé en mai 2003 à l'hôpital provincial de Tete et a été décentralisé dans des centres de santé périphériques sélectionnés en 2006. En 2008, en collaboration avec des patients et les autorités locales de la santé, MSF -OCB a lancé un modèle pilote de distribution d'ARV et de suivi du respect du traitement par des groupes communautaires, en complément à la méthode conventionnelle de distribution des ARV dans les cliniques et hôpitaux. Ce modèle a initialement été proposé lors de discussions entre des conseillers et des patients sous TARV comme moyen d'améliorer le respect du traitement tout en offrant davantage de responsabilités aux patients pour certains aspects de la prestation de service. Ce modèle permet aux patients de devenir acteurs de leurs soins à vie, ce qui réduit les obstacles liés aux soins. Ce modèle implique un réseau de patients sous TARV, le groupe communautaire d'adhérence au traitement (CAG), qui prend en charge la distribution des ARV, le contrôle du respect du traitement et la surveillance. Fin 2010, MSF -OCB a délégué au ministère mozambicain de la santé (MS) les activités de soutien clinique classique des centres de santé de la ville de Tete et du district de Moatize. En janvier 2011, le projet CAG a été lancé pour une durée de trois ans et, en avril 2011, un atelier national a été organisé à Tete au sujet du déploiement de la stratégie CAG au niveau national. En avril 2011, un outil de laboratoire innovant pour le diagnostic de la TB et de la TB -MR, le GeneXpert (GE), a été installé dans le centre de santé de Moatize. La même année, MSF a commencé à travailler avec les professionnels du sexe (PS). En 2013, l'objectif principal du projet reste l'autonomisation des patients et nous avons lancé des activités pilotes de soutien communautaire et d'autogestion pour des groupes comme les femmes enceintes, les patients pré -TARV et les patients souffrant de la TB. - General objective: Pour ce programme, nous avons donc choisi pour thème global l'optimalisation du traitement du VIH/TB et des maladies chroniques non transmissibles, et la conception de modèles de soins intégrés pour les maladies chroniques au niveau communautaire, du patient et des centres de santé. Background: afrique du Sud : ...MSF est intervenue en Afrique du Sud dans les années 1980 afin de venir en aide aux réfugiés du Mozambique. Elle y est restée jusqu'en 1993. En 1999, l'organisation est retournée sur le terrain pour répondre à la flambée de l'épidémie de VIH/sida. La mission, coordonnée à partir du Cap, couvre à la fois l'Afrique du Sud et le Lesotho. Projets récent de la mission : · Le projet TB/VIH à Khayelitsha, précédemment soutenu par la DGD, inclue au total plus de 15 000 personnes actuellement en vie et sous traitement antirétroviral (TARV). Ce projet comporte un volet axé sur les soins et le traitement de la TB résistante, de plus en plus important. · Un projet axé sur la santé des migrants, à Musina, propose des services mobiles axés sur la TB/VIH dans les exploitations agricoles, des services de soins de santé aux immigrés/réfugiés venant d'arriver du Zimbabwe ainsi que des services de soins intégrés pour les victimes de violences sexuelles et à caractère sexiste (VSS). · Un projet mis en oeuvre au Lesotho montrera que l'intégration des services VIH/TB et des services de santé mèreenfant et de santé reproductive, l'amélioration des soins VIH, notamment dans le cadre de la PTME, et des améliorations minimes au niveau des services de santé mère -enfant et de santé reproductive peuvent permettre de diminuer rapidement et considérablement la mortalité maternelle et infantile et que ces initiatives peuvent être reproduites. · Un projet axé sur la santé des migrants dans le centre défavorisé de Johannesburg vise à améliorer l'accès aux services publics de santé des migrants vulnérables qui vivent dans des immeubles abandonnés du centre -ville. · Le projet de « réduction de l'incidence du VIH », dans le KwaZulu -Natal, s'emploiera à démontrer qu'il est possible de diminuer sur le long terme l'incidence de la TB et du VIH en améliorant considérablement l'accès au traitement contre la TB et l'offre de TARV. Ce projet a démarré en 2011. MSF a également mis en oeuvre un programme VIH/sida au Lesotho, qui s'est terminé à la mi -2010 (passation) et à Lusikisiki, dans la province du Cap oriental, en Afrique du Sud, projet confié à la fin 2006 au Département provincial de la santé. Les trois projets de Khayelitsha, de Lusikisiki et du Lesotho ont fait office de sites pilotes de démonstration, l'objectif étant d'amener le gouvernement sud -africain et du Lesotho, ainsi que la communauté internationale, à assurer des traitements ARV dans des contextes pauvres en ressources. Le projet du KZN devrait aussi devenir un site pilote permettant à MSF de démontrer la faisabilité d'une série d'interventions visant à diminuer les nouvelles infections au VIH et à la TB et d'influencer les politiques sud -africaines et régionales en vue du contrôle de l'épidémie de VIH et de sida. Background: The concrete obj...ective of this Partnership Agreement shall be to support the actions of UNAIDS which are aimed at reinforcing the multi -sectoral and integrated response to the HIV epidemic, especially in the following 8 selected countries of the Southern African Development Community (SADC): Botswana, Lesotho, Malawi, Mozambique, Namibia, South Africa, Swaziland and Zimbabwe. In this way Flanders acknowledges UNAIDS as a strategic partner in achieving the relevant specific target within Millennium Development Goal 6 to have halted by 2015 and begun to reverse the spread of HIV/AIDS. Flanders thus endorses the general strategy adopted by UNAIDS, which is to assist partner countries in moving towards universal access to HIV prevention, treatment, care and support. However, at the same time it would like its resources to be used specifically for implementing this strategy in the aforementioned 8 partner countries. More generally speaking, UNAIDS and the Flemish Government want to conclude this new agreement to reconfirm the relevance of structural cooperation between them. Background: En réponse à la... situation nutritionnelle alarmante dans la région, MSF a ouvert la mission au Zimbabwe en février 2003 en mettant sur pied un centre nutritionnel thérapeutique au Murambinda Mission Hospital, dans le district de Buhera. Comme il est apparu clairement que la plupart des enfants souffrant de malnutrition souffraient de pathologies sousjacentes liées au VIH/sida, nous avons décidé d'ouvrir une clinique spécialisée dans les infections opportunistes (IO) – ainsi que dans l'instauration du TARV – en mars 2004. Au cours des deux premières années, le nombre de nouveaux patients mis sous traitement ARV dans la clinique pour la prise en charge des Infections Opportunistes est resté faible, commençant à augmenter à partir de la 3e année. En 2006, MSF a pris la décision de décentraliser les soins ARV vers les cliniques du district. Jusqu'en 2008, un réseau de 2 équipes mobiles géré par MSF a pu répondre aux besoins. En 2009, quatre équipes mobiles de sept personnes chacune couvraient 22 sites, qui bénéficiaient de visites hebdomadaires. Lorsqu'elle a pu atteindre l'objectif de couverture universelle, en 2010 (pour des seuils d'instauration du traitement fixés à CD4 =350 cellules/mm³), MSF a décidé d'intégrer ces services de soins dans les services du Ministère de la Santé. À cette fin, elle a formé le personnel dans les cliniques, en utilisant une approche de mentorat mobile. L'intégration des services de soins a été achevée à la mi -2013, et le personnel du MS prend à présent en charge une cohorte de plus de 15 000 patients sous traitement de 1ère intention. MSF se concentre sur les patients en traitement de 2e intention et les groupes spéciaux comme les enfants, les adolescents et les hommes. À la mi -2010, MSF a décidé d'étendre son soutien à 2 districts voisins (Gutu et Chikomba) qui avaient signalé une faible couverture par ARV. En effet, à la mi -2010, le district de Gutu – dont la population et le taux de prévalence sont comparables à ceux du district de Buhera – ne recensait que 1 800 patients sous ARV et intégrés dans le parcours de soins, alors que le district de Buhera en comptait 10 fois plus. Au début 2011, MSF a donc lancé son projet à Gutu, en mettant en oeuvre une approche de mentorat mobile et en optant d'emblée pour l'intégration des soins dans les services du MS. En deux ans, MSF a réussi à améliorer l'accès des patients en décentralisant les soins vers 23 structures (11 sites d'instauration du traitement et 6 sites pour le suivi) faisant ainsi passer la couverture TARV de 10 - 15 % en 2010 à 44 % à la fin 2012. Depuis la fin 2011, MSF soutient également le district de Chikomba. En novembre 2008, MSF -OCB est intervenue en réponse à ce qui allait devenir la plus grave épidémie de choléra que le Zimbabwe ait connue. General description: The conc...rete objective of this Partnership Agreement shall be to support the actions of UNAIDS which are aimed at reinforcing the multi-sectoral and integrated response to the HIV epidemic, especially in the following 8 selected countries of the Southern African Development Community (SADC): Botswana, Lesotho, Malawi, Mozambique, Namibia, South Africa, Swaziland and Zimbabwe. In this way Flanders acknowledges UNAIDS as a strategic partner beyond the targets within Millennium Development Goal 6, working towards ending the AIDS epidemic as a public health threat by 2030.Flanders thus endorses the UNAIDS 2011–2015 strategy GETTING TO ZERO adopted by UNAIDS in 2011, and supports decision 7.2. taken at the 34th meeting of the UNAIDS Programme Coordinating Board (PCB) with regard to agenda item 5.1. (Mid-term review of the UNAIDS 2012-2015 Unified Budget, Results and Accountability Framework, UBRAF) to extend the duration of the existing strategy for two years through 2017. However, at the same time, Flanders would like its resources to be used specifically for implementing this strategy in the aforementioned 8 partner countries. More generally speaking, UNAIDS and the Government of Flanders want to conclude this new agreement to reconfirm the relevance of structural cooperation between them. In addition, the Government of Flanders wishes to reaffirm the lasting, objective need for the existence of UNAIDS as a special UN Joint Programme for global, regional and national (1) coordination, (2) leadership, (3) policy impact and (4) the collection and channelling of strategic information in the fight against HIV/AIDS. -- Specific purpose : The UNAIDS Secretariat shall use the resources provided by Flanders: -- • to support the development and dissemination of normative standards and good practices for the coordinated response to HIV/AIDS, especially in the 8 selected countries; -- • to facilitate the extension to universal access to combination prevention, treatment, care and support in the 8 selected countries, in accordance with the relevant international declarations, and shall to this end continue to advocate the universal application of human rights as an important instrument; -- • to ensure the SADC region, but especially the 8 selected countries, a proper place on the global AIDS agenda and in the global strategy development and implementation; -- • to stimulate insistently and well coordinate integrated co-sponsor activities in the 8 selected countries, in keeping with the agreed national plans and with the strategy defined by the UBRAF and other relevant UNAIDS-guidelines; -- • to facilitate and to support the work regarding HIV/AIDS of the SADC Secretariat and in particular of the 8 selected countries through the UNAIDS Regional Support Team for East and Southern Africa and the Regional Directors' Forum; -- • to faithfully implement the relevant recommendations from the Mid-Term Review of the 2012-2015 UBRAF. -- --

|
General description: In Mozam...bique, the equality between men and women is one of the major challenges for the future generation and the health sector is affected by strong inequalities that prevent women from having a healthy life. Maternal mortality is still high in Mozambique, the main indirect causes for maternal mortality is HIV, hence prevention of mother to child transmission of HIV is very important. -- -- The general objective of the present project is the promotion of good sexual and reproductive health in young girls and women in Mozambique. In particular, the present project will be implemented in the south of the country in the nearby of the capital Maputo, in the areas of Zimpeto, Maputo city center, Machava and Matola II. -- -- The specific objective of the project is that women living in the areas involved make use of a package of health and social services for the improvement of sexual and reproductive health (PMTCT, HIV treatment, HPV screening and treatment). -- -- The results of the project will be: (1) a high quality diagnostic service, (2) a high quality care service, and (3) a support service for the HIV+ women living in the areas of Zimpeto, Maputo city center, Machava and Matola II DREAM centers. -- The number of direct beneficiaries of the present project is estimated to be 8.200 women and 400 young girls. Indirect beneficiaries will be the male teenagers involved in the support groups, male partners of the women, children born in the PMTCT program, and relatives of the women. Background: This project will ...deliver high-quality interactive trainings and produce a highly effective Interpersonal Communication (IPC) Toolkit of resources such as job aids and complementary multimedia materials for use in mass media, health facility, school and community settings by a variety of actors. We will draw on Social and Behaviour Change Communication methodology to make research-based improvements to existing training and IPC resources, as well as the development and implementation of new tools for long-term sustainability and potential national expansion. The direct beneficiaries of this project are adolescent girls and boys aged 10 to 19 as well as community members such as parents and other gatekeepers who will benefit through improved SRHR knowledge. Indirect beneficiaries include various service providers and educators who will benefit from capacity building through improved training and access to resources to support service provision and communication activities for adolescents and community gatekeepers. The results we expect to deliver through this project are as follows: 1. Health workers are better equipped with the resources and motivation to provide inclusive and appropriate SRH services to explain and promote reproductive health rights and increase demand for services 2. Local, traditional and religious leaders, teachers, community groups and activists, Geração Biz peer educators, parents and other family members are reached with high quality, useful information on ASRHR through media initiatives 3. Large numbers of adolescents, including those with disabilities, are reached with high quality, accessible SHRH information and communications materials in order to improve their skills and ability to discuss the issues. 4. Programme design will be optimized and improved, and gaps identified through formative and other research, including using a positive deviance approach to improve effective interpersonal communication on ASHRH.

Background: PSI will strengthe...n demand for family planning services by co-designing with AGYW and working to remove community barriers to services, building girls' agency, and fostering an enabling environment. To address supply- side challenges, including insufficient number of trained human resources, PSI/MZ will enhance provider competency and confidence to provide family planning services, including insertion and removal of long acting reversible contraception (LARC), thereby strengthening the overall Mozambique health system. The project' aims to increase use of modern contraception among AGYW as measured by 127,000 couple years of protection (CYPs), including an estimated 25% of CYPs delivered to AGYW 10-19 years and 25% delivered to AGYW 20-24 years, and a 50% increase in LARC services delivered through partner health clinics. To achieve this goal, PSI/MZ will build on its existing networks of family planning providers and communication agents, implementing transformative changes for adolescents within their networks using human-centred design (HCD) approaches and scaling what works. The proposed project will work with a range of target groups to ensure a positive enabling environment for improved AGYWs' access to SRH information and services in-cluding health care providers, adolescent boys, and primary influencers and gatekeepers (i.e. parents, boyfriends or sexual partners, community and religious leaders, and community radio stations).

Background: The INGUISSA Proje...ct aims to increase adolescent access to and use of sexual and reproductive health services through health system strengthening, human resource capacity building, service delivery, development of education and health programs, and adolescent engagement. in the provision of SRHR services. Thus, the project brings an approach to providing preventive and curative health services for adolescents. The project will be implemented in Maputo Province, Manhiça and Matola districts where two mobile brigades will be implemented to cover schools and the community, 1 model SAAJ, 8 secondary schools, and 5 SAAJ services. With these strategies we anticipate that over the 3 years a provincial intersectoral committee (Health and Education) will be established whose inclusion of adolescents is a requirement, 24 teachers will be trained in the school package, 200 health workers in SAAJ services, established union network of community life, revitalized / created 08 school corners, 230248 adolescents will receive family planning, 259028 will receive HIV testing and 230248 will have access to sexual and reproductive health services in both districts.

This project seeks to improve ...the health of the population in all districts of Bangladesh by strengthening the Bangladesh Ministry of Health and Family Welfare's management of the public health care system and by improving public health services. The project forms the cornerstone of Canada's support for achieving development objectives in the health sector in Bangladesh and its main focus is to help implement the Ministry's Health, Population and Nutrition Sector Development Program. The project activities include: (1) improving the financial management of the Ministry's aid budget; (2) procuring obstetric drugs and family planning supplies in low performing districts with high maternal, newborn and child mortality rates; (3) updating the regulatory framework for the accreditation of health personnel; (4) improving the planning, development and management of human resources for health; (5) increasing the availability of maternal health services; and (6) developing and implementing a monitoring and evaluation plan within the Ministry of Health and Family Welfare. As with all pooled funding initiatives, Canada is working in close cooperation with other donors and the Government of Bangladesh to focus on effective, transparent, and accountable country systems; increase donor coordination and harmonization; and strengthen mutual accountability. This project is continuously monitored and evaluated in coordination with other donors, which include the World Bank, the United Kingdom, Sweden, Australia, Germany and the United States. / Ce projet vise à améliorer la santé de l'ensemble de la population vivant dans les districts du Bangladesh, en renforçant la gestion du système public de soins de santé par le ministère de la Santé et du Bien-être de la famille et en améliorant les services de santé publique. This project is in line with B...urkina Faso s poverty reduction strategy and rural development strategy. The project contributes to poverty reduction, in a spirit of partnership, by developing Burkina Faso s production capacity (livestock breeding and agriculture). The activity follows a strategic approach based on sub-sector analyses and the respect of market forces. The purpose of the activity is to revitalize three target agri-food sub-sectors (milk, bananas, shea butter), mainly for the local market but also for the sub-regional and international markets, depending on the sub-sector, to increase household income and job opportunities in target urban and rural areas. To this end, three basic objectives have been identified: (1) revitalizing sub-sectoral consultation tables; (2) professionalizing services by professional organizations to operators; and (3) innovation and market access. The main impacts of this project are the diversification of Burkina Faso s production base and improved competitiveness as well as an improved food balance. / Ce projet s inscrit dans le cadre de la stratégie de réduction de la pauvreté et de la stratégie de développement rural du Burkina Faso. La finalité du projet est de contribuer, dans un esprit de partenariat, à réduire la pauvreté via le développement du potentiel productif (élevage et agriculture) du Burkina Faso. Suivant une approche stratégique par filière sur la base d une logique commerciale, le but de l intervention est de dynamiser trois (3) filières agro-alimentaires cibles (lait, banane et karité) destinées surtout au marché local mais aussi, suivant les filières, aux marchés sous-régional et international, afin d augmenter les revenus des familles et d accroître les opportunités d emploi dans les milieux rural et urbain ciblés. Pour y parvenir, trois objectifs essentiels ont été dégagés : 1) la dynamisation des tables filières; 2) la professionnalisation des services des organisations professionnels aux opérateurs; et 3) l innovation et l accès aux marchés. Les principaux impacts de ce projet sont la diversification de la base productive du Burkina Faso ainsi que l amélioration de sa compétitivité et de son bilan alimentaire.

The goal of this project is to... help strengthen the detection, treatment, and prevention of acute malnutrition and to help combat some of the major causes of child mortality, through immunization and improved care at the community level. The project aims to support the provision of basic health services, contribute to the response to the nutrition crisis in Mali, and ensure that the Malian population has access to vaccines and essential drugs. The project involves establishing a mechanism for active detection and treatment of severely malnourished children at the community level; promoting adequate nutrition for infants and young children; as well as ensuring the timely supply of vaccines included in the national immunization program. Additionally, the project includes providing refrigerators to community health centers for vaccine storage, ensuring timely supply of medicines for essential care at the community level, and distributing light medical equipment to community health centres. This project is part of Canada s maternal, newborn, and child health commitment. / Ce projet a pour but de renforcer la détection, le traitement et la prévention de la malnutrition aigüe et d aider dans la lutte contre certaines des principales causes de mortalité infantile, grâce à l immunisation et l amélioration des soins dans les collectivités. Ce projet vise à soutenir la prestation de services de santé de base, à venir en aide aux victimes de la crise alimentaire au Mali et à veiller à ce que la population du Mali ait accès à des vaccins et à des médicaments vitaux. Le projet consiste à établir un mécanisme pour la détection active et le traitement des enfants atteints de malnutrition sévère dans les collectivités; à faire la promotion d une nutrition adéquate pour les nourrissons et les jeunes enfants; et à veiller à la prestation, en temps opportun, des vaccins compris dans le programme de vaccination national. This project is in line with the priorities of the Muskoka Initiative relating to the health of mothers, newborns and children. / Ce projet vise à réduire les cas de maladie et le nombre des décès chez les mères et les enfants en améliorant la prestation de services de santé sexuelle et génésique complets et intégrés aux collectivités pauvres, marginalisées et à risque élevé. L'accès aux services de santé sexuelle et génésique, incluant la planification familiale, est essentiel pour une amélioration durable de la santé maternelle et infantile. L'accès à ces services rend les femmes et filles plus autonomes, améliore leurs perspectives de vie et réduit la mortalité maternelle et infantile causée par la transmission du VIH et les grossesses involontaires. Ce projet est conforme aux priorités établies par l'Initiative de Muskoka à l'égard de la santé des mères, des nouveau-nés et des enfants.

The Health Services Delivery P...rogram (HSDP) targets up to 54 districts in the provinces of Niassa, Nampula and Cabo Delgado, which together are home to 6 million Mozambicans or approximately 30 percent of the population. These provinces have some of the lowest ranking health indicators in the country. The project is implemented by the Mozambican Ministry of Health, in collaboration with the World Bank supported by CIDA, the Swiss Agency for Development and Cooperation and the Russian Development Cooperation. The project aims to scale up access to health service delivery through: facility-based service delivery, outreach, and community-based services. It aims to provide a cost-effective and innovative way for the Government of Mozambique to increase health services to some of the most disadvantaged populations, while helping to build the basis of a health management system. Key activities include training 1,400 community-based health workers, 162 nurses and medical technicians, 200 traditional healers and midwives in child and maternal health; expanding the breadth of training for up to 54 teams of outreach workers; purchasing vehicles necessary for outreach workers to access isolated communities; rehabilitating existing health facilities; and constructing 21 new health facilities including staff housing. The World Bank s HSDP project team works closely with the Ministry of Health to ensure that this project aligns with the Ministry s new Community Health Worker (CHW) Revitalization program. The CHW program aims to standardize training, work conditions and equalize distribution of workers throughout the country. / Le Programme de livraison des service de santé (PLSS) cible jusqu à 54 districts dans les provinces de Niassa, de Nampula et de Cabo Delgado, qui comptent au total six millions de Mozambicains ou environ 30 % de la population. The overall goal of this proje...ct is to support the implementation of the Caribbean Regional Strategic Plan of Action for HIV/AIDS. The project supports six initiatives implemented by regional organizations and institutions to reduce the spread and impact of HIV/AIDS: (i) The Caribbean Community (CARICOM) Secretariat is improving regional coordination of activities addressing HIV/AIDS and assisting member states in responding to the legal, ethical, and human rights challenges posed by the epidemic. (ii) The CARICOM Secretariat is also strengthening networking, linkages, and the exchange of information between Caribbean countries, in order to help ensure a coordinated regional response to the epidemic. (iii) The Caribbean Food and Nutrition Institute is enhancing the quality of life of people infected with and affected by HIV/AIDS. The Institute focuses on improving nutrition and reducing the transmission of HIV/AIDS from mothers to children. (iv) The Caribbean Conference of Churches is helping churches and other faith groups to provide care and assistance to people living with HIV/AIDS and to increase awareness about HIV/AIDS among communities and congregations. (v) The Caribbean Family Planning Affiliation is focusing on youth through the development of peer education activities and youth-friendly health services. (vi) Population Services International is contributing to prevention efforts in the Eastern Caribbean to reduce the incidence of HIV/AIDS. / Le but général de ce projet est d appuyer la mise en application du Cadre stratégique régional des Caraïbes pour le VIH/sida. Le projet appuie six initiatives mises en oeuvre par des organisations et institutions régionales en vue de freiner la propagation du VIH/sida et de réduire les effets de celui-ci : i) Le Secrétariat de la Communauté des Caraïbes (CARICOM) améliore la coordination régionale des activités de lutte contre le VIH/sida et appuie les États membres aux prises avec des problèmes d ordre juridique et moral et sur le plan des droits de la personne que pose l épidémie. ii) Par ailleurs, le Secrétariat de la CARICOM travaille aussi à renforcer les réseaux, les relations et les échanges d information entre les pays de la région, dans le but d assurer une action coordonnée dans la lutte contre l épidémie. iii) L Institut des Caraïbes pour l alimentation et la nutrition contribue à améliorer la qualité de vie des personnes infectées et affectées par le VIH/sida. The project aims to improve th...e health of people affected by AIDS, particularly children, lower rates of HIV and tuberculosis (TB) infection, as well as increase community awareness of HIV/AIDS and TB testing services, prevention, and treatment.At the time of the project launch, a study from the Human Sciences Research council indicated that the prevalence of HIV in South Africa was at its highest rate in history. In the project target area of Ngqushwa district in the Easter Cape province, HIV prevalence rates fluctuate between 33% and 45% of pregnant women of which only 10% present for voluntary screening.In response, the initiative provides education and prevention services regarding pediatric AIDS, prevention of mother-to-child transmission and TB, and improves the quality of care and support for people living with AIDS and for TB patients. Pregnant women are encouraged to attend pre- and post-natal care, get screened for TB, be tested for HIV, and receive appropriate care during pregnancy. The project benefits 29,500 HIV- and TB-infected people in Ngqushwa municipality. Project activities are implemented in collaboration with Primate's World Relief and Development Fund's local partner, Keiskamma Trust (KT)./ Le projet vise à améliorer la santé des personnes affectées par le sida, particulièrement les enfants, à réduire le taux d'infection au VIH/sida et à la tuberculose et à augmenter la sensibilisation des communautés vis-à-vis les services de prévention et de traitement.Au moment du lancement du projet, une étude du Human Sciences Research Council indiquait que la prévalence du VIH en Afrique du Sud en était à son taux le plus élevé. Dans la région visée par le projet, soit le district de Ngqushwa de la province du Cap-Oriental, les taux de prévalence du VIH fluctuent entre 33 % et 45 % chez les femmes enceintes, dont seulement 10 % ont participé volontairement au dépistage.En réponse, l'initiative offre des services d'éducation et de prévention du sida chez les enfants et de prévention de la transmission du VIH de la mère à son enfant. Elle offre également des services de prévention de la tuberculose et améliore la qualité des soins et l'appui aux personnes vivant avec le sida et aux patients de la tuberculose. Les femmes enceintes sont encouragées à se prévaloir des soins pré et post natals, à être testées pour le VIH, et à recevoir des soins appropriés durant la grossesse. Le projet profite à 29 500 personnes infectés par le VIH/sida ou atteint de tuberculose dans la municipalité de Ngqushwa. Les activités du projet sont mises en oeuvre en collaboration avec le partenaire local du Fonds du Primat pour le secours et le développement mondial, le Keiskamma Trust (KT).

Le projet appuie six initiatives mises en oeuvre par des organisations et institutions régionales en vue de freiner la propagation du VIH/sida et de réduire les effets de celui-ci : i) Le Secrétariat de la Communauté des Caraïbes (CARICOM) améliore la coordination régionale des activités de lutte contre le VIH/sida et appuie les États membres aux prises avec des problèmes d ordre juridique et moral et sur le plan des droits de la personne que pose l épidémie. ii) Par ailleurs, le Secrétariat de la CARICOM travaille aussi à renforcer les réseaux, les relations et les échanges d information entre les pays de la région, dans le but d assurer une action coordonnée dans la lutte contre l épidémie. iii) L Institut des Caraïbes pour l alimentation et la nutrition contribue à améliorer la qualité de vie des personnes infectées et affectées par le VIH/sida. L Institut concentre ses efforts sur l amélioration de la nutrition et sur la prévention de la transmission du VIH/sida de la mère à l enfant. iv) La Conférence des Églises des Caraïbes aide les Églises et d autres groupes confessionnels à prodiguer soins et assistance aux personnes vivant avec le VIH/sida ainsi qu à sensibiliser leurs membres et la population en général au problème du VIH/sida. v) L Association de planification familiale des Caraïbes s adresse surtout aux jeunes en offrant des activités éduc

This grant will establish a We...st African-driven partnership with Canadian and European collaborators to enhance HIV research and carry out clinical trials in Sénégal (Université Cheikh Anta Diop), the Gambia (Medical Research Council) and Guinea Bissau (Bandim Health Project). The partners will jointly manage and share their cohort and bio-resource data, thereby providing access to an expanded HIV population. The project will involve constructing a unified database, reinforcing the existing laboratory infrastructure, providing training in clinical trial support, and applying social science research in support of intervention preparedness and evaluation. The project will also offer specialized postgraduate training with a view to building African research leadership in the management of clinical trials.

Second only to South Africa in... HIV burden, Nigeria's complex epidemic of HIV viral subtypes is a vital target for HIV vaccine evaluation research. This grant will support a partnership between the Institute of Human Virology-Nigeria (IHV-Nigeria) - a not-for-profit indigenous research organization - and clinical research collaborators from the University of Ottawa to create an internationally certified HIV clinical research site capable of conducting HIV vaccine clinical trials. This will involve capacity building in clinical practice, research ethics, regulation, community mobilization, risk cohort development and clinical research, plus strengthening clinical laboratory facilities to the level of international certification. The idea is to achieve a fully functional clinical trials unit within a well engaged community and robust ethical and regulatory infrastructure.
