Antibiotic resistance (ABR) is a worldwide publichealth concern, with serious health, economic, and so-cietal repercussions. Its emergence is attributed to the se-lective pressure exerted by antib...iotic use in the community, hospitals, veterinary health, agriculture, aquaculture, and the environment. Additionally aggravating the situation is the fact that very few new antibiotics have recently been produced by pharmaceutical companies. It is widely acknowledged that food animals are key reservoirs of antibiotic-resistant bacteria and that antibiotic usage in this population favors the emergence, selection, and spread of resistance among animals and humans, both through zoonoses (infectious diseases trans-mitted between animals and humans) and the food chain.

We can reduce or even reverse antibiotic resistance by using antibiotics more responsibly. But how do we do that and still ensure that patients are treated effectively?WHO has developed a ...tool to help global, regional and national decision-making on which antibiotics to use when. The tool indexes the most effective antibiotics into three groups – ACCESS, WATCH, RESERVE (AWaRe for short). Evidence shows that to optimize use of antibiotics and reduce resistance, countries should increase the proportion of ACCESS antibiotics to correspond to at least 60% of total national consumption.

It is also important to provide for the safe and effective use of veterinary antimicrobial drugs in veterinary medicine by maintaining their efficacy. This document defines the respective responsibilities of authorities and groups involved in the authorization, production, control, distribution and use of veterinary antimicrobials such as the national regulatory authorities, the veterinary pharmaceutical industry, veterinarians, distributors and producers of food-producing animals.

Lorsque des pathogènes deviennent résistants aux agents antimicrobiens, ils peuvent présenter un risque plus important pour la santé découlant d’un éventuel échec thérapeutique, de la diminution de choix thérapeutiques et de la probabilité accrue d’apparition et de gravité de la maladie. Les problèmes en rapport avec la RAM sont liés de façon inhérente à l’usage des antimicrobiens dans tous les types d’environnements, y compris les utilisations humaines et non humaines. Cuando l...os patógenos se hacen resistentes a los agentes antimicrobianos, pueden ser más peligrosos para la salud humana a causa de un posible fracaso de los tratamientos, de la pérdida de opciones para los tratamientos y de la mayor probabilidad y gravedad de las enfermedades. Los problemas relacionados con la RAM están intrínsecamente relacionados con el uso de antimicrobianos en cualquier ámbito, lo que comprende los usos humanos y no humanos. El uso de agentes antimicrobianos en animales/cultivos destinados a la producción de alimentos constituye un factor potencialmente importante de riesgo de selección y propagación a los seres humanos de microorganismos resistentes y determinantes de RAM a partir de dichos cultivos/animales a través del consumo de alimentos.

The purpose of this guidance is to assist WHO Member States, and other stakeholders, in the establishment and development of programmes of integrated surveillance of antimicrobial resistance in foodborne bacteria (i.e., bacteria commonly transmitted by food). In this guidance, “integrated surveill...ance of antimicrobial resistance in foodborne bacteria” is defined as the collection, validation, analyses and reporting of relevant microbiological and epidemiological data on antimicrobial resistance in foodborne bacteria from humans, animals, and food, and on relevant antimicrobial use in humans and animals. Integrated surveillance of antimicrobial resistance in foodborne bacteria therefore includes data from relevant food chain sectors (animals, food and humans) and includes data on both antimicrobial resistance and antimicrobial use. Integrated surveillance of antimicrobial resistance for foodborne bacteria expands on traditional public health surveillance to include multiple elements of the food chain, and to include antimicrobial use data, to better understand the sources of infection and transmission routes.
