Le développement d’une immunité face à un agent pathogène à la suite d’une infection naturelle comporte plusieurs étapes et prend généralement une à deux semaines. Une infection virale entraîne immédiatement une réponse immunit...aire innée, non spécifique, au cours de laquelle les macrophages, les neutrophiles et les cellules dendritiques ralentissent la progression du virus et peuvent même éviter l’apparition de symptômes. Cette réponse non spécifique est suivie d’une réponse adaptative, au cours de laquelle l’organisme produit des anticorps qui se lient spécifiquement au virus. Ces anticorps sont des protéines appelées immunoglobulines. L’organisme produit également des lymphocytes T qui reconnaissent et éliminent les cellules infectées par le virus. Cette réponse adaptative permet parfois d’éliminer le virus de l’organisme et, si elle est suffisamment forte, d’éviter l’aggravation de la maladie ou la réinfection par le même virus. Ce processus est souvent mesuré par la présence d’anticorps dans le sang.

WHO has published the guidance “Clinical management of severe acute respiratory infection (SARI) when COVID-19 disease is suspected” This document is intended for clinicians taking care of hospitalized adult and paediatric patients with severe acute respiratory infection (SARI) whe...n COVID-19 infection is suspected. Optimized supportive care should be provided to ensure the best possible chance for survival of COVID-19 patients as described in the WHO guidance:
1. Management of severe COVID-19 requires oxygen therapy and monitoring. Supplemental oxygen therapy should be given immediately to patients with SARI and respiratory distress, hypoxaemia or shock.
2. Management of critical COVID-19 (acute respiratory distress syndrome (ARDS)) requires advanced oxygen/ventilatory support.

The WHO Global Health Estimates show that nearly half a million deaths (493 471) occurred in the WHO European Region due to violence and injuries in 2016. This represents a decline of 29% from 2000. Injuries account for 5.3% of all deaths and 9.6 of all years of life lost. They are a leading cause o...f death in people aged 15–29 years and the second leading cause of death for young people aged 5–14. The three leading causes of injury deaths are self-directed violence (141 089), falls (83 325) and road-traffic injuries (78 198). Inequalities in injury deaths exist in the Region, with mortality rates 2.4 times higher in males than in females and 1.5 times higher in middle-income compared to high-income countries.

The PPC published here describes the characteristics of new types of insecticide-treated nets (ITNs) to control malaria transmission in areas with insecticide-resist...ant mosquito populations. The document was developed to address the public health need caused by the evolution and spread of insecticide resistance, particularly to pyrethroids. Such resistance threatens the effectiveness of the current standard of malaria vector control in many countries, namely pyrethroid-only long-lasting insecticidal nets (LLINs).

“Preferred product characteristics” (PPCs) are key tools to incentivize and guide the development of urgently needed health products. Some of the vector control interventions deployed in complex emergencies and in response to natural disasters – namely insecticide-treated nets (ITNs) and indoo...r residual spraying (IRS) – have already met identified public health needs in more stable settings; other tools such as insecticide-treated tarpaulins have been specifically designed for this use case. Given the diverse mix of existing and potential new interventions and the considerable gaps in the associated evidence base, this PPC aims to clearly articulate the unmet public health needs for tools designed to control malaria transmission in complex emergencies and in response to natural disasters.
