The world is facing an unprecedented range of emergencies. In reaction to these complex adversities, many people experience considerable distress and impairment, and a minority may even go on to develop mental health conditions. Meanwhile, those with pre-existing mental health conditions may experie...nce a worsening of their condition and are at risk of neglect, abandonment, abuse and lack of access to support. Unfortunately, evidence-based mental health care is often extremely limited in humanitarian settings. In response, the World Health Organization (WHO) and the United Nations High Commissioner for Refugees (UNHCR) published the Mental Health Gap Action Programme (mhGAP) Humanitarian Intervention Guide (mhGAP-HIG) in 2015. This practical tool supports health-care providers in assessing and offering first-line management of mental, neurological and substance use (MNS) conditions in humanitarian emergency settings.

The current report, Stories of change from four countries: Building capacity for integrating mental health care within health services across humanitarian settings, describes efforts in four countries to build evidence-based mental health systems in humanitarian emergency settings using the mhGAP-HIG. This report includes three sections, the first describing the importance of scaling up mental health care in emergency contexts, the second outlining case studies (“stories of change”) to scale up the Mental Health Gap Action Programme (mhGAP) programme in four settings and the third describing lessons learned by stakeholders.

A Organização Pan-Americ...ana da Saúde (OPAS) trabalhará com os Estados Membros para alcançar a meta e objetivo desta agenda: assegurar uma vida saudável e promover o bem-estar para todos, em todas as idades, usando um enfoque sustentável e equitativo que priorize a redução das iniquidades em saúde. A Agenda foi desenvolvido sob a égide da Estratégia Mundial da Organização Mundial da Saúde (OMS) sobre a Saúde, o Meio Ambiente e a Mudança Climática e se baseia nos compromissos estabelecidos na Agenda de Saúde Sustentável para as Américas 2018–2030 e no Plano Estratégico da OPAS 2020–2025. A Agenda foi desenvolvida em consulta com o Grupo Técnico Assessor (GTA), por meio de um processo decisório consensual com os Estados Membros, durante os anos de 2019 e 2020. Para alcançar o Objetivo de Desenvolvimento Sustentável 3, a Agenda enfoca: melhoria do desempenho dos programas e instituições de saúde pública ambiental; promoção de sistemas de saúde ambientalmente resilientes e sustentáveis; e promoção de cidades e comunidades ambientalmente saudáveis e resilientes. A implementação da Agenda deverá ser contextual, com base nas necessidades e realidades de cada país. Ela beneficiará países e territórios ao promover boas práticas de governança; fortalecer as funções de liderança e coordenação do setor da saúde; favorecer ações intersetoriais; focar na prevenção primária; e melhorar as evidências e a comunicação. Facilitará o acesso aos recursos humanos, técnicos e financeiros necessários para abordar os determinantes ambientais da saúde e garantir que a Região esteja totalmente engajada nos processos e acordos globais de saúde, meio ambiente e mudança climática. O objetivo desta Agenda é fortalecer a capacidade dos atores da saúde, tanto no setor da saúde quanto em outros setores, para abordarem e se adaptarem aos determinantes ambientais da saúde (DAS), priorizando as populações que vivem em condições de vulnerabilidade, a fim de atingir o Resultado Intermediário 18 do Plano Estratégico da OPAS 2020–2025, diretamente, e vários outros resultados do Plano, indiretamente. L’objectif des présentes orientations est de renforcer la capacité des établissements de santé à protéger et à améliorer la santé des communautés desservies face à un climat instable et changeant ; et de donner aux établissements de santé les moyens d’assur...er leur durabilité environnementale, grâce à une meilleure utilisation des ressources et à une diminution du rejet de déchets dans l’environnement. En étant résilients au changement climatique et écologiquement viables, les établissements de santé peuvent offrir des soins de qualité et des services plus accessibles, et en contribuant à réduire les coûts des établissements, ils garantissent également la prestation de soins plus abordables. Ils représentent par conséquent un élément important de la couverture sanitaire universelle (CSU).
Le présent document a pour objectif de :
guider les professionnels travaillant dans des structures de soins de santé de manière à ce qu’ils comprennent les risques sanitaires supplémentaires liés au changement climatique et qu’ils s’y préparent efficacement ;
renforcer la capacité à exercer une surveillance efficace des maladies liées au climat ; et à suivre, anticiper, gérer les risques sanitaires associés au changement climatique et à s’y adapter ;
inciter les responsables des établissements de santé à collaborer avec les secteurs déterminants pour la santé (notamment l’eau et l’assainissement, l’énergie, les transports, l’alimentation, l’urbanisme, l’environnement) afin de se préparer aux risques sanitaires supplémentaires posés par le changement climatique grâce à l’adoption d’une approche de résilience, et à promouvoir des pratiques écologiquement durables dans la prestation des services ;
fournir des outils pour aider les responsables des établissements de santé à évaluer leur résilience face aux menaces liées au changement climatique et leur durabilité environnementale, sur la base de l’utilisation appropriée des ressources (en particulier l’eau et l’énergie et les achats durables), et du rejet de matières dangereuses (biologiques, chimiques, radiologiques) dans leur environnement ;

Monitoring financial protection and utilization of health services in Mongolia 2009-2018 is based on national representative household socioeconomic surveys. The study finds that between 2009 and 2018, despite ambitious health reforms, the incidence of catastrophic health spending and impoverishing ...spending at the relative poverty line have increased. These increases were mainly driven by out-of-pocket spending on medicines and inpatient care. In the same period, inequity in access to and utilization of health services remained constant among population groups. Evidence suggests health financing policies need to be further strengthened to make progress towards universal health coverage. Continuous tracking of out-of-pocket payments and service utilization to inform policymaking is needed.

Over the past twenty years, huge efforts made by a broad coalition of stakeholders curbed the last epidemic and brought the disease to the brink of elimination. In this paper, the latest figures on disease occurrence, geographical distribution and control activities are presented. Strong evidence in...dicates that the elimination of sleeping sickness ‘as a public health problem’ by 2020 is well within reach. In particular, fewer than one thousand new cases were reported in 2018, and the area where the risk of infection is estimated as moderate, high or very high has shrunk to less than 200,000 km2. More than half of this area is in the Democratic Republic of the Congo. The interruption of transmission of the gambiense form, targeted by the World Health Organization (WHO) for 2030, will require renewed efforts to tackle a range of expected and unexpected challenges. The rhodesiense form of the disease represents a small part of the overall HAT burden. For this form, the problem of under detection is on the rise and, because of an important animal reservoir, the elimination of disease transmission is not envisioned at this stage.

Children with disabilities are particularly vulnerable in humanitarian settings, yet they are often not able to access the services and protection they need. While multiple factors create these barriers, a major cause is how data about children with disabilities is collected and mapped. Data collect...ion processes often exclude or underrepresent the views of children with disabilities and thier caretakers. When the experiences of children with disabilities and their caretakers are not defined and collected, they become excluded from mainstreamed protective services, which are meant to serve all children. Children with disabilities also do not get the specialised interventions they need.
This guidance note explores how to use qualitative methods to create more robust assessment processes to ensure more effective programming and services for children with disabilities. This note provides promising practices for engaging with children with disabilities and includes sample tools that can be tailored to fit the needs of a particular assessment process. The note also explores the importance of thoughtful cross-sectoral responses so that children with disabilities, and their families, are carefully considered in areas like water, sanitation, and hygiene (WASH), education, health, and nutrition, and therefore receive the holistic support they need and deserve.
This note is intended for a broad audience of relevant child protection actors, including practitioners, coordination groups, researchers, and donors. The information is not limited to one type of humanitarian setting, geographic region, or culture. As a result, the practices and guidance should be adapted to each specific context, ideally in partnership with well-informed local actors, such as representatives from local organisations for persons with disabilities.
