Histoplasmosis is a disease caused by the fungus Histoplasma capsulatum. This disease is highly endemic in some regions of North America, Central America, and South America and is also reported in certain countries of Asia and Africa. It often affects people with impaired immunity, including people ...living with HIV, among whom the most frequent clinical presentation is disseminated histoplasmosis. The symptoms of disseminated histoplasmosis are non-specific and may be indistinguishable from those of other infectious diseases, especially disseminated tuberculosis (TB), thus complicating diagnosis and treatment. Histoplasmosis is one of the most frequent opportunistic infections caused by fungal pathogens among people living with HIV in the Americas and may be responsible for 5–15% of AIDS-related deaths every year in this Region. There are indigenous communities at high risk in every country of the region. At stake are the lives of 45 million people who belong to more than 800 indigenous peoples. Of these, some 100 are spread across several countries, around 200 maintain voluntary isolation or are in initial contact, and nea...rly 500 are at risk of disappearing due to their reduced numbers. Due to their lower immune resistance, their lack of access to hospital care and the increasing penetration of extractive activities in their territories, indigenous communities in voluntary isolation or in initial contact are cause for particular concern.
Far from hospitals and the news cameras, indigenous people in Latin American become ill and die without access to the means needed to protect themselves. They face the pandemic in conditions of social exclusion, racism and discrimination, which highlights historical inequalities and extreme precariousness in basic and health services.

La pandemia ha devenido en una inédita crisis económica y social y, si no se toman medidas urgentes, podría transformarse en una cr...isis alimentaria, humanitaria y política. Tanto para las acciones de control de la pandemia como para las etapas de reactivación y reconstrucción, es imprescindible aumentar la inversión pública en salud hasta alcanzar los parámetros recomendados, de modo de asegurar el fortalecimiento de los sistemas de salud, ampliando y reconfigurando los servicios de salud de calidad, con un enfoque de atención primaria de salud, y abordar de manera inmediata y acelerada las necesidades de salud insatisfechas, reducir inequidades y mejorar las condiciones de acceso a servicios esenciales, incluida la protección financiera.

Recommendations on contraceptive methods used by
women at high risk of HIV
La hemorragia posparto (HPP) se define comúnmente como la pérdida de sangre
de 500 ml o más en el término de 24 horas después del parto. La HPP es la causa
principal de mortalidad materna en países de ingresos bajos y la causa primaria de
casi un cuarto de todas las defunciones maternas en ...todo el mundo. La mayoría de las
muertes provocadas por HPP ocurren durante las primeras 24 horas después del parto:
la mayoría de estas podrían evitarse a través del uso profiláctico de agentes uterotónicos
durante el alumbramiento y mediante un tratamiento oportuno y apropiado.
Mejorar la atención de la salud de las mujeres durante el parto para prevenir y tratar la HPP
representa un paso necesario para el cumplimiento de los Objetivos de Desarrollo del Milenio.
Por lo tanto, el objetivo primario de esta directriz es proporcionar los cimientos para el
desarrollo estratégico de políticas y programas necesarios para garantizar la implementación
sostenible de intervenciones eficaces para reducir la carga global que representa la HPP.
