International commitment to eliminate trachoma as a public health problem worldwide is supported by resolution WHA51.11 of the World Health Assembly .1 Important progress towards this goal has been made by harnessing the mostly informal relationships that exist between partners including Member Stat...es, the World Health Organization (WHO), academic institutions, donors and nongovernmental organizations. Recognizing that work remains to be done and that the 2020 target2 for elimination is rapidly approaching, in February 2015 the WHO Department of Control of Neglected Tropical Diseases convened a group of academic institutions that had for many years helped WHO to implement its mandate on trachoma and to work towards establishing a Network of WHO collaborating centres (WHOCCs) for Trachoma. The previous report of the WHO Expert Committee on this disease
followed a meeting in 1995. Intensive, coordinated efforts against HAT during
the intervening 18 years have resulted in a decrease in incidence to a point at
which elimination is considered feasible. This report provides informati...on about
new diagnostic approaches, new therapeutic regimens and better understanding
of the distribution of the disease with high-quality mapping. The roles of human
and animal reservoirs and the tsetse fly vectors that transmit the parasites are
emphasized. The new information has formed the basis for an integrated strategy
with which it is hoped that elimination of HAT will be achieved. The report also
contains recommendations on the approaches that will lead to elimination of the
disease.

This important research emphasises that many governments are not meeting spending goals, and in many countries the financing gaps are so great that, even if they met the spending goals, expenditure would still fall short of what is needed (expenditure would cover only 64% of estimated future funding... requirements, leaving a gap of around a third of the total US$7·9 billion needed). Quantification of the gaps in domestic spending encourages us to consider whether the amount of funding is appropriate and how much more could and should be done to fight HIV/AIDS. The important question of how governments make allocation choices also comes to the forefront. The aim of this Comment is to draw attention to the many dimensions that contribute to the complexity of these decisions on health resource allocation.
