The manual is written for clinicians working at the district hospital (first-level referral care) who diagnose and manage sick adolescents and adults in resource constrained settings. It aims to support clinical reasoning, and to provide an effective clinical approach and protocols for the managemen...t of common and serious or potentially life-threatening conditions at district hospitals. The target audience thus includes doctors, clinical officers, health officers, and senior nurse practitioners. It has been designed to be applicable in both high and low HIV prevalence settings.
Volume 2 provides a symptom-based approach to clinical care for acute and subacute conditions (including mental health). It provides short summaries of the management of diseases that affect multiple systems of the body, focusing on communicable diseases. It also includes the chronic or long-term management of HIV, TB, alcohol, and substance use disorders.

Diseases previously regarded as relatively easy to manage are much harder to treat as doctors must use “last-resort” drugs that are more costly, take longer to work
and are often unavailable or unaffordable in developing countries. Moreover, regular prescription of antibiotics, random treatment, over the counter sales, inadequate dosage, inclusion of antibiotics in animal feeds and agriculture has contributed equally to emergence of antibiotics resistance as silent epidemic within the country.

This small scale project aimed to optimize antibiotic prescriptions for Urinary Tract Infections (UTI) at Okhaldhunga Community Hospital in Nepal. A review of 18 months data from urine cultures taken in the hospital was completed in 2017. Presentations about antibiotic resistance, the local bacteria...l culture results and possible ways of changing prescription pattern were given for doctors, lab staff and community medical assistants (CMA). 16 months later the prescription rate of antibiotics frequently used for UTIs was followed up, with a 57% reduction of ciprofloxacin consumption!

I've been getting a mysterious headache for some days now. I will be seeing the doctor today." This clearly speaks of a relatio...nship: asking for prayer so as to be relieved of a "mysterious headache", yet going to see a doctor whose job is not to cure mysterious headaches. Even though both areas of human experience have their peculiar and largely unrelated methodologies, this paper argues that any extreme separation of the two is injurious to the teleology of both disciplines in relation to human well-being, which forms the core of spirituality and medicine.

For IST to be effective there is need for utilization of multiple techniques that will lead to transfer of competences (Bluestone et al. 2013). Learning settings should be selected to support relevant and realistic practice so as to increase the efficiency of IST. Alternatives to hotels such as trai...ning institutions and hospitals are viable options for reducing costs of IST as well as being appropriate venues (MOH 2012). There is documented evidence of involvement of academic institutions in providing health leadership capacity building through IST in other countries; for example, in Uganda, IST in leadership for doctors and nurses was done through a blended approach that included didactic and online sessions (Nakanjako et al. 2015). Many are community health workers and midwives, though they can also include local emergency responders/paramedics, pharmacists, nurses, and doctors who serve in... community clinics.
The growing burden of non-communicable diseases (NCDs) on low- and middle-income countries threatens many health systems that are already weakened. In many countries, health systems—and health workers—are not prepared to address the complex nature of NCDs. Health systems are often fragmented, and designed to respond to single episodes of care or long-term prevention and control of infectious diseases.1 Many countries also continue to face shortages and distribution challenges of trained and supported health workers. As most NCDs are multifactorial in origin and are detected later in their evolution, health systems face significant challenges to provide early detection as well as affordable, effective, and timely treatment, particularly in underserved communities.
