(CF) is intended for use by all peripheral health workers in the Region and
is based on the strategy outlined above. This document will focus mainly
on preventing, predicting and detecting outbreaks, and after detection,
investig...ating and containing them.

The EYE strategy is a comprehensive and long-term strategy built on lessons learned that aims at ending yellow fever epidemics by 2026, and consists of three strategic objectives:
protect at-risk populations;
prevent international spread; and
contain outbreaks rapidly.

Argentina's geographical location presents a wide territorial extension throughout diffe...rent latitudes, which determines a wide climatic variety, maintaining the conditions for the enzootic transmission of the yellow fever virus in jungle areas of the northeast of the country bordering Brazil and Paraguay. After controlling the major urban epidemics that hit the port city of Buenos Aires in the 20th century, Argentina maintains foci of enzootic activity in the northeast and isolated human cases for jungle acquisition. The increases in viral activity usually occur in a regional context of epizootics that affect southern Brazil and eastern Paraguay. Argentina has not presented autochthonous cases since 2008. Outbreaks have been sporadic with long intervals without evidence of viral activity.

In the Region of the Americas, between epidemiological week (EW) 1 and EW 52 of 2018, a total of 560,586 cases of dengue were reported (incidence rate of 57.3 cases per 100,000 population), including 336 deaths. Of the total cases, 209,192 (37.3%) were laboratoryconfirmed and 3,535 (0.63%) were clas...sified as severe dengue. Cases reported in 2018 were higher than the total reported in 2017 but lower than the historical average reported in the previous 11 years (2006-2016) (Figure 1). Similarly, the proportion of cases of severe dengue and dengue with warning signs reported in 2018 was higher than the previous two years, but lower than the preceding ten years, and it remains below 1% which was reached in 2015.

The socioeconomic factors and public health inadequacies that facilitated the rapid spread of this infection continue to exist. As it is a new and emerging disease it has not received sufficient coverage yet in the medical curricula of Member States. Specific treatment is not available, and there is... no vaccine for the prevention of chikungunya fever. It has therefore become imperative to develop guidelines, based on the limited clinical experience gathered from managing patients so far, for appropriate management of patients in communities and in health facilities. Experts engaged in managing patients with chikungunya fever in the Region were brought together by the WHO Regional Office for South-East Asia to outline guidelines for managing various situations and stages of the disease.

This guide is a revised edition to the previous version published in 2017.
This updated publication provides programme managers with a user-friendly tool that can: (i) analyse and draw conclusions from historic dengue datasets; (ii) identify appropriate alarm indicators that can predict forthcoming... outbreaks at smaller spatial scales; and (iii) use these results and analyses to build an early warning system to detect dengue outbreaks in real time and respond accordingly. This web-based tool can ensure enhanced, fast and secured communication between national and subnational levels, and standardized utilization of surveillance data.

Methodological field approaches for scientists with a basic background in entomology to prepare and implement a yellow fever entomological assessment during outbreaks
Dengue is a mosquito-borne viral disease that occurs mainly in the tropics and subtropics but has a high potential to spread to new areas. Dengue infections are climate sensitive, so it is important to better understand how changing climate factors affect the potential for geographic spread and futu...re dengue epidemics. Vectorial capacity (VC) describes a vector's propensity to transmit dengue taking into account human, virus, and vector interactions. VC is highly temperature dependent, but most dengue models only take mean temperature values into account. Recent evidence shows that diurnal temperature range (DTR) plays an important role in influencing the behavior of the primary dengue vector Aedes aegypti. In this study, we used relative VC to estimate dengue epidemic potential (DEP) based on the temperature and DTR dependence of the parameters of A. aegypti. We found a strong temperature dependence of DEP; it peaked at a mean temperature of 29.3°C when DTR was 0°C and at 20°C when DTR was 20°C. Increasing average temperatures up to 29°C led to an increased DEP, but temperatures above 29°C reduced DEP. In tropical areas where the mean temperatures are close to 29°C, a small DTR increased DEP while a large DTR reduced it. In cold to temperate or extremely hot climates where the mean temperatures are far from 29°C, increasing DTR was associated with increasing DEP. Incorporating these findings using historical and predicted temperature and DTR over a two hundred year period (1901-2099), we found an increasing trend of global DEP in temperate regions. Small increases in DEP were observed over the last 100 years and large increases are expected by the end of this century in temperate Northern Hemisphere regions using climate change projections. These findings illustrate the importance of including DTR when mapping DEP based on VC.

The waves of yellow fever transmission in the Region of the Americas in 2016–2018 involved the largest number of human and epizootic cases to be reported in several decades. Yellow fever is a serious viral hemorrhagic disease that poses a challenge for health professionals. It requires early recog...nition of signs and symptoms, which are often nonspecific, and it can mimic other acute febrile syndromes. Early detection of suspected or confirmed cases, monitoring of vital signs, life support measures, and treatment of acute kidney failure continue to be the recommended strategies for case management. This report is the result of discussions among experienced specialists in the Americas on the clinical management of yellow fever patients, especially during outbreaks and epidemics, in the context of current medical and scientific evidence and taking into account the technical guidelines already available in the countries of the Region. It includes flowcharts for initially addressing patients with clinical suspicion of yellow fever and proposes a minimum package of laboratory tests that may be useful in contexts where resources are limited. In addition, it considers aspects of health system organization for dealing with yellow fever outbreaks and epidemics.
