Conformément aux recommandations de divers comités de l’Organisation mondiale de la Santé (OMS), le Bureau régional de l’OMS pour l’Afrique a mis en place trois initiatives phares afin de permettre aux États Membres de la Région africaine de se préparer aux urgences de santé publique, ...de les détecter et d’y riposter. Ces programmes sont le fruit de consultations approfondies avec plus de 30 Ministres africains, ainsi qu’avec des acteurs techniques, des partenaires à travers le continent et des institutions régionales telles que le Centre africain de contrôle et de prévention des maladies (CDC-Afrique), dont les contributions ont façonné les activités prioritaires. Le présent rapport est le quatrième rapport de synthèse trimestriel sur les progrès accomplis dans la mise en œuvre des initiatives phares.

Le Ministère de la Santé Publique et de la Prévention en collaboration avec les partenaires techniques et financiers, a procédé à l’élaboration du Plan National de Développement Sanitaire quatrième de la Génération (PNDS4), couvrant la période 2022-2030. Ce PNDS4, contrairement aux PND...S antérieurs couvre une période restante pour la mise en œuvre de la Politique Nationale de Santé 2016-2030. Le PNDS4 est le dernier segment du cycle de la planification stratégique de la mise en œuvre de la Politique Nationale de Santé (PSN) qui est alignée sur la vision du « Tchad que nous voulons » et l’atteinte des Objectifs de Développement Durable.

La atención concedida a la equidad en la Agenda 2030 para el Desarrollo Sostenible obliga a encontrar nuevas formas de ampliar progresivamente los servicios a las poblaciones que no los reciben. Las alianzas satisfactorias entre el sector encargado del suministro de agua, el saneamiento y la higien...e (WASH, por su sigla en inglés) y los programas de lucha contra las enfermedades tropicales desatendidas (ETD) pueden contribuir a lograr esta aspiración. Sin embargo, colaborar para encontrar juntos esas nuevas formas, exige nuevos modos de pensar. En esta edición corregida se presenta un conjunto de herramientas para ayudar a los países y los programas de lucha contra la ETD a colaborar con la comunidad relacionada con las acciones de agua, saneamiento e higiene, y guía en la creación de alianzas, en la movilización de recursos y en el diseño, la aplicación y la evaluación de las intervenciones. Más que una guía de “buenas prácticas”, se trata de un conjunto de herramientas basadas en la experiencia adquirida en la realidad de un programa.

Las olas de transmisión de la fiebre amarilla ocurridas en la Región de las Américas entre el 2016 y el 2018 causaron el mayor número de casos humanos y epizoóticos registrados en varios decenios. La fiebre amarilla es una enfermedad hemorrágica viral grave que representa un desafío para el p...rofesional de salud: exige el reconocimiento temprano de signos y síntomas muchas veces inespecíficos, que pueden parecerse a otros síndromes febriles agudos. La detección temprana de los casos sospechosos o confirmados, el monitoreo de los signos vitales y las medidas de soporte vital, y el tratamiento de la insuficiencia hepática aguda siguen siendo las estrategias recomendadas para el manejo de los casos. El presente informe es el resultado de las deliberaciones sobre la experiencia de expertos de la Región en cuanto al manejo clínico de pacientes con fiebre amarilla, especialmente en brotes y epidemias, mediante la contextualización de esa experiencia en el conjunto actual de la evidencia médico-científica y la consideración de las directrices técnicas ya disponibles en los países de la Región. Presenta flujogramas para la evaluación inicial del paciente con sospecha clínica de fiebre amarilla y sugiere un conjunto mínimo de pruebas de laboratorio que puede ser útil cuando hay pocos recursos; además, detalla aspectos de la organización de los sistemas de salud para enfrentar brotes y epidemias de fiebre amarilla.

While there has been real progress in addressing the burden of disease in the WHO African region, the COVID-19 pandemic has highlighted the link between health, economics and security, as the region saw decades of progress threatened, including positive trends in decreasing inequality. In the Africa...n Region the momentum towards achieving the 2030 SDG disease burden reduction targets (SDG targets 3.3, 3.4 and 3B) has stalled.
The COVID-19 pandemic was also a major threat to gains made, such as the eradication of polio in the region, declared in 2020; reduced numbers of new HIV infections in 2021 compared to 2010; and passing the 2020 milestone of the End TB Strategy, with a 22% reduction in new cases compared with 2015. However, the pandemic also disrupted essential health services in 92% of countries globally, 22.7 million children missed basic immunization, there was an increase in malaria and TB, and global deaths from TB rose for the first time since 2015.

En la reunión de alto nivel de las Naciones Unidas del 2018 se fijó el objetivo de tratar al menos a 40 millones de personas con tuberculosis (TB) entre el 2018 y el 2022; sin embargo, en el 2021 ese objetivo solo se había cumplido en un 66%. Las pruebas diagnósticas son fundamentales para logra...r el objetivo, pero constituyen un eslabón débil en la continuidad de la atención. Las pruebas de diagnóstico rápido recomendadas por la Organización Mundial de la Salud (PDRO) son sumamente precisas, acortan el tiempo hasta el inicio del tratamiento, influyen en resultados importantes para el paciente y son costo-eficaces. Aunque el objetivo para el año 2025 es que todos los pacientes notificados se hagan inicialmente una PDRO, en el 2021 tan solo el 38% se hizo una PDRO como prueba inicial, y se determinó que el acceso a las pruebas diagnósticas era un problema crítico. Una de las principales consecuencias del uso insuficiente de las PDRO es la gran brecha en la detección de la farmacorresistencia. Many times, you will find included in the recipes neglected underutilised indigenous foods that are contextual to the area.... The menus included in this book were successfully used in a food-based rehabilitation and behaviour change approach called, "Positive Deviance/Hearth (PDH)" programme to rehabilitate undernourished children and prevent malnutrition within the communities where they were designed.

Significant progress has been made in the eradication of three priority diseases in the African Region, as a result of extensive collaboration between the Regional Office, WHO country offices and countries. For example, in August 2020, the region was certified free of wild poliovirus. In the area of... neglected tropical diseases, Guinea worm disease is on the verge of eradication, and 12 member states are within reach of being certified as having eradicated yaws by the end of this year.

The Democratic Republic of Timor-Leste has the highest TB incidence rate in the South East Asian Region - 498 per 100,000, which is the seventh highest in the world. In Timor-Leste TB is the eighth most common cause of death.
The salient observations are as follows:
In 2018, 487 (12.5%) of the... 3906 notified TB patients were tested for RR-TB and only 12 lab confirmed RR-TB patients were initiated on standard MDR-TB treatment of 20-months duration, (a 3-fold increase in RR-TB detection compared with 2017). This amounts to treatment coverage of only 17% of 72 estimated MDR/RR-TB among notified TB patients (3906) and 5% of 240 estimated incident MDR-TB patients as compared to 62% treatment coverage of 6300 incident drug sensitive TB patients estimated in TLS. The treatment success in the 2016 annual cohort of 6 MDR-TB patients has been reported at 83%. 80% of TB patients know their HIV Status with around 1% TB-HIV co-infection, 37/ 77 (48%) TB-HIV Co-infection Detected. Of the 387 PLHIV currently alive on ART, exact status on TB screening and testing is unknown. % of PLHIV newly enrolled in HIV care who received IPT is not known.
In 2018, the mortality rate for TB was 94 deaths per 100,000 people (1200 per annum) in TL with an increasing mortality trend (Figure 1), despite TB services being available for nearly two decades.
A survey of catastrophic costs due to TB (2016) highlights that 83% of TB patients are reported to be facing catastrophic costs due to the disease. This is the highest rate in the world.

A global Pandemic, Preparedness and Response (RRR) architecture
A year ago, the second Special Session of the World Health Assembly (WHASS) unanimously agreed to start a diplomatic process for a new binding instrument aimed at ensuring the international community is better prepared for the next health emergencies. The establishment of an Intergovernmental Negoti...ating Body (INB) at the WHO paved the terrain for a proper negotiation, which has started to unfold. The INB will be releasing the “conceptual zero draft” of the treaty text in early December 2022.
