This collection of forms (in MS Excel) should be used in training and included in the outbreak response guidelines. The forms include tally sheets, check lists, household assessment and missed children forms. They can be adapted to specific national conditions.
This field study to measure access to and use of medicines was undertaken in GHANA in May-June 2008. The study assessed information on the socio-economic level of households, and access to and use of medicines for acute and chronic conditions as well as opinions and perceptio...ns about medicines. The survey was conducted in six regions. In each region, six reference public heath care facilities were selected among those participating in the Level II Facility Survey that was carried out in parallel. Within defined distances from each reference public health care facility, households were selected by purposive cluster sampling. This report documents the findings from the Behavioral Surveillance Survey conducted among youuth aged 15-24 in Rwanda in 2009. The 2009 Youth BSS documented HIV knowledge, attitudes, and behaviors (KAB) among youth in Rwanda. The data provided a cross-sectional look at the current HIV KAB among yo...uth, and allowed for changes over time to be detected when analyzing these data against the results of the 2006 Youth BSS.
