Buruli ulcer caused by Mycobacterium ulcerans is a neglected tropical disease characterized by extensive ulceration involving predominantly the upper and lower limbs of patients. The disease is common in rural tropical communities in West and Central Africa, where access to proper health care is lim...ited. Pathogenesis of the characteristic painless ulcers is linked to the elaboration by M. ulcerans of a lipid toxin called mycolactone that has potent cytopathic, immunosuppressive, and analgesic effects on a host of cells in cutaneous tissues. Mycolactone is known to profoundly inhibit secretion of a plethora of proteins that are essential for wound healing. Even though a combination antibacterial therapy of streptomycin and rifampicin for 8 weeks is effective for treatment, it relies on good and appropriate wound management to prevent secondary bacterial infections and improve healing. Evidence-based interventions for wound care in Buruli ulcer disease are often lacking and have relied on expert advice and recommendations. Surgical interventions are limited to debridement of necrotic tissue and grafting of extensive ulcers, usually after antibiotic therapy. Patients’ rehabilitation is an important component of care to reduce disabilities associated with the disease and proper integration into the community after treatment.

protocols. The aim of using standard treatment protocols is to improve the quality
of clinical care, reduce clinical variability and simplify the treatment options,
particularly in primary health care. Standard treatment protocols can be developed by preparing new national treatment guidelines or by adapting or adopting international guidelines.
The Evidence-based protocols module uses hypertension and diabetes screening
and treatment as an entry point to control cardiovascular risk factors, prevent target organ damage, and reduce premature morbidity and mortality. A comprehensive risk- based approach for integrated management of hypertension, diabetes, and high cholesterol is included in the Risk-based CVD management module.
This module includes clinical practice points and sample protocols for:
1. hypertension detection and treatment
2. type 2 diabetes detection and treatment
3. identifying basic emergencies – care and referral.
HEARTS emphasizes adaptation, dissemination, and use of a standardized set of
simple clinical-management protocols, which should be drug- and dose-specific,
and include a core set of medications. The simpler the protocols and management tools, the more likely they are to be used correctly, and the higher the likelihood that a programme will achieve its goals.

There is an expanding market of no- and low-alcohol beverages (NoLos). However, their effects on global ethanol consumption and public health are still questioned. Policies and regulations about NoLos’ availability, acceptability and affordability are lacking and evidence about their benefits is l...imited. Concerns have been raised about the impact of NoLos in reducing alcohol consumption and its associated harm and the possible drawbacks and implications, such as misleading minors, pregnant women, abstainers or those seeking to stop drinking about their actual ethanol content. Further, there are concerns about the implications of NoLo branded products being displayed close to the brand’s main alcoholic beverages and their potential to subtly lead to new occasions of drinking. There is a need to monitor their consumption and impact on aggregated alcohol consumption to understand the public health implications of NoLos. The alcohol by volume content of NoLos must be defined, harmonised and clearly labelled. NoLo marketing needs to be regulated to protect children, pregnant women and those seeking to stop drinking. Fiscal and pricing policies to reduce the affordability of products with higher strengths of ethanol may favour a shift towards lower alcohol strength beverages.

Improving the survival chances and quality of life of women, newborns, and children remains an urgent global challenge. Since 2012, substantial progress has been made in reducing maternal and under-5 deaths, and a only handful of countries are on target to meet the SDG targets in 2030. Yet, 5 millio...n children still die each year under the age of 5, and nearly half of those are newborns less than a month old. Worse still, the global maternal mortality ratio is going in the wrong direction.
A Decade of Progress and Action for the Future will examine the tenacity and innovation that helped us make gains, the lessons learned through monitoring, country-led adaptation and leadership, analysis, and reflection, as well as the approaches we must take to reinvigorate the momentum and global commitment to improving maternal and child survival. Increasing coverage, strengthening the quality of care, and enhancing equity will be tantamount to our global progress.

Les personnels de santé et d’aide à la personne représentent environ 3,4 % de la population active mondiale, notamment quelque 10 % dans les pays à revenu élevé e...t un peu plus de 1 % dans les pays à revenu faible ou intermédiaire. Partout dans le monde, ce secteur est hautement féminisé – les femmes y représentent environ 67 % des travailleurs à l’échelle mondiale – et connaît un degré significatif de ségrégation professionnelle entre femmes et hommes. Cependant, la part des femmes dans la population active du secteur varie selon le degré de développement économique. Dans les pays à revenu faible ou intermédiaire, les femmes représentent 63,8 % de la population active du secteur, contre 75,3 % dans les pays à revenu élevé. Les estimations figurant dans le présent rapport montrent que le haut degré de féminisation du secteur est universel, à la fois au niveau des pays et des régions. Les pays où une plus grande part de femmes travaille dans le secteur n’ont pas nécessairement des dépenses plus élevées en matière de santé et de soins.

Todos los países, culturas y comunidades padecen enfermedades mentales y la Organización Mundial de la Salud (OMS) afirma que el 10% de la carga global de enfermedad está relacionada con... trastornos mentales y neurológicos, así como con el consumo de sustancias. En los países de renta baja y media, más del 75% de las personas con trastornos mentales no reciben ningún tratamiento. En 2020, como resultado de la pandemia mundial, el 93% de los países comunicaron que sus servicios de salud mental se habían detenido o interrumpido (OMS, 2020e). La OMS ha denunciado un incremento del 25% solo en las tasas de depresión y ansiedad durante la pandemia. La Organización para la Cooperación y el Desarrollo Económicos estima que el coste de la depresión y la ansiedad para la economía mundial asciende a 1 billón de dólares estadounidenses al año. Todas las enfermeras tienen una función de prestación de cuidados en materia de salud mental y consumo de sustancias.
