Globally, millions of vulnerable people are experiencing increased hunger and poverty due to droughts, floods, storms and extreme temperature fluctuations as a result of a climatic occurrence: El Niño. This phenomenon is not an individual weather event but a climate pattern which occurs every two t...o seven years and lasts 9-12 months. The 2015/2016 occurrence is one of the most severe in a half-century and the strongest El Niño since 1997/1998 which killed some 21,000 people and caused damage to infrastructure worth US$ 36 billion. The negative consequences of El Niño are foreseen to continue through 2017, particularly in Southern Africa where this event has followed multiple droughts compounding the already fragile situation.

The aim of the Annual Inspection Report is to present findings of public sector health establishments inspected by the OHSC to monitor compliance with the National Core Standards (NCS) during the 2016/2017 financial year in South Africa.
The NCS define fundamentals for quality of care based on six ...dimensions of quality: Acceptability,Safety, Reliability, Equity, Accessibility, and Efficiency.
The NCS structured assessment tools were used to collect data during inspections across the seven domains namely: Patient Rights; Patient Safety, Clinical Governance and Clinical Care; Clinical Support Services; Public Health; Leadership and Governance; Operational Management and Facilities and Infrastructure. A total of 851 routine inspections were conducted with 201 of these facilities re-inspected. Inspection data was captured on District Health Information System (DHIS) data entry forms and exported for analysis to Statistical Analysis Software (SAS) version 9.4.

These events have resulted in lost lives, serious injuries and severe damage to housing and infrastructure. A single hurricane event can affect more than one island, and completely erase the gains of many years of productivity. As a result, these unpredictable hazards create disruption and have an ongoing impact on people’s lives - affecting their mental health and psychosocial well-being.
Although everyone is affected in some way by disaster events, there are a wide range of reactions and feelings that a person can experience. People may feel overwhelmed, confused or very uncertain about what is happening. They can also feel very fearful, anxious, numb or detached. People who feel safe, connected, calm and hopeful, have access to social, physical and emotional support and find ways to help themselves after a disaster, however, will be better able to recover long-term from mental health effects

During the reporting week, the monsoon rains brought 252 mm of rainfall compared to 95 mm during the previous week. The downpour caused 65% of the week’s weather-related incidents (i.e. landslides, wind-storms and floods). Three rain gauges were installed in Chakmarkul (...near Camp 21), Camp 16 and Kutupalong, complementing existing rain gauges in Cox’s Bazar and Teknaf, as well as the Meteorological Station installed by Samaritan Purse in Camp 12. This network of rain gauges provides localized rainfall data at regular intervals throughout the day, which will allow the humanitarian community to better monitor, anticipate and respond to developments within the camps. Relocation of families at risk of landslides and flooding continued; a total of some 200 families have already moved to Camp 20 Extension and more than 100 families to Camp Extension 4. Repair of access roads, culverts, bridges and infrastructure is ongoing with continued attention to preparing for further heavy rains.

Cognitive deficits in schizophrenia can massively impact functionality and quality of life, furthering the importance of cognitive training. Despite the development of the field in Europe and in the United States, no programmes have been developed and tested in developing countries. Different cultur...al backgrounds, budget restrictions, and other difficulties may render treatment packages created in high income countries difficult for adoption by developing nations. We performed a pilot double-blind, randomized, controlled trial in order to investigate the efficacy and feasibility of an attention and memory training programme specially created in
a developing nation. The intervention used simple, widely available materials, required minimal infrastructure, and was conducted in groups.The sample included seventeen stable Brazilians with schizophrenia. Sessions were conducted weekly during five months. The cognitive training group showed significant improvements in inhibitory control and set-shifting over time. Both groups showed improvements in symptoms, processing speed, selective attention, executive function, and long-term visual memory. Improvements were found in the control group in long-term verbal memory and concentration. Our findings reinforce the idea that cognitive training in schizophrenia can be constructed using simple resources and infrastructure, facilitating its adoption by developing countries, and it may improve cognition.

Humanitarian emergencies result in a breakdown of critical health-care services and often make vulnerable communities dependent on external agencies for care. In resource-constrained settings, this may occur against a backdrop of extreme poverty, malnutrition, insecurity, low literacy and poor infra...structure. Under these circumstances, providing food, water and shelter and limiting communicable disease outbreaks become primary concerns. Where effective and safe vaccines are available to mitigate the risk of disease outbreaks, their potential deployment is a key consideration in meeting emergency health needs. Ethical considerations are crucial when deciding on vaccine deployment. Allocation of vaccines in short supply, target groups, delivery strategies, surveillance and research during acute humanitarian emergencies all involve ethical considerations that often arise from the tension between individual and common good. The BRACED Myanmar Alliance was a three-year project aiming to ‘build the resilience of 350,000 people across Myanmar to climate extremes’. The project worked in 7 states, 8 townships and 155 communities. The main impact for project populations was intended to be ‘improved well-being and reduc...ed loss and damage despite climate shocks’, and the project sought to do this by addressing immediate hazard-related needs at community level while encouraging longer-term solutions driven and delivered by communities and subnational and national government.
Community Resilience Assessments (CRAs) were the first activities delivered as part of the project, and the list of community-identified needs became the basis from which local-level project interventions were selected. The selection typically involved an infrastructure requirement (linked to addressing a natural hazard, and sometimes shared between communities); a package of livelihood support (assets and trainings); capacity-building on climate change/resilience topics; and village savings and loans association (VSLA) support. A particular emphasis was placed on women’s empowerment, and leadership trainings and support to women’s self-help groups were provided.

Drawing on its existing structures, supply chains and capacities, the Agency will continue to adapt its int...erventions to respond to ongoing and evolving needs in an effective and agile manner. In Syria, it is expected that the spontaneous return of Palestine refugees from within and outside the country will continue in areas that experience relative calm and where basic infrastructure is rehabilitated, as observed in Sbeineh and Khan Eshieh camps in recent years. In 2020, UNRWA will increase its efforts to rehabilitate its facilities and restore its services in areas of spontaneous return, including in Dera’a, where small scale returns have been observed in 2019.

Senegal has adopted the World Health Organization–Joint United Nations Programme on HIV/AIDS recommended 90-90-90 targets.5 The adoption of this strategy means that the country is expected, by 2020, to have 90% of its population living with HIV diagnosed, 90% of all those diagnosed receiving susta...ined HIV treatment, and 90% of those receiving antiretroviral therapy having suppressed viral load measures.5 To achieve these outcomes, having good clinical laboratory services for diagnosis and follow-up will be critical.6 More specifically, investments will be needed to improve laboratory infrastructure, and to facilitate the access and availability of routine viral load and early infant diagnosis (EID) measures through the implementation of point-of-care (POC) diagnostic platforms along with an efficient and sustainable quality assurance programme.
