After eight years of armed conflict in the east of the country, the Russian Federation started a military offensive in Ukraine on 24 February 2022. The impact of this war has been devastating. It has so far caused 16,200 civilian casualties and destroyed key infrastructure, such as hospitals, school...s, homes, and water installations.
Since the beginning of the conflict, nearly 14 million people - a third of all Ukrainians - have been forced to leave their homes, 90% of them women, children, and elderly people. An estimated 6.2 million people are displaced within Ukraine, while more than 7 million sought safety in Poland, Romania and Moldova or passed through to other destinations in Europe. Some have returned to Ukraine. Another 13 million people are estimated to be stranded in or unable to leave affected areas within the country.

Since the first edition of the CDAC snapshot of communication, community engagement and accountability (CCEA) across the Ukraine response was published in May 2022, a conscious effort has been made by international, national and local responders to ramp up CCEA action. Progress has been made in esta...blishing coordination mechanisms and launching activities, particularly around accountability to affected people (AAP). As CCEA work continues to scale up on the ground, a greater focus and more effort are still needed on community-facing information and two-way communication; this should be accorded a higher priority than at present.

Since the beginning of the Ukraine crisis on 24 February 2022, WHO has supported Government-led efforts and initiatives alongside key partners on the ground. Building on efforts to date, and working alongside Bulgaria’s health authorities to bring added value to existing mechanisms, the WHO Countr...y Office Bulgaria, the WHO Regional Office for Europe and the WHO Health and Migration Programme, in close collaboration with the Ministry of Health of Bulgaria and key partners, undertook a joint review mission to support Bulgaria, with a focus on addressing the health system needs of refugees, migrants, asylum seekers and vulnerable host populations in Bulgaria.
The assessment team developed this report on the key findings and a package of potential interventions based on the opportunities identified and the need for technical support and assistance. The report summarizes concrete areas of work for which collaborations can be further strengthened. The joint review team identified key recommendations for consideration across 8 priority areas. Each humanitarian setting provides distinct opportunities and challenges for actors to coordinate and collaborate at strategic and operational levels. The Health and Protection Joint Operational Framework has been developed to ensure that the health and protection response during humanitarian emerge...ncies can adapt to each environment and is adequately coordinated to ensure high-quality services to meet the needs of affected individuals and at-risk groups based on their situation or vulnerabilities.
The Health and Protection JOF was conceived in 2019 as a collaboration between the Global Health Cluster (GHC), the Global Protection Cluster (GPC) and its Areas of Responsibility (AoRs), the Inter-Agency Standing Committee Reference Group on Mental Health and Psychosocial Support in Emergency Settings (IASC MHPSS RG), and the Inter-Agency Working Group for Reproductive Health in Crisis (IAWG), in addition to key technical experts.
A Steering Group (SG) comprised of representatives from each of these entities guided the framework through a joint global analysis of good practices, gaps, and barriers to integrated and inter-sectoral response coordination. This included a mixed methods review of policy and practice, a survey of humanitarian experts, multiple case studies, structured stakeholder interviews, and field visits. This exercise produced a zero-draft which was then reviewed by field practitioners in three operational contexts to clarify and fully coordinate its operationally focused lens. Finally, the JOF was reviewed by the SG including via a series of consultations in early 2023 to consolidate the current framework.
