Les périodes périnatale et néonatale constituent des moments critiques dans la vie des mères et des nouveau-nés. En effet, pendant ces périodes, des complications obstétricales, foetales et néonatales surviennent entrainant des mort-nés, des décès néonatals et des possibilités d’incap...acités si l’enfant survit. Les présentes directives nationales de santé des nouveau-nés, définies comme des lignes de conduite données par l’Autorité, en l’occurrence le Ministre de la Santé, visent à prévenir, dépister et prendre en charge ces complications en vue de d’améliorer la qualité des soins et de réduire la morbidité et la mortalité foetales et néonatales, ainsi que les handicaps à moyen ou long terme chez l’enfant. Les directives se focalisent principalement sur la période autour de l’accouchement, les premières heures et les sept (7) premiers jours de vie de l’enfant.

The specific objectives of the plan are to:
- Scale up evidence-based, cost effective interventions through effective strategies within a HSS approach and provide equitable coverage with quality.
- Reduce neonatal mortality by improved home-based newborn ...care, early identification of sick newborns and improved access to institutional newborn care of adequate quality.
- Reduce common childhood illness related mortality (due to pneumonia and diarrhoea in all areas and malaria in endemic areas) by improving key family and community practices, community-based early diagnosis and management and referral care for complicated cases.

2018 monitoring report: current status and strategic priorities
The report sets out the status of women’s, children’s and adolescents’ health, and on health systems and social and environmental determinants. Regional dashboards on 16 key indicators highlight where progress is being made o...r lagging. There is progress overall, but not at the level required to achieve the 2030 goals. There are some areas where progress has stalled or is reversing, namely neonatal mortality, gender inequalities and health in humanitarian settings.

Neonatal mortality is a major challenge in reducing child mortality rates in Nepal. Despite efforts by the Government of Nepal, data from the last three demographic and health surveys show a rise in the contribution of neonatal deaths to infant and child mortality. The Government of Nepal has implem...ented community-based programs that were piloted and then scaled up based on lessons learned. These programs include, but are not limited to ensuring safe motherhood, birth preparedness package, community-based newborn care package, and integrated management of childhood illnesses. Despite the implementation of such programs on a larger scale, their effective coverage is yet to be achieved. Health system challenges included an inadequate policy environment, funding gaps, inadequate procurement, and insufficient supplies of commodities, while human resource management has been found to be impeding service delivery. Such bottlenecks at policy, institutional and service delivery level need to be addressed incorporating health information in decision-making as well as working in partnership with communities to facilitate the utilization of available services.

Nepal is on target to meet the Millennium Development Goals for maternal and child health despite high levels of poverty, poor infrastructure, difficult terrain and recent conflict. Each year, nearly 35000 Nepali children die before their fifth birthday, with almost two-thirds of these deaths occurr...ing in the first month of life, the neonatal period. As part of a multi-country analysis, we examined changes for newborn survival between 2000 and 2010 in terms of mortality, coverage and health system indicators as well as national and donor funding.

The plan is guided by the principles of country-leadership, integration, accountability, equity, human rights, innovation and research. This blue print outlines our commitment as government and stakeholders to repositioning newborn health as we implement approaches that impact on the lives of newborns for improved health outcome.

Standard Treatment Guidelines
The purpose of the review was to assign a cause of death, to ascertain the fac-tors that contributed to the death and to identify any systemic issues that could be addressed to prevent future similar deaths. The perinatal mortality audit process in 2019 provided important insights and evidence-based... recommendations that can be used both to address system errors and barriers and to identify and praise points of strength. The aim is to pro-vide recommendations for better care for mothers and their infants during pregnancy, childbirth and the neonatal period and improve the quality of care provided throughout the health system.
