This publication contribute to ensuring consistent exchange of information that underpins the WMO State of the Climate Reports, Climate Watches, climate ...change studies and other emerging applications.
The purpose of the present guidelines is not to change the practice at the national level. Instead, it provides guidance for generic definitions, which are useful in contributing to WMO State of the Climate reports, climate watches, climate change studies and other emerging applications, including the recently adopted methodology for cataloguing hazardous events (WMO-CHE). These applications require regional and/or international exchange of information on extreme events.

Due to high routine vaccination coverage, overall counts of diphtheria case have significantly declined in the Western Pacific Region recently. However, diphtheria is still prevalent in several countries and areas of the Region and remains a public health issue due to its high case fatality rate.

This Field Guide for Preparedness and Response to Diphtheria Outbreaks in the Western Pacific Region is a reference resource for Member States to develop national guidelines adapted to their local context. Countries may also use this Guide to facilitate outbreak preparedness and public health responses to reduce morbidity and mortality due to diphtheria.

La serovigilancia es una herramienta que complementa los métodos tradicionales de salud pública para la vigilancia de las enfermedades transmisibles y proporciona información valiosa sobre la transmisión de enfermedades en los grupos de la población; por ejemplo, para detectar brechas en la inm...unidad frente a las enfermedades prevenibles mediante vacunación. Esta información es útil para hacer un seguimiento de la exposición de la población a enfermedades como la malaria, las enfermedades infecciosas desatendidas, las enfermedades transmitidas por alimentos, agua y vectores, así como las enfermedades infecciosas emergentes. Como muchas enfermedades infecciosas están o han estado presentes en grupos que viven en entornos donde se superponen diversos factores de riesgo, la serovigilancia integrada facilita las sinergias y optimiza la utilización de los recursos de salud pública. Este conjunto de herramientas se elaboró para facilitar el diseño, la puesta en marcha, el análisis, la interpretación y el uso de los resultados de las encuestas serológicas integradas para reforzar las capacidades de los países con vistas a la eliminación de las enfermedades transmisibles. En la primera parte se describen los conceptos básicos sobre encuestas y vigilancia serológicas, sus usos, ventajas y desafíos, formas de mejorar su eficiencia, así como su potencial para contribuir a la toma de decisiones de salud pública. Posteriormente, se presenta un proceso gradual para la puesta en marcha de la vigilancia serológica integrada basada en encuestas serológicas. Incluye recomendaciones sobre cómo determinar la necesidad y el propósito de recopilar información serológica; el diseño y la metodología de la encuesta; los métodos del laboratorio; las consideraciones prácticas para la realización de encuestas; el análisis e interpretación de los datos y el uso de los resultados para respaldar la toma de decisiones. Su objetivo principal es apoyar a los directores de programas y equipos que participan en el control y eliminación de las enfermedades transmisibles. The Regional Action Framework for Noncommunicable Disease Prevention and Control provides a unified vision of objectives and recommended actions to combat the noncommunicable disease (NCD) epidemic in the Western Pacific Region. Implementation should be supported by cross-sectoral coordination..., sustainable financing, evidence-based policy, and community engagement, tailored to each Member State’s unique context. In doing so, Member States are encouraged to transform a disease treatment-centered “sick system” into a “health system” in which a population’s health and well-being enable socioeconomic development.

Since the last situation report on the multi-country outbreak of cholera was published on 1 June 2023 (covering data reported until 15 of May), no new country reported a cholera outbreak. In total, 24 countries have reported cases since the beginning of 2023. The WHO African Region remains the most ...affected region with 14 countries reporting cholera cases since the beginning of the year. The overall capacity to respond to the multiple and simultaneous outbreaks continues to be strained due to the global lack of resources, including shortages of the Oral Cholera Vaccine (OCV) and cholera supplies, as well as overstretched public health and medical personnel, who are dealing with multiple parallel disease outbreaks and other health emergencies. Based on the large number of outbreaks and their geographic expansion, as well as a lack of vaccines and other resources, WHO continues to assess the risk at global level as very high.

The aim of this Process Guide is to support key stakeholders involved in these processes (such as the Cabinet, Ministries of Finance and Healt...h, the Parliament, citizens, media, and civil society organizations) to reorient budgetary arrangements in order to facilitate the ability of national governments to respond to the COVID-19 pandemic by delivering, therapeutics, diagnostics, and vaccine services to their populations. Reorienting budgetary arrangements positions governments to sustain the capacity to mitigate and respond to COVID-19 while concurrently delivering other essential health services and working towards Universal Health Coverage (UHC). The reorientation process is an opportunity to better align budgetary arrangements to sustain systemic capacity to prevent emerging health threats over the short, medium, and long terms.

In 2017, $37.4 billion of development assistance was provided to low- and middleincome countries to maintain or improve health. This amount is down slightly compared to 2016, and since 2010, development assistance for health (DAH) has grown at an annualized rate of 1.0%. While global development ass...istance for health has seemingly leveled off, global health spending continues to climb, outpacing economic growth in many countries. Total health spending for 2015, the most recent year for which data are available, was estimated to be $9.7 trillion (95% uncertainty interval: 9.7–9.8)*, up 4.7% (3.9–5.6) from the prior year, and accounted for 10% of the world’s total economy. With some sources of health spending growing and other types remaining steady, and with major variations in spending from country to country, it is more important than ever to understand where resources for health come from, where they go, and how they align with health needs. This information is critical for planning and is a necessary catalyst for change as we aim to close the gap on the unfinished agenda of the Millennium Development Goals (MDGs) and move forward toward universal health coverage (UHC) in the Sustainable Development Goals (SDGs) era.

The cholera outbreak has affected 14 countries in the WHO African Region. The climate-induced natural disasters such as cyclone and flooding in the southern African region and drought in the Horn of Africa led to increase in cases of cholera in many of the affected countries. With the rainy season c...ommencement in the west African region there is risk of more cholera outbreaks on the horizon. The trend across the region is being closely monitored and this highlights the need for Member States to enhance readiness, heighten surveillance and institute preventive and control measures in communities and around border crossings to prevent and mitigate cross border infection. Since 1 January 2022, a cumulative number of 213 443 cholera cases has been reported to the WHO Regional Office for Africa (AFRO), including 3 951 deaths with a case fatality ratio (CFR) of 1.9% as of 16 July 2023 (Table 1). Malawi accounts for 28% (58 941) of the total cases and 45% (1 766) of all deaths reported, and together with Cameroon, Democratic Republic of the Congo, Mozambique, and Nigeria contribute to 85% (181 300) of the overall caseload and 88% (3 464) of cumulative deaths. In Epidemiologic week 28, six countries Burundi, Cameroon, Ethiopia, Kenya, Malawi and Mozambique reported a total of 667 new cases.
