Ce plan stratégique 2013- 2017 s’inscrit dans la continuité du précédent et a pour but de contribuer à la réduction de la charge de la tuberculose au Sénégal d’ici 2015 conformément aux Objectifs du Millénaire (OMD 6 Cible 8) et aux cibles du partenariat Halte à la tuberculose.
Les i...nterventions porteront, principalement, sur les orientations stratégiques déclinées lors de la revue externe. Elles seront axées sur la poursuite d’une stratégie DOTS de qualité, le renforcement des interventions communautaires et la prise en charge des grands défi s (TB/VIH, TBMR, TB de l’enfant et le contrôle de l’infection). Durant cette période, l’implication des prestataires du privé sera accentuée et des stratégies de communication porteuses d’impact seront menées.
L’atteinte des objectifs de ce plan stratégique nécessitera une importante mobilisation de ressources et un plaidoyer fort sera mené auprès de tous les partenaires du secteur santé et en dehors.

The international community sits at the tipping pointof a post-‐antibiotic era, where common bacterial infections are no longer treatable with the antibiotic armamentarium that exists. In South Africa, t...he identification of the first case of pan-‐resistant Klebsiella pneumoniae(Brink et al, J Clin Microbiol. 2013;51(1):369-‐72) marks a watershed moment and highlights ourtip of the antibiotic resistance ‘iceberg’ in this country. Multi-‐drug resistant (MDR)-‐bacterial infections, predominantly in Gram-‐negative bacteria such as Klebsiella pneumoniae, Escherichia coli, Pseudomonas aeruginosaand Acinetobacter baumanniiare now commonplace in South African hospitals. Whilst a number of expensive new antibiotics for Gram-‐positive bacterial infections have been manufactured recently (some of which are licenced for usein South Africa), no new antibiotics active against Gram-‐negative infections are expected in the next 10-‐15years. Antimicrobial resistant (AMR) organisms are increasing globally, threatening to render existing treatments ineffective against many infectious diseases. In Africa, AMR has already been documented to be a problem for HIV and the pathogens that cause malaria, tuberculosis, typhoid, cholera, meningitis..., gonorrhea, and dysentery. Recognizing the urgent need for action, the World Health Assembly adopted the Global Action Plan on Antimicrobial Resistance in May 2015. In accordance with the Global Action Plan and to meet needs specific to Africa, Africa CDC will establish the Anti-Microbial Resistance Surveillance Network (AMRSNET). AMRSNET is a network of public health institutions and leaders from human and animal health sectors who will collaborate to measure, prevent, and mitigate harms from AMR organisms.

Les organismes résistants aux antimicrobiens (RAM) sont de plus en plus répandus à l'échelle mondiale, menaçant de rendre inefficaces les traitements existants contre les nombreuses maladies infectieuses. Il a déjà été établi qu’en Afrique la résistance aux antimicrobiens est un problè...me pour le VIH et les pathogènes responsables du paludisme, de la tuberculose, de la typhoïde, du choléra, de la méningite, de la gonorrhée et de la dysenterie. Reconnaissant le besoin urgent d'agir, l'Assemblée mondiale de la Santé a adopté, en mai 2015, le Plan d'action mondial pour la résistance aux antimicrobiens. Conformément à ce Plan d'action, et pour répondre aux besoins spécifiques de l'Afrique, les Centres africains pour le contrôle et la prévention des maladies (CDC) ont créé le Réseau de surveillance de la résistance antimicrobienne (AMRSNET). L'AMRSNET est un réseau d'institutions de santé publique et de dirigeants des secteurs de la santé humaine et animale qui collaborent pour mesurer, prévenir et atténuer les dommages causés par les organismes RAM

the fourth edition of the WHO Laboratory Biosafety Manual (LBM4) is now available and the terminology in this guidance was aligned with the LBM4.

The consolidated guidelines are complemented by an operational handbook which is designed to assist with implementation of the WHO recommendations by Member States, technical partners and others who are involved in the management of patients with DR-TB. The WHO Operational Handbook on Tuberculosis, ...Module 4: Treatment - Drug-Resistant Tuberculosis Treatment provides practical guidance on how to put in place the recommendations at the scale needed to achieve national and global impact.
The document provides information on different aspects of care and support for TB patients. In particular, the handbook provides practical guidance on the implementation of the interventions that enable treatment adherence such as social support, treatment administration options, digital adherence technologies. The practical guidance also includes models of care for all TB patients, models of care for children and adolescents, integrated care for TB, HIV and comorbidities, engagement of private sector, managing of TB in health emergencies. This new practical handbook also includes two important chapters on health education and counselling, and palliative care for patients with TB.

Considerar uma boa rede de diagnósticos, tipos... de amostras, intervenções e estratégias em cada um dos países e em todas as partes interessadas nacionais, regionais e parceiros contribuirá para apoiar este esforço, melhorar a colaboração e maximizar o investimento nos diagnósticos para um impacto clínico visível.

L’expansion de la mise en œuvre des approches de suivi du traitement par la réalisation de tests de mesure de la charge virale et de tests de diagnostic chez le nourrisson sera indispensable pour garantir la qualité des soins et des traitements ainsi que le succès des programmes. Le fait de ch...oisir le réseau de diagnostic, les types d’échantillons ainsi que les interventions et les stratégies les plus appropriés dans chaque pays et pour chaque partenaire au niveau national et régional permettra de soutenir cet effort, de renforcer la collaboration et d’optimiser les investissements réalisés dans la mise en œuvre des tests en vue d’obtenir des effets manifestes.
