on Sustainable Financing, contained in Appendix 2 of the Working Group’s report to the Seventy-fifth World Health Assembly. As part of the recommendations, the Secretariat was requested to “explore the feasibility of a replenishment mechanism to broaden further the financing base, in consultation with Member States and taking into consideration the Framework of Engagement with Non-State Actors; and to present a report that includes relevant options for Member States to consider, to the Seventy-sixth World Health Assembly, through the 152nd session of the Executive Board and the thirty-seventh meeting of the Programme, Budget and Administration Committee in January 2023” (paragraph 39(f) of Appendix 2 of the Working Group’s report). In response to this request, the Secretariat reviewed the feasibility of a WHO replenishment mechanism in line with the principles set out by the Working Group on Sustainable Financing. It consulted with Member States through the work of the Agile Member States Task Group on strengthening WHO’s budgetary, programmatic and financing governance and benchmarked a set of replenishment mechanisms within and beyond the global health arena. This report outlines the Secretariat’s review and proposals on key elements of a potential WHO replenishment mechanism.

e of CKD and kidney failure are increasing all over the world – and thereby also the need for dialysis. Unfortunately, the prevalence increases most rapidly in lowand middle-income countries. Globally, there are great inequities in access and quality of management of kidney failure. Many low- and middle-income countries cannot meet the increased need for dialysis. If the patients receive dialysis, it might only be for a limited period due to the out-of-pocket expenses. There are global disparities in CKD mortality reflecting the disparities in access to care. Lack of access to dialysis is an important cause of the increased CKD mortality in low- and middle-income countries.

As scientists of the UN’s Intergovernmental Panel on Climate Change meet in Geneva this week to publish their Special Report on Climate Change and Land (August 8), a new report by the development charity Christian Aid shows that climate change is having a disproportionate impact on the food systems of the country’s least responsible for causing the climate crisis.
The IPCC is expected to show how climate change will affect global food supply, spiking prices and reducing nutrition. It is also likely to recommend that countries will need to drastically cut emissions if global food security is to be protected.

The 2022 Global Hunger Index (GHI) brings us face to face with a grim reality. The toxic cocktail of conflict, climate change, and the COVID-19 pandemic had already left millions exposed to food price shocks and vulnerable to further crises. Now the conflict in Ukraine—with its knock-on effects on global supplies of and prices for food, fertilizer, and fuel—is turning a crisis into a catastrophe. But the speed and severity of the global food crisis reflects the fact that millions of people were already living on the precarious edge of hunger—a legacy of past failures to build more just, sustainable, and resilient food systems. This year’s report therefore focuses on food systems transformation and local governance.
According to the 2022 GHI, Hunger is at alarming levels in 5 countries—Central African Republic, Chad, Democratic Republic of the Congo, Madagascar, and Yemen— and is provisionally considered *alarming *in 4 additional countries— Burundi, Somalia, South Sudan, and Syria. In a further 35 countries, hunger is considered serious, based on 2022 GHI scores and provisional designations.
