The scale of West Africa’s Ebola epidemic has been attributed to the weak health systems of affected countries,
their lack of resources, the mobility of communities and their inexperience in dealing with Ebola. This briefing for African Affairs argues that these explanations lack important contex...t. The briefing examines responses to the outbreak and offers a different set of explanations, rooted in the history of the region and the political economy of global health and development. To move past technical discussions of “weak” health systems, it highlights how structural violence has contributed to the epidemic. Both the crisis response and post-Ebola ‘reconstruction’ will be strengthened by acknowledgment of its long term structural underpinnings and from a more collaborative inclusion of local people.

The current guidelines on Integrated Management of Acute Malnutrition (IMAM), addresses the issue of improved management of severe acute malnutrition (SAM), particularly in children under 5 years of age. In the absence of standard protocols, mortality in children admitted to hospital with SAM can ra...nge between 20 -30% with the highest levels of 50-60% among those with oedematous malnutrition. With modern treatment regimens and improved access to treatment, case-fatality rates can be reduced to less than 5%. These provincial guidelines on IMAM in KZN, includes inpatient care protocols on the management of SAM, and outpatient and community outreach components to manage moderate acute malnutrition (MAM) and prevent deterioration to SAM.

DHS Further Analysis Reports No. 90 - In Rwanda, between 2005 and 2010, there have been radical declines in the desired number of children, actual fertility, and child mortality along with a large increase in contraceptive prevalence. This study reviews trends in some of these measures. Multivariate... analyses evaluate the relative importance for
the desired number of children of years of schooling, wealth, urban residence, media exposure, child mortality, and attitudes toward gender equality. Variations in reproductive preferences, the total fertility rate, and unmet need for family planning are mapped for the 30 districts of Rwanda. The explanations for the rapid changes in reproductive attitudes and behavior are clearly related to the concerns of the country, the rapid rate of population growth, and its implications for economic development and reproductive health.
