                                                                Este artículo analiza un grupo particularmente vulnerable a la pandemia de la COVID-2019: las  personas  privadas  de  libertad  (PPLs).  Con  base  en  el  análisis  del  Censo  Carcelario  de  2019,  presentamos  la  situación  actual  de  las  PPLs  y  las  condiciones  de  los  establecimient...os  penitenciarios de Bolivia, con énfasis en los aspectos que impactan en la salud. Posteriormente, identificamos   los   principales   factores   estructurales   detrás   de   la   mayor   vulnerabilidad   epidemiológica  y  sanitaria  de  las  PPLs,  no  solo  a  la  COVID-2019,  sino  a  epidemias  en  general.  Diferentes  instituciones  involucradas  en  el  combate  de  la  pandemia  desarrollaron  recomendaciones  para  su  prevención  y  tratamiento  en  contextos  carcelarios.  Considerando  estas  recomendaciones  y  los  datos  de  la  situación  de  las  PPLs  de  Bolivia,  desarrollamos  propuestas  de  reformas  políticas  y  legales  para  reducir  la  vulnerabilidad  epidemiológica  de  esta población. Argumentamos que, mientras algunas son sencillas de implementar, muchas se  enfrentan  a  obstáculos  estructurales  para  su  ejecución,  demandando  reformas  profundas  en la política carcelaria y el sistema penal.

Elimination means bringing the disease burden down to a very low level. This will lead to a reduction in the source of infection, so that leprosy is likely to disappear naturally as it already h...as in many parts of the world. WHO has defined “elimination” as a prevalence rate of less than 1 case per 10,000 inhabitants.

                                                                WHO Director-General Dr Tedros Adhanom Ghebreyesus has determined that the upsurge of mpox in the Democratic Republic of the Congo (DRC) and a growing number of countries in Africa constitutes a public health emergency of international concern (PHEIC) under the International Health Regulations (2005...) (IHR). Dr Tedros’s declaration came on the advice of an IHR Emergency Committee of independent experts who met earlier in the day to review data presented by experts from WHO and affected countries. The Committee informed the Director-General that it considers the upsurge of mpox to be a PHEIC, with potential to spread further across countries in Africa and possibly outside the continent.

                                                                Financing Global Health 2016: Development Assistance, Public and Private Health Spending for the Pursuit of Universal Health Coverage presents a complete analysis of the resources available for health in 184 countries, with a particular focus on development assistance for health (DAH). DAH was estim...ated to total $37.6 billion in 2016, up 0.1% from 2015. After a decade of rapid growth from 2000 to 2010 (up 11.4% annually), DAH grew at only 1.8% annually between 2010 and 2016. In low-income countries, where much DAH is targeted, DAH made up 34.6% of total health spending in 2016. In upper-middle- and high-income countries, which generally do not receive DAH, DAH accounted for only 0.5% of total health spending. The other 99.5% of health spending – government, prepaid private, and out-of-pocket spending – is the subject of our further analysis.

                                                                This template dossier complements and should be used after fulfilling the criteria and preconditions specified in the Process of validation of elimination of kala-azar as a public health problem in South-East Asia. The national kala-azar programme should be in the consolidation phase of elimination;... that is, the annual incidence of kala-azar in the implementation unit is maintained below 1 case (new plus relapse) per 10 000 population for a minimum of 3 consecutive years.
The template is designed to help national kala-azar elimination programmes prepare a dossier documenting the essential evidence supporting the request to the World Health Organization (WHO) to validate the status of kala-azar elimination as a public health problem in their country. The information presented in this document will help independent assessors understand the national programme’s specific context, achievements and relevant epidemiological data.
The dossier should be organized according to the following sections:
- Description of the country context and health system capabilities
- Historical data and delineation of endemic areas
- Surveillance and elimination activities
- Epidemiological data
- Vector control strategy and activities
- Post-validation surveillance plan
Once the dossier is prepared, it should be examined and duly endorsed by the National Task Force on kala-azar elimination and/or neglected tropical diseases, or a similar body, before submission to WHO.
