This module is part of the WHO series The Immunological Basis for Immunization, which was initially developed in 1993 as a set of eight modules, comprising one module on general immunology and seven modules each devoted to one of the vaccines recommended for the Expanded Programme on Immunization, i....e. vaccines against diphtheria, measles, pertussis, polio, tetanus, tuberculosis and yellow fever. Since then, this series has been updated and extended to include other vaccines of international importance. The main purpose of the modules is to provide national immunization managers and vaccination professionals with an overview of the scientific basis of vaccination against a range of important infectious diseases. The modules developed since 1993 continue to be vaccine-specific, reflecting the biological differences in immune responses to the individual pathogens and the differing strategies employed to create the best possible level of protection that can be provided by vaccination. The modules also serve as a record of the immunological basis for the WHO recommendations on vaccine use, published in the WHO vaccine position papers.*

Après plus de trois ans sans cas, Haïti a signalé le 2 octobre 2022 un ensemble de cas de choléra dans la zone métropolitaine de Port-au-Prince, alors que le pays était sur le point d'être déclaré exempt de choléra.
Cette résurgence du choléra en Haïti survient dans un contexte opér...ationnel complexe, au milieu d'un environnement socio-politique instable marqué par des blocus, des pénuries de carburant, des activités de bandes criminelles et une insécurité galopante. Les troubles civils et le manque d'accès aux communautés touchées aggravent la crise humanitaire complexe et entravent les efforts de réponse d'urgence.

After more than three years without cases, Haiti reported on 2 October 2022 a cluster of cholera cases in the metropolitan area of Port-au-Prince, just as the country was on the verge of being declared cholera- free.
This cholera resurgence in Haiti is happening in a complex operational context, ...amid a volatile socio- political environment marked by blockades, fuel shortages, criminal gang activity and rampant insecurity. Civil unrest and lack of access to the affected communities are deepening the complex humanitarian crisis and hindering emergency response efforts.

Since late August 2022, cases of severe acute watery diarrhoea have been increasingly reported across Syria, concentrated
particularly along the Euphrates river. These were later confirmed to be cholera cases.3 Cholera is a disease caused by
bacteria that can be found in faeces, and spreads throug...h people consuming contaminated water or food. It causes severe
watery diarrhoea and vomiting which lead to dehydration. If treated immediately, less than 1% of cases result in patients
dying. However, if timely treatment is not available, cholera can lead to death within hours in 25 to 50% of cases. The
situation is critical in Syria as the local population is facing a severe water crisis due to drought, falling groundwater levels,
reduced flow in the Euphrates River, and reduced functionality of Alouk water station. REACH has been monitoring
developments in Northeast Syria through regular data collection cycles, remote sensing data, and rapid needs assessments

L'objectif de ce projet pilote est de mitiger l’ampleur et l’impact de flambées de choléra à grande échelle en République démocratique du Congo (RDC). Avec 13 000 cas suspects et 236 décès notifiés depuis le début de l’année 2022, le choléra demeure un problème de santé publique ...en RDC. La mise en œuvre d’un plan d'action anticipatoire, qui consiste en des interventions ciblées dans les domaines de l'eau, de l'assainissement, de l’hygiène et de la santé, sera accompagnée d'un programme d'apprentissage complet visant à comprendre l'efficacité, adaptabilité et la dimension éthique du projet pilote.

Infection prevention and control (IPC) in a CTC/ CTU IPC are all practical measures taken in the healthcare facility to prevent harm caused by infections to patients, health workers and communities.
The main goal of IPC in the cholera response is to
• To reduce transmission of health care-as...sociated infections of cholera and any other infectious disease
• To enhance the safety of staff, patients and visitors
• To enhance the ability of the organization/health care facility to respond to an outbreak
• To reduce the risk of the hospital (health care facility) itself amplifying the outbreak
Water, Sanitation and Hygiene (WASH)
WASH are all measures taken to guarantee environmental hygiene, safe water of all used within the health facility. It encompasses water, sanitation, waste management, cleaning within the health facility which in this case is CTU/C. A complete WASH package in the CTU/CTC reduces the risk of spread of Vibrio cholerae inside and outside the CTC/CTU.
The probability of spreading or acquiring cholera through a CTC/CTU can be highly reduced when proper IPC and WASH measures are respected, followed and monitored. These measures are, in principle, valid in CTC/CTUs and ORPs, although they need to be adapted to the specific characteristics of the facility concerned.

In 2024, the cholera outbreak in the WHO African Region in 2024 has affected 14 countries (Burundi, Cameroon, Comoros, Democratic Republic of the Congo, Ethiopia, Kenya, Malawi, Mozambique, Nigeria, South Africa, United Republic of Tanzania, Uganda, Zambia and Zimbabwe). Five countries – Comoros, ...Democratic Republic of the Congo, Ethiopia, Mozambique and Zimbabwe – are currently categorized as being in acute crisis.
