L’objectif central de cette thèse est d’évaluer, dans des conditions réelles d’implantation, les effets du programme burkinabé de prise en charge communautaire du paludisme sur le recours aux soins de...s enfants fébriles. Les objectifs spécifiques sont : (1) de sonder les perceptions des ASC à l’égard du programme et explorer les facteurs contextuels susceptibles d’affecter leur performance ; (2) d’estimer le recours aux ASC par les enfants fébriles et identifier ses déterminants ; (3) de mesurer, auprès des enfants fébriles, le changement des pratiques de recours aux soins induit par l’introduction d’une intervention concomitante – la gratuité des soins dans les centres de santé.

Theoretical frameworks chosen for this study are Mark Salmon White’s construct for public health nursing and Tannahill’s model for health promotion. Both models focus on promoting and contributing to the health and well-being of the public. The Ghanaian Cabinet approved the antimicrobial resistance (AMR)Policy and Implementation plan(hereafter referred to as the national action plan or NAP)in December 2017, whilst the country case study was in progress. This has set in motion the implementation phase for Ghana, which is a long awaited... event since the drafting of the Policy started in 2011. This case study, whilst limited in its ability to interact with all stakeholders, has identified entrypoints within the operational divisions of Ghana Health Services,as potential areas where the AMR policy platform may seek to embed AMR activities. Much work has already been done within Ghana to identify the key entrypoints within the various ministries and government agencieswhere AMR can be incorporated. These stakeholders already form part of the AMR Policy Platform which is the governance structure for AMR and have been participating actively in the development of the AMR Policy and NAP activities formulation.

La elaboración, mejora y ejecución de las políticas y estrategias nacionales de calidad son una prioridad para los países que se esfuerzan por mejorar el desempeño de sus sistemas de sal...ud. El creciente impulso hacia la cobertura universal de salud se acompaña de la correspondiente conciencia de que un mejor acceso requiere esfuerzos orientados a mejorar la calidad de los servicios de salud para lograr las mejoras deseadas en los resultados en materia de salud. Los países están solicitando asesoramiento para orientar sus esfuerzos en relación con las políticas y estrategias nacionales de calidad. Están adoptando diversos enfoques con múltiples puntos de entrada para mejorar la calidad, y muchos de ellos también están centrando su atención en el nivel subnacional como un ámbito de acción. Sin embargo, el objetivo sigue siendo el mismo: aumentar la calidad de la atención de salud como un punto de entrada crucial para el fortalecimiento de los sistemas de salud, y en último término lograr mejorar la salud de la población.

This policy brief has been developed in response to the contemporary challenge of antibiotic resistance (ABR). ABR poses a formidable threat to global health and sustainable development. It is now increasingly recognized that the systematic neglect of cultural factors is one of the biggest obs...tacles to achieving better health outcomes and better standards of living worldwide. Using a cultural contexts of health approach, the policy brief explores the centrality of culture to the challenge of ABR. The brief examines how the prescription and use of antibacterial medicines, the transmission of resistance, and the regulation and funding of research are influenced by cultural, social and commercial, as well as biological and technological factors. The brief moves beyond the ready equation of culture with individual behaviours and demonstrates how culture serve as an enabler of health and provide new possibilities for change.
