Albania has a low prevalence o...f HIV and AIDS. However, data show an upward trend of new diagnoses and suggest a larger number of undiagnosed cases. As of June 2008, the National HIV/AIDS Program reported a total of 324 HIV infections, with 48 new cases midway through the year. The Institute of Public Health implements Albania's response to HIV and AIDS under a national strategy for the period 2004 to 2010. The program supported by this Round 5 grant aims to maintain low HIV prevalence among all vulnerable groups in Albania – including Roma,female sex workers, men who have sex with men and injecting drug users – and to provide care, support and treatment for people living with HIV and AIDS. The program focuses on harm reduction, drug substitution treatment and HIV and AIDS education for injecting drug users in prisons.

HIV infection in Argentina is ...mostly concentrated in most at-risk populations (MARPs) such as gay men and other men who have sex with men (MSM), transgender people, sex workers (SWs) and injecting drug users (IDUs). The Program focuses on these groups, supporting and strengthening the national response to the epidemic. The program aims to strengthen preventive activities aimed at MARPs; to produce quality data and studies about most at-risk groups; to support the most at-risk populations living with HIV (including partners and/or family members) and to improve the health care services. The goal of this Program is to reduce the HIV incidence among gay men and other MSM, transgender people, SWs and IDUs; to reduce stigma and discrimination because of sexual orientation, gender identity and HIV; and to promote universal access to integral health care and better quality of life for gay men and other MSM, transgender people, IDUs and SWs living with HIV, their partners and family members.

The Asia- Pacific and the Glob...al Fund- An Aidspan Regional Report (the report) will provide a concise yet comprehensive portrait of the Asia Pacific region, examining the underlying public health, human rights and vulnerability issues that could challenge or compromise the effectiveness of Global Fund-supported programming. The analysis will review the major fiscal constraints and health system capacity issues that underlie many of the region's problems, mapping those issues and threats as closely as is possible to the Global Fund supported programs. An accompanying short film will be developed which will raise awareness of key challenges faced in ensuring Global Fund grants are spent as efficiently and effectively as possible in the Pacific region in a visually striking and intellectually engaging way. The short film will bring to life the human story behind problems of accountability and transparency in Papua New Guinea, adding impact to Aidspan's written report and engaging a wide audience with the issues at stake.

Benin is a country with a gene...ral HIV epidemic. At the end of 2007, UNAIDS estimated HIV prevalence in adult population at 1.2 percent with a significant gender disparity of 1.5 percent among women and 0.8 among men. These estimates did not consider the appearance of new determinants which favor the propagation of infection such as sex workers and their clients, men who have sex with men (MSM) and injecting drug users (IDUs). The Program was developed based on the lessons learned from Round 2 and Round 5 of Global Fund grants and aims to ensure that objectives of the National Strategic Framework for the fight against AIDS are met by 2015. It encompasses achieving universal access to the prevention, treatment, care and support of persons infected and affected by HIV, through strengthening health and community systems. The Program consolidates Round 5 and Round 9 HIV grants and is implemented by three Principal Recipients.

In August 2014, in response th...e outbreak of Ebola Viral Disease (EVD) in West Africa, Ghana developed its National Preparedness and Response Plan for the Prevention and Control of EVD. Ghana is at a particular risk for the spread of EVD due to its proximity to affected countries, high-volume traffic between it and affected countries, frequent land, sea and border crossings, and Ghanaian troops stationed in Liberia, one of the affected countries. The objectives of the National Plan focus on themes such as early detection, diagnostic capacity, public information and education, case management, protection of healthcare workers, bolstering prevention and control capacities and coordination. Community understanding and engagement is a fundamental aspect of all Ebola control strategies. Australia's contribution of $250,000 will contribute to the public information, education and social mobilisation component of Ghana's EVD Preparedness and Response Plan. Australia's funding will go to UNICEF, which is taking lead in rolling out prevention activities. It will contribute to developing and disseminating communications materials, communicating health messages in schools and communities and engagement and dialogue through IT-based platforms such as media, cell-phone messaging and an Ebola Hotline.

Chad ranks among the least dev...eloped countries. A 2003 epidemiological study estimated HIV prevalence at around 8 percent of the adult population, with 200,000 individuals infected with HIV. The program supported by this grant aims to decentralize services through coordination with the National AIDS Committee A 'minimum activity package' for prevention, health care and treatment will also be set up in 57 health care districts according to criteria based on the characteristics of the epidemic, the health care chart, the possibility of progression for the health care system, the demographic and cultural context, and the geographic disparities of Chad. The program targets women living in vulnerable situations; people living with HIV; young people; orphans and other vulnerable children; people who sell sex; and mobile groups and nomads. It will focus on behavior change communication and community outreach and on strengthening managerial and organizational capacities of community-based organizations and networks of people living with HIV.

Somalia is a fragile state wit...h a government that has been unable to resolve the ongoing conflict and has mostly failed to provide the conditions for security education employment and realization of human rights. Somalia?s living standards remain still among the lowest in the world. In Somalia women are particularly vulnerable to discrimination poverty and restrictions on education and employment. Serious human rights abuses that women face include genital mutilation and sexual violence.African Care?s and its partner organisation?s Banadir Women Development Organization (BWDO) project aims to empower disadvantaged women in Banadir and Lower Shabelle areas by vocational business health and human rights education. Furthermore it strives to strengthen the local civil society actors by offering trainings in order for them to work more efficiently for women?s rights and empowerment. The purpose of the project is in accordance with the cross cutting objectives of Finland s development cooperation to promote gender equality and women s human rights and to reduce inequality.The project is based on the previous project of African Care and BWDO where altogether 265 women participated in a 4-month education programme on handicraft and business skills in Yasmin women?s resource centerin 2013 and 2014.Vocational training includes courses and workshops e.g. on building smokeless stoves ceramics shoe and mat making and laundry business. The business skills courses include e.g. bookkeeping advertising and IT-skills. The continuation project will further develop the old education programme by providing courses on edible oil processing and Moringa tree planting. In addition the continuation project pays more attention on capacity building for the civil society organisations. The project provides trainings e.g. on project management and advocacy work.

The HIV epidemic in Cuba has b...een characterized by its slow, urban growth and low prevalence of 0.1 percent. This grant funds the expansion and decentralization of prevention as well as integrated care and support activities to prevent new infections and decrease AIDS-related sickness and death in Cuba. The program supported by this grant targets adolescents and laborers, including traveling construction workers. The program aims to strengthen the municipal health care system by addressing nutrition, and participation of HIV-positive women in prevention. In addition, grant funds are being used to acquire medical and laboratory equipment, reagents and antiretroviral drugs to expand the epidemiological vigilance network and to finance training programs.

The program supported by this grant aims to reduce the incidence of HIV and AIDS in Swaziland and mitigate the impact on infected and affected individuals, families and communities. Key delivery areas under the program include developing infrastructure and building capacity to scale up HIV testing, prevention of mother-to-child transmission of HIV and antiretroviral treatment services; providing care and support particularly through palliative and home-based care; and assisting orphaned and vulnerable children by supporting communities to attain food security.

The overall goal of the program supported by this grant is to contribute to the ongoing scale-up of access to HIV services under the 2008-2012 National Strategic Plan to fight HIV and AIDS. The program focuses on reducing both the number of new HIV infections among the most vulnerable and high-risk populations, and the impact of HIV among those most affected. Strategies include expanding national coverage of services for people living with HIV, including positive prevention services that enable them to protect their own health and avoid transmitting the virus to others, and psychosocial and nutritional care. The program will also strengthen operational capacities for services to prevent mother-to-child transmission of HIV; provide medical and nutritional care of infants under 18 months; distribute food and school vouchers to children; and ensure diagnosis and treatment of sexually transmitted infections. The activities supported by this grant continue and build on those funded by the Round 3 grant.

1.Background and Summary: Bas...ed on the results of the National Sentinel Surveillance (2006) it was estimated that 2.8% of pregnant women in The Gambia are HIV positive. HIV sentinel surveillance is the only data source in The Gambia, which enables follow up of the evolution of the epidemic. The Gambia is a low income country (155 out of 177 in Human Development Index 2007) with 60% of people living on less than 1 US Dollar a day. Thus, this Program aims to halt and reverse the spread of HIV/AIDS by increasing universal access to an elevated system of health care. To reach this goal, behavior change communication (BCC) and community prevention will be a key factor. Indeed, despite the major achievement in increasing knowledge levels through HIV/AIDS prevention programs, there are still major misconceptions as evidenced in the 2005 Behavior Surveillance Survey (BSS) and Multi-Indicator Cluster Survey III (MICS3 - 2005/6). Stigma and discrimination continue to affect uptake of Antiretroviral Therapy (ART) in the country. Stigma engenders fear, anger, and suspicion and militates against access and utilization of treatment, care and support of People Living with HIV/AIDS (PLWHA) and their families. Another significant challenge to reach PLWHA in The Gambia is the inadequate number of skilled staff and the high attrition rate among the health workers. Additionally, infrastructure, equipment and logistics available in the health sector are inadequate and maintenance activities are infrequently performed. Given this country context, the Country Coordinating Mechanism (CCM) proposed a dual-track financed Program including a Health System Strengthening component. This Principal Recipient, will focus on the treatment of AIDS and will work in collaboration with the Ministry of Health, and the University of the Gambia to reinforce the Health System while a second Principal Recipient, at the time of signature of this Agreement Action Aid The Gambia, will mainly focus on mass media and community prevention and on the home-based care. 2. Goal: To contribute to the halt and reverse the prevalence of HIV infection by providing universal access to high standards of prevention, treatment, care, and support. 3.Target Group/Beneficiaries: • PLWHA; • Pregnant women and children; • Orphans and vulnerable children (OVC); • Students; and • Commercial sex workers (CSW). 4. Strategies: • Refurbish of existing sites and establish more sites; • Establish of youth friendly Volunteer Counseling and Testing (VCT) centre in each of the seven Regions; • Encourage private sector participation in the provision of ART; • Ensure greater involvement of the private sector, both for profit and not-for-profit to prevent mother to child transmission; and • Prevent and treat opportunistic infections. 5. Planned Activities: • Consolidate existing 34 sites and establish new 50 VCT sites both public and private; • Scale up community outreach VCT services; • Scale up community outreach PMT

Ghana has a generalized HIV ep...idemic with pockets of high prevalence among most-at-risk populations, namely men having sex with men and their female partners, female sex workers, prison inmates, most-at-risk youth (15-24), workers (miners, uniformed services, transport workers), and vulnerable groups such as young boys and girls and female porters, as well as population in some geographic areas which are disproportionally affected by the epidemic. The prevalence rate has remained stable and below 5% over the past 16 years. The current national prevalence is 1.9% and prevalence among pregnant women is 2.6% (2007 sentinel surveillance report: National AIDS Control Program/Ghana Health Services). This Program addresses gaps in the national response such as 1) superficial understanding of HIV in many segments of the population including youth, and an inability to move people from general knowledge and awareness of risk to actually adopting safer behaviors; 2) insufficient targeting of most-at-risk populations (MARPS) and vulnerable groups; 3) slow integration of HIV/AIDS and Sexual Reproductive Health (SRH); 4) shortfalls in institutional and community capacities to rapidly scale-up comprehensive HIV services. Recognizing that clients of most-at-risk populations serve as a bridge between most-at-risk populations and general population in HIV transmission, this Program combines the targeting of interventions to MARPS and vulnerable groups and the targeting of the general population. As part of health system strengthening measures directly related to HIV services, this Program targets blood safety, Prevention of Mother-to-Child Transmission (PMTCT), early infant diagnosis, and integration of HIV and Sexually Transmitted Infections (STI) services. As part of strengthening national health system in its response to HIV epidemic and emphasize the need of stronger partnerships between the public health sector, civil society and private sector, this Program will be implemented by multiple Principal Recipients: Ministry of Health/Ghana Health Services and Ghana AIDS Commission (public sector), Planned Parenthood Association of Ghana (civil society), Adventist Development and Relief Agency of Ghana (faith-based organization), and Ghana Employers' Association (private sector) which will become Principal Recipient in Phase 2 of the Program.

The health sector faces serious difficulties due to the insufficient and uneven distribution of health professionals and the inadequacy of financial resources and of available clinical techniques. The HIV and AIDS pandemic has emerged as a public health issue in the country. The program supported by this grant aims to decrease trends of HIV infection in the general population; improve living conditions for people living with HIV and AIDS; provide complete and reliable strategic information on the trends of the epidemic to decision making and resource allocation; and involve all HIV and AIDS partners in the country to obtain a quality national response to the epidemic. The program targets people living with HIV and AIDS, pregnant women, orphans and vulnerable children and the general population.

The HIV epidemic in Guyana is ...generalized, having spread beyond specific high-risk groups into the general population. Heterosexual contact is the major exposure category for HIV infection, and AIDS-related illness is one of the leading cause of death among the 20-49-year-olds, thus affecting the most productive age groups in society. The program financed by this grant supports the expansion of multisectoral efforts outlined in the National Strategic Plan for HIV/AIDS. The program seeks to reduce the spread of HIV and HIV-induced morbidity and mortality, and to mitigate the social and economic impact of the epidemic in the country. Funded activities include the provision of antiretroviral therapy and voluntary counseling and testing, support services for orphans and other vulnerable children (OVC), and condom distribution through social marketing programs and implementation of behavior change campaigns to reduce stigma and discrimination related to HIV and AIDS. Funds are also being used to provide treatment and prophylaxis for opportunistic infections, support services for OVC and home-based care for people living with HIV.The program ended on 31 March 2010 but was approved for funding under the RCC with an approved grant ceiling of $ 18,337,100 (Years 1-3). The Grant was signed during the month of June 2010 with a total grant amount of $34,109,447 ($20,150,872 under Round 3 + $13,958,575 RCC incremental amount). This HIV/AIDS Program funded under the Rolling Continuation Channel aims to scale up the response in prevention, treatment, care and support, based on the achievements reached in the previous five years and by building on and consolidating the strengths of the Round 3 Grant. The Program focuses on the effectiveness of the National HIV/AIDS Strategy 2007-2011 and will continue to expand the treatment and care and support services, while strengthening the capacity to measure, monitor and evaluate the results of these efforts and the underlying epidemiology: Know your epidemic, know your response. In the interest of the sustainability and scale up of treatment, care and support services, the Program will intensify and focus the prevention efforts on the most at risk and vulnerable groups, which include people engaging in concurrent sexual relations and sex work, MSM, mine workers and pregnant women.
