Ethiopia is one of the countri...es most affected by the HIV and AIDS pandemic, with an estimated 2.2 million citizens currently living with HIV. The program supported by this grant focuses on increasing access to treatment care and support services and strengthening preventive services by accelerating information, education and communication activities, with a special focus on women and youth. It aims to increase access to antiretroviral drugs for people living with HIV and AIDS. In addition, voluntary counseling and testing services will be made available throughout Ethiopia and interventions to prevent mother-to-child transmission of HIV will be made more accessible. Care and support of HIV-infected and affected families are also integral components of the program, which targets HIV and AIDS patients, vulnerable communities, health, community and household workers, and public health officials and policy makers.

Ethiopia is one of the countri...es most affected by the HIV and AIDS pandemic, with an estimated 2.2 million citizens currently living with HIV. The program supported by this grant is designed to strengthen and expand the scale-up of social mobilization, care and support programs for people living with HIV and AIDS as well as orphans and children made vulnerable by HIV and AIDS. The program intends to increase coverage and adherence to antiretroviral therapy in 40 districts of four major regional states. The program also aims to enhance community ownership and response to HIV and AIDS with community conversation and other means, and mobilizing local communities to support orphans and vulnerable children. Mitigation of the social and economic impact of HIV and AIDS on people living with the disease and their families through job creation, income-generating activities and the provision of food and nutritional support to people living with HIV and AIDS and their families is another important component of the program.

With a predominantly urbanized... structure accompanied by continuous rural-urban migration and a high rate of commercial sex workers, Gabon is at high risk for a generalized HIV and AIDS epidemic. The program supported by this grant will scale up existing interventions and enable the expansion of a comprehensive response to HIV and Aids to the provincial level of the country. The program has a particular focus on preventing mother-to-child transmission of HIV and the management of people living with HIV and AIDS, including the provision of antiretroviral drugs. In addition, the program is using grant funds to provide psycho-social support to AIDS orphans and to strengthen capacities of nongovernmental organizations and civil society organizations in providing prevention services. The program targets in particular vulnerable groups, including school drop-outs, unmarried mothers, truck drivers and commercial sex workers.

1.Background and Summary: Ba...sed on the results of the National Sentinel Surveillance (2006) it was estimated that 2.8% of pregnant women in The Gambia are HIV positive. HIV sentinel surveillance is the only data source in The Gambia, which enables follow up of the evolution of the epidemic. The Gambia is a low income country (155 out of 177 in Human Development Index 2007) with 60% of people living on less than 1 US Dollar a day. Thus, this Program aims to halt and reverse the spread of HIV/AIDS by increasing universal access to an elevated system of health care. To reach this goal, behavior change communication (BCC) and community prevention will be a key factor. Indeed, despite the major achievement in increasing knowledge levels through HIV/AIDS prevention programs, there are still major misconceptions as evidenced in the 2005 Behavior Surveillance Survey (BSS) and Multi-Indicator Cluster Survey III (MICS3 - 2005/6). Stigma and discrimination continue to affect uptake of Antiretroviral Therapy (ART) in the country. Stigma engenders fear, anger, and suspicion and militates against access and utilization of treatment, care and support of People Living with HIV/AIDS (PLWHA) and their families. Another significant challenge to reach PLWHA in The Gambia is the inadequate number of skilled staff and the high attrition rate among the health workers. Additionally, infrastructure, equipment and logistics available in the health sector are inadequate and maintenance activities are infrequently performed. Given this country context, the Country Coordinating Mechanism (CCM) proposed a dual-track financed Program including a Health System Strengthening component. This Principal Recipient, will focus on the treatment of AIDS and will work in collaboration with the Ministry of Health, and the University of the Gambia to reinforce the Health System while a second Principal Recipient, at the time of signature of this Agreement Action Aid The Gambia, will mainly focus on mass media and community prevention and on the home-based care. 2. Goal: To contribute to the halt and reverse the prevalence of HIV infection by providing universal access to high standards of prevention, treatment, care, and support. 3.Target Group/Beneficiaries: • PLWHA; • Pregnant women and children; • Orphans and vulnerable children (OVC); • Students; and • Commercial sex workers (CSW). 4. Strategies: • Refurbish of existing sites and establish more sites; • Establish of youth friendly Volunteer Counseling and Testing (VCT) centre in each of the seven Regions; • Encourage private sector participation in the provision of ART; • Ensure greater involvement of the private sector, both for profit and not-for-profit to prevent mother to child transmission; and • Prevent and treat opportunistic infections. 5. Planned Activities: • Consolidate existing 34 sites and establish new 50 VCT sites both public and private; • Scale up community outreach VCT services; • Scale up community outreach

Georgia has a low HIV/AIDS pre...valence rate, but high potential for rapid spread of the disease. According to the AIDS centre, in 2009 the prevalence rate among general population was 0.1 percent, and the epidemic is driven by Injecting Drug Users (IDUs) representing 2.1 percent in 2008 according to the BSS report. The country has recently developed the HIV/AIDS National Strategic Plan for 2011 to 2016 with the goal to stabilize epidemic growth, primarily within the Most-At-Risk Populations (MARPs) and improve health outcomes for People Living with HIV (PLHIV) through improved coordination and strengthened advocacy of the national response. The program will ensure that the IDUs have access to the comprehensive package for the prevention, treatment and care, and support. It aims to contribute towards preventing the further spread of HIV through reducing transmission of HIV among IDUs and their sexual partners and achieving improved survival among IDUs living with HIV. The program combines is a Single Stream of Funding with the most recent consolidation of the Round 10 HIV proposal.

Guatemala has the third highes...t HIV prevalence in Central America (0.8 percent) after Belize and Panama. In 2009 62,000 people were living with HIV, double the figure for 2001 according to the UNAIDS Report on the Global AIDS Epidemic 2010. The epidemic is concentrated in groups with behaviors that expose them to a high risk of HIV infection such as men who have sex with men, people who inject drugs, male and female sex workers and their clients, and in other vulnerable groups such as prisoners. This Rolling Continuation Channel grant finances the continuation of the Round 3 program and is implemented by two Principal Recipients – one from the public sector and one from the nongovernmental sector. In addition to scaling up Round 3 activities, the program supported by this grant introduces new strategies to reach more people with fewer resources. The program concentrates on reaching most-at-risk populations and people living with the disease, and on extending prevention services to their sexual partners.

Haiti has an estimated HIV pre...valence among adults of 1.9 percent (UNAIDS Report on the Global AIDS Epidemic 2010). Only 43 percent of the estimated 60,000 people in need of antiretroviral therapy were receiving it in 2009 according to the same report. This grant consolidates two previous Global Fund HIV grants to Haiti (Round 1 Rolling Continuation Channel and Round 5 Phase 2) to provide an integrated approach to HIV services. Services covered by the constituent grants will continue to be funded. Prevention activities include behavior change communication campaigns, promotion of condom use targeting youth, female sex workers and men who have sex with men; voluntary counseling and testing; antiretroviral prophylaxis for pregnant women to prevent mother-to-child transmission of the virus; initiating TB prophylaxis for newly detected seropositive persons; and blood safety precautions. Treatment will focus on providing antiretroviral therapy for people living with advanced HIV infection, and treatment for sexually transmitted infections and opportunistic infections. Care and support activities will focus on community-based interventions.

The Indian government estimate...s that approximately 2.4 million people in India were living with HIV as of 2007. The epidemic is highly varied across states and regions but the states identified as being most affected in 2006 are the four southern states of Andhra Pradesh, Karnataka, Tamil Nadu and Maharashtra, and the northeastern state of Manipur. The program supported by this grant is strengthening human and institutional capacities of the national health system to ensure long-term sustainability of the national AIDS control interventions This grant is implementing a link worker scheme to reach populations with high-risk behaviors in rural areas with education, service referrals and counseling referrals.

Indonesia, which had approxima...tely 193,000 adults living with HIV in 2005, has an overall adult prevalence of about 0.1 percent. However, the epidemic is concentrated among most-at-risk groups and not yet generalized among the population. The program supported by this grant aims to reduce HIV-related illness and death in 33 provinces of Indonesia and to strengthen health and community systems so that performance can be improved. The program targets people who inject drugs and their partners, prisoners, women who sell sex and their clients, men who have sex with men, male-to-female transgender people and people living with HIV and their families. Grant funds will be used for diagnosis and treatment of sexually transmitted infections; methadone maintenance treatment; HIV testing and counseling; opportunistic infection prophylaxis and treatment; antiretroviral therapy; HIV/TB co-infection services; and specific health systems strengthening interventions.

With less than 0.1 percent, HI...V prevalence in Jordan is low with less than 1,000 people in the country living with the disease. The main objective of the program supported by this grant is to scale up prevention and care across the country and to better understand the vulnerabilities of most at risk populations. Program targets include maintaining a low HIV prevalence rate by providing proper care and treatment, and reducing the impact of HIV and AIDS on those people living with the disease and their families. In addition, the program aims to strengthen the role of civil society organizations through capacity building, advocacy and awareness programs.

Kazkahstan is experiencing a c...oncentrated HIV epidemic with as yet low overall prevalence. There is particular risk of new outbreaks among the high number of injecting drug users in the country. The overall goal of the program supported by this Round 2 grant, now in close-out, was to prevent HIV transmission by reducing high-risk behavior among injecting drug users, female sex workers and men who have sex with men, as well as by providing information and knowledge about HIV and AIDS to promote more healthy lifestyles for the nation's young people. The program also aimed to provide medical treatment, care and psychosocial support to individuals living with HIV and AIDS.

Kenya has an adult HIV and AID...S prevalence of 13.5 percent with an estimated 2.2 million people living with HIV and AIDS. It is estimated that 1.5 million people have died from AIDS and that there are more than one million children have been orphaned because of the epidemic. The program supported by this grant aims to contribute to national efforts to curtail the proliferation of the HIV and AIDS epidemic in Kenya by improving the quality of life of people infected by HIV and AIDS and reducing HIV infections. The program focuses on sustaining antiretroviral treatment and scaling up preventive treatment for opportunistic infections. It also seeks to expand prevention services to most-at-risk populations and underserved areas.

Kenya faces both a generalized... and a concentrated HIV epidemic that continues to have a devastating impact on all sectors of society. The HIV epidemic in Kenya affects more females than males and data has also shown that HIV is spreading very rapidly in most-at-risk populations (MARPs) who now account for a third of all the new infections in the country (Kenya Modes of Transmission Study 2008) while heterosexual sex continues to be the main mode of transmission. The goal of this Program is to contribute to the reduction of new infections and AIDS-related morbidity and mortality for an HIV-free society. This Program is a stand-alone single stream of funding (SSF) Round 10 HIV grant and is implemented by two Principal Recipients (PR) under dual-track financing. The current PR will ensure adequate resources to reach Universal Access targets, and implement pragmatic interventions that respond directly to its mixed epidemic – a generalized one that cuts across all social, geographical and economic strata, with key pockets of concentration of risk and transmission among MARPs.

The program supported by this grant aims to maintain the prevalence of HIV to less than one percent among the general population and to improve the quality of life of people living with HIV. The program will focus on key populations most at risk of HIV infection in 152 vulnerable communities in eight regions. Grant funds will be used to provide antiretroviral treatment and prevention and treatment of opportunistic infections; to provide prevention services including counseling, testing and treatment for sexually transmitted infections, HIV counseling and testing, family planning, prevention of mother-to-child transmission and condom distribution services; to organize communication campaigns addressing stigma and discrimination; and to organize targeted advocacy and information campaigns. The program targets commercial sex workers and their clients; men who have sex with men; prisoners; injecting drug users; marginalized youth; people living with HIV; and pregnant women.

Malawi is among the countries ...in Southern Africa at the epicenters of the HIV and AIDS pandemic, with HIV infection predominantly transmitted through heterosexual intercourse. The program supported by this grant aims to bring a balanced approach between prevention, care, support and treatment of HIV and AIDS in Malawi and to reduce the burden of HIV and AIDS-related illnesses and deaths so that they no longer pose a threat to economic growth and political stability. The program consists of assistance to expand voluntary counseling and testing centers, to provide services to HIV-infected mothers to prevention transmission of the disease to their infants; and commencement of antiretroviral therapy to eligible patients. The program is also strengthening home-based care and treatment of opportunistic infections.

At less than one percent, the ...Maldives has one of the lowest rates of HIV prevalence in South and West Asia. The challenge for the country is to ensure that it remains so, despite the increase in high-risk behaviors among some population groups. The overall goal of the program financed by the grant is to support the country's efforts to maintain low HIV prevalence. More broadly, the program recognizes the importance of creating a supportive environment not only to ensure assistance for HIV and AIDS initiatives but also to reduce the stigma and discrimination often facing people living with HIV. Program strategy emphasizes prevention, targeting vulnerable populations, such as young people, injecting drug users, migrants and mobile workers. The objectives identified include increasing awareness and knowledge about sexually transmitted infections and HIV among young people; expanding access and coverage of quality HIV testing and counseling; strengthening the prevention and control of sexually transmitted infections; strengthening health service capacity to provide quality care, support and treatment for people living with HIV; and strengthening the strategic information system for HIV.

In Mauritania, epidemiological... research is limited so that it is difficult to assess the scope and progression of the HIV epidemic. Although the current HIV prevalence in the country is low (0.8 percent in 2005), there is a risk that the epidemic may spread because of social and behavioral factors. The program supported by this grant will strengthen and expand the national response to HIV and AIDS. Grant funds will be used to expand access to antiretroviral therapy and treatment of opportunistic infections for those eligible for treatment; expand preventive activities, particularly through a peer education approach targeting specific vulnerable populations; consolidate comprehensive care for people living with HIV and their families; and promote and strengthen participation of nongovernmental organizations and civil society, including people living with HIV, in the program.

Despite overall good indicator...s in health, the HIV infection rate in Mauritius has risen exponentially over the last few years. The program supported by this grant aims to reduce the rate of new HIV infections among the most-at-risk populations and in the general population and to provide care and support to people living with HIV and AIDS. The program targets injecting drug users, commercial sex workers, men who have sex with men, prisoners, migrants, street children, women, youth and mobile populations. Grant funds are being used to conduct social mobilization advocacy through the National Women's Council by forming HIV and AIDS Advocacy Committees at the village, district and municipal levels; to expand needle exchange and substitution therapy programs through establishing more methadone dispensing units; to train prison staff in HIV and AIDS awareness and promote the establishment of detoxification and substitution therapy; and to introduce prevention of mother-to-child transmission of HIV as part of a broader package of integrated health care with links to family planning services.

The national estimates confirm... the low prevalence of HIV and AIDS among the general population (0.1%). The vulnerable populations which represent 4.5 % of the total Moroccan population (men who have sex with men [MSM], commercial sex workers [SW], injection drug users [IDU]), remain predominately at risk of HIV and AIDS infection with the proportion of 67% of new infections. This Program aims to scale-up and improve the quality of existing services provided to vulnerable population as well as to people living with HIV and AIDS (PLWHA), which integrates HIV prevention, treatment, care and support. This program also includes a component on capacity building of both community and health systems. The goals of the Program are to: reduce the incidence of HIV infection; reduce morbidity and mortality rates by improving the comprehensive care of adult and children PLWHA; and improve the performance of the national response to HIV.

The Pacific Women Initiative (...Vanuatu) initiative provides support to the Vanuatu Women's Centres' Against Violence Against Women program and expands the centre's direct assistance to women and children who suffer violence in Vanuatu. The program aims to achieve the following: strengthen community education and advocacy to achieve universal acceptance of women and children's human rights; and help to put an end to violence and discrimination. The program has five components to achieve its outcomes: legal assistance, counselling and support through the main centre in Port Villa, and regional branches; Community education for social change through selected partnerships, e.g. with schools, faith-based organisations, youth groups and through the media; legal advocacy, lobbying and human rights training to reduce discrimination and improve gender equality in law, policy and practice; and management, training and technical assistance to improve program management and facilities. The total value of this initiative is $6.8 million over 4 years, starting 2012-13.

Mozambique faces a severe HIV/...AIDS epidemic, with prevalence rate of 11.5 percent among adults aged 15 to 49. A lack of basic infrastructure compromises the scale-up for the fight against the epidemic, as only an estimated 50 percent of the population has access to minimum standard of a health centre for every 10,000 people. The country's national AIDS response strategies are based on the National Strategic Plan for HIV and AIDS (PEN III) which identifies Prevention, Advocacy, Treatment, Mitigation, Stigma and Discrimination as the priority areas for intervention. The Round 9 proposal aims to strengthen the capacity of community systems and the private sector to respond to HIV and AIDS, with a goal of reducing national HIV incidence by 57 percent. It also strives to reduce HIV related morbidity and mortality, and the impact of the HIV epidemic on persons living with HIV and AIDS and on orphans and children made vulnerable (OVC) by AIDS. The program intends to increase uptake of HIV counselling and testing, improve utilization, and quality of diagnosis and treatment of Sexually Transmitted Infections (STIs) provided in health facilities. Part of the dual-track financing scheme, the current grant will carry out advocacy and social mobilization activities.

The Australia-Indonesia Partne...rship for Emerging Infectious Diseases (AIP-EID) Human Health Program will be delivered by The World Health Organisation (WHO) Indonesia in close collaboration with the Ministry of health (MOH) and sub-national agencies. Program outcomes will be achieved through four components: 1) Expanding and Strengthening the Early Warning Alert and Response System (EWARS); 2) Strengthening and Ensuring Sustainability of the Outbreak Command Post; 3) Enhancing the quality and sustainability of the Field Epidemiology Training Program; and 4) Strengthening of capacity in the area of zoonosis risk reduction, risk assessment and risk communication and epidemic response. The activities under these components build on work supported by Australia to date and are critical elements of Indonesia's EID management system. Another component serves to provide emergency support to the MOH to respond to EID outbreaks. Expected outcomes include: The MOH will have an increased number of provinces/districts that have strengthened systems for the detection, control and prevention of EIDs and strengthened systems for the response to EIDs; and Indonesia has strengthened human resources for the detection, control and prevention of EIDs. The total value of this initiative is $5.0 million over 5 years, starting 2010-11.
