The Indian government estimate...s that approximately 2.4 million people in India were living with HIV as of 2007. The epidemic is highly varied across states and regions but the states identified as being most affected in 2006 are the four southern states of Andhra Pradesh, Karnataka, Tamil Nadu and Maharashtra, and the northeastern state of Manipur. The program supported by this grant is scaling up care and prevention behavior among pregnant women in the six states with high HIV prevalence. Funded activities included implementation of a comprehensive care package for HIV-infected mothers, their infants and partners to provide preventive treatment with antiretroviral drugs, family planning, voluntary counseling and testing and counseling on infant feeding. In view of strong program performance and demonstrated potential for impact, the grant activities are to be scaled up throughout the country under recently approved additional funding. The grant is being consolidated with IDA-304-G04-C and the testing and counseling component of IDA-607-G11-H.

With less than 0.1 percent, HI...V prevalence in Jordan is low with less than 1,000 people in the country living with the disease. The main objective of the program supported by this grant is to scale up prevention and care across the country and to better understand the vulnerabilities of most at risk populations. Program targets include maintaining a low HIV prevalence rate by providing proper care and treatment, and reducing the impact of HIV and AIDS on those people living with the disease and their families. In addition, the program aims to strengthen the role of civil society organizations through capacity building, advocacy and awareness programs.

The country's Round 7 HIV gran...t supports a program that builds on the achievements of the Round 2 grant by providing people living with HIV and communities affected by the epidemic with equal opportunities to receive prevention, treatment, care and support services, expanding both the scale and scope of HIV and AIDS prevention services provided to the most vulnerable groups (injecting drug users, sex workers, men having sex with men, prisoners and mobile populations). The program supported by this grant seeks to strengthen the capacity of both government and civil society organizations to respond to the national epidemic. Scaled up methadone substitution therapy is a salient feature of the Round 7 grant.

Mongolia is in the very early ...stages of an HIV epidemic. The country has a relatively small population – 2.9 million people – and by the end of 2005 had only 21 reported cases of HIV infection, though the actual number of infections is much higher. The program supported by this grant aims to maintain a low HIV and AIDS prevalence through reduction of vulnerability among most-at-risk group and to control sexually transmitted infections in Mongolia. Grant funds are being used to conduct outreach activities for most-at-risk populations, particularly men who have sex with men and commercial sex workers; promote sexual and reproductive health services; provide antiretroviral treatment; train medical social workers; and create a comprehensive monitoring and evaluation system.

Mozambique faces a severe HIV/...AIDS epidemic, with prevalence rate of 11.5 percent among adults aged 15 to 49. A lack of basic infrastructure compromises the scale-up for the fight against the epidemic, as only an estimated 50 percent of the population has access to minimum standard of a health centre for every 10,000 people. The country's national AIDS response strategies are based on the National Strategic Plan for HIV and AIDS (PEN III) which identifies Prevention, Advocacy, Treatment, Mitigation, Stigma and Discrimination as the priority areas for intervention. Mozambique faces a severe HIV ...and AIDS epidemic, with prevalence rate of 11.5 percent among adults aged 15 to 49. A lack of basic infrastructure compromises the scale-up for the fight against the epidemic, as only an estimated 50 percent of the population has access to minimum standard of a health centre for every 10,000 people. The country's national AIDS response strategies are based on the National Strategic Plan for HIV and AIDS (PEN III) which identifies Prevention, Advocacy, Treatment, Mitigation, Stigma and Discrimination as the priority areas for intervention. Based on the UNAIDS 2008 Repor...t, Namibia has one the world's highest HIV/AIDS infection rates with an adult HIV prevalence rate of 15.3 percent. Being the second most sparsely populated country in the world, providing comprehensive HIV/AIDS services to rural population requires a fully decentralized, community-based approach with strong policies and leadership from the central level. The program aims to reduce the impact of HIV/AIDS through an integrated approach to prevention, treatment, care and support, and mitigation of the impact on the affected populations. Prevention activities include strengthening of community based organizations, social mobilization, condom distribution, voluntary counselling and testing, and prevention of mother-to-child transmission (PMTCT). Treatment activities focus on the roll-out of anti-retroviral treatment (ART), care and support activities on the provision of home-based care and mitigation activities provide support to orphans and vulnerable children (OVC). The Rolling Continuation Channel (RCC) program is implemented by two Principal Recipients and includes Health Systems Strengthening (HSS) component to address cross-cutting health systems weaknesses, which have significant impact on HIV.

Just over half a percent of Ne...pal's adult population is estimated to be HIV-positive. The low prevalence rate masks the imminent threat of an HIV pandemic due to the country's large male migrant population, the lack of voluntary counseling and testing services and the limited capacities of the authorities and health workers to address existing cases. The overall goal of the program supported by this grant is to reduce HIV transmission in Nepal and enhance the quality of life of people living with HIV in the country, focusing on West, Mid-West and Far-West development regions of Nepal. The program's interventions target mobile populations, injecting drug users and men who have sex with men, as well as people who are HIV-positive. Under the program, prevention efforts and community outreach through peer education and interpersonal communication will be scaled up to reduce the stigma related to HIV and to lower the barriers to accessing HIV and AIDS treatment and counseling services. The program will also reach people living with HIV with antiretroviral treatment and opportunistic infection drugs and services.

The HIV and AIDS epidemic in P...eru is concentrated in most-at-risk populations (MARPs), mainly the men who have sex with men (MSM) population, with the highest prevalence in the transsexual population. The program will focus on 8 regions of the country (Lambayeque, La Libertad, Lima, Callao, Ica, Loreto, San Martin and Ucayali) which account for 80% of the total HIV and AIDS cases reported so far. The Program will address different needs of the MARPs sub-populations in Peru, confronting the barriers they face in terms of stigma and discrimination, socio-economical, educational and employment challenges and the prevalence of sexual work. The Program aims to strengthen the capacity of transsexuals and gay/MSM community based organizations (CBOs) by improving their technical and management capacities to be better equipped to exercise community surveillance; support the fight against homophobia, stigma and discrimination; produce quality data and studies about most at risk groups, and to improve access to comprehensive healthcare. The goal is to help MSM populations gain universal access to high quality comprehensive healthcare, intensifying and increasing preventive actions and strengthening their base organizations in an environment that respects their rights and guarantees equal opportunities.

Sri Lanka is currently conside...red to have very low HIV prevalence in both the general population and high-risk subpopulations. Injecting drug use is not reported as a problem in the country. However, the true HIV and AIDS burden may be higher as many cases are likely to remain unreported due to stigma, discrimination and the limited availability of counseling and testing facilities. The program supported by this grant aims to prevent the spread of HIV in the target districts among plantation workers by building human resources for outreach to encourage positive health-seeking behavior, and among school children by increasing their knowledge of HIV transmission and prevention methods. The program is also assisting the government of Sri Lanka to continue to deliver on its commitment to provide antiretroviral therapy for all who need it.

Sri Lanka is currently conside...red to have very low HIV prevalence in both the general population and high-risk subpopulations. At the end of 2008, between 3,000 to 5,000 people were living with HIV, 58 percent of HIV detections were in men and 33 percent detections in children, and 186 AIDS-related deaths were reported. The current grant implements a program under the Dual Track Financing scheme, aiming to maintain the current low prevalence of HIV and Sri Lanka and improve the quality of life of people infected with an affected by HIV. It will outline the prevention activities fundamentally different from those being Global Fund Round 6 resources as it addresses only the unmet needs for the first key strategies of the National HIV/AIDS Strategic Plan from 2007 to 2011. It supports activities to increase the use of the voluntary counselling and testing services (VCT), Behaviour Change Communication (BCC), and the coverage of HIV and AIDS treatment services and community based care for people living with HIV.

1. Background and Summary: Th...e activities carried out under this Program take into consideration Tanzania's experience in scaling up care, treatment and support services. Experience from programs funded under previous Global Fund rounds highlights the need for greater coordination among relevant stakeholders in the HIV/AIDS sphere. Accordingly, this Program intends to support the scale up of HIV/AIDS services by strengthening capacity and coordination among key stakeholders in the public and private sectors, CSOs (Civil Society Organizations), FBOs (Faith Based Organizations) as well as PLHA (People Living with HIV/AIDS) groups. Taken together, support sought for these entities is intended to both form the institutional backbone to facilitate service delivery expansion as well as fill the gap that will emerge when World Bank TMAP funding, which has been catalytic in generating a multi-sectoral response to HIV/AIDS at the community level, comes to an end in September 2009. The absence of funding for securing and sustaining the national response to HIV/AIDS would not only undermine the progress already achieved, but also threaten to create a dangerous funding gap as momentum to tackle HIV/AIDS in Tanzania is increasing. The primary goal of this Program is to assist the Government of Tanzania to secure its response to the HIV/AIDS epidemic by expanding service delivery to the lower tiers of the health system. Two Principal Recipients will work on attaining this goal, namely the Ministry of Finance and AMREF. The Ministry of Finance grant will focus on distribution of drugs and consumables; while AMREF will focus on supporting activities relevant to maintaining and strengthening the system, such as refresher training for health care workers (HCWs), supervision, and mentoring. 2. Goal: Strengthen coordination and implementation capacity of key stakeholders. 3. Target Group/Beneficiaries: • PLHA who are eligible for care and treatment; • Patients on care and treatment; • Pregnant women and their partners; • Infants born to HIV positive mothers; • Sexually active population, especially youth; • Health care workers and PLHA accessing HIV/AIDS services; • Private sector and Individuals and groups served by FBOs, PLHA, CSOs; and • General population of Tanzania. 4. Strategies: • Facilitate the push of care and treatment services to the lower tiers of the health system; • Support the provision of home-based care services to facilities in targeted districts; • Support the provision of resources and supportive structures required to mitigate the adverse effects of chronic illness and opportunistic infection among PLHA; • Support the expansion of coverage of PMTCT services to targeted sites that provide Reproductive and Child Health Services (RCHS); • Strengthen supportive supervision and mentoring networks and the provision of service delivery tools; and • Build the human resources and organizational capacity of key public sector, private sector, and C
