Myanmar has the second highest... number of HIV-positive people in Southeast Asia. Surveillance data from year 2008 on three key populations at higher risk estimate HIV prevalence as 18.4 percent in female sex workers, 28.8 percent in men who have sex with men and 37.5 percent in injecting drug users. The program supported by this grant aims to reduce transmission and HIV related morbidity, mortality, disability and social and economic impact. The target groups are sex workers, men who have sex with men (MSM), injecting drugs users (IDU) and people living with HIV. The program is consistent with the National Strategic Plan (NSP) on HIV and AIDS and its Operational Plan and is prioritized according to epidemiological data and the HIV disease burden. It is implemented by two principal Recipients, based on the Dual Track Financing scheme. The current grant plans to provide packages for the prevention services, including Behaviour Change Communication (BCC) and Sexually Transmitted Infection (STI), prevention and treatment to support sex workers and to provide harm reduction services IDUs and engages in community and home based care services.

One of the lessons learned fro...m the program supported by the Round 2 Multicountry Western Pacific grant was the need to address human capacity in low-resource countries with competing health priorities. The program supported by this Round 7 grant focuses on key capacity challenges faced by the 11 participating small island countries in addressing HIV, The countries supported by this regional grant are: Cook Islands, Marshall Islands, Micronesia (Federated States), Kiribati, Nauru, Niue, Palau, Samoa, Tonga, Tuvalu and Vanuatu. The Solomon Islands receives only antiretroviral (ARV) treatment under the program. The overall goal of the program is to reduce the spread and impact of HIV while embracing people living with HIV. Funded activities include strengthening national-level human resources dedicated to HIV and other sexually transmitted infections; advocacy for national-level legal reforms and the provision of legal advice to decision-makers; provision of test kits and drugs for sexually transmitted infections, including ARVs; campaigns aimed at increasing the availability of free condoms to key populations in all participating countries, as well as condom social marketing initiatives; and counselor training at the national level.

The funding will support:

2) Social Marketing of condoms to reduce HIV infection
3) Increased and consistent use of Family Planning commodities to improve family planning
4) Improving Health systems and accountability mechanisms
5) Improving the efficiency of government health facilities
6) Piloting Health Financing mechanisms to improve access to government health services.
The UK funding is expected to
1) Distribute 6.48 million bed-nets to pregnant women for malaria
2) Treat 522,400 pregnant women for malaria
3) Provide 662,000 women with family planning
4) Distribute 173.5 million condoms
This will avert more than 36,000 malaria deaths, 16,765 HIV infections and 770,800 unwanted pregnancies

Continuation project ?Mental h...ealth for groups with complex needs: A multidisciplinary model combining client centred and community approacher?. The plan is to participate in four years time in the mental health strategic planning process. In previous period the Family Guidance Centers (FGC) treated 316 children who were referred to FGC. One third of them completed the treatment successfully. Among them there were 75 children with special needs. FGCs arranged special education and rehabiliation service for them. All parents express high satisfaction. During the project period concerning the coordination and development issues it started to emerge needs for changes in the setting. To tackle them and step forward organisation has decided to work on a strategic plan for mental health program. However planning has not really started maybe because of big workload but also therefore that the tacit special knowledge accumalated during 20 years is not easy to shape public and divisible. During 4 years time FiPSR will by the support of Ministry for Foreing Affairs accompany NISCVT in the strategy planning. Mental health program works in a landscape which the management and regional sciences call wicked problem. During the work FiPSR supports the basic work in two family guidance centers and rehabilitation of children with special needs in three centers. Estimated number of children is 370-420 of them 60-80 children with special needs.

The program aims to scale up t...he national response to HIV and AIDS in order to advance toward universal access to prevention, diagnosis and treatment, following the priorities identified in the Strategic Plan for the National Response to STIs, HIV and AIDS 2008-2012. In particular, it aims to expand access to the services dealing with prevention, diagnosis and treatment of HIV and AIDS and sexually transmitted infections for the target population, while promoting respect for sexual diversity and the eradication of stigma and discriminatory attitudes. Activities will include the involvement of peer counselors; an increase in community-based organizations' participation in the supply of specific services; improvement in the quality and availability of integral health care for all; increase of access to counseling services and voluntary HIV testing; improvement of access to antiretroviral treatment and prevention services dealing with opportunistic infections. Target populations include: men who have sex with men, female sex workers, transgender sex workers, injecting drug users and people living with HIV. The program aims to expand and consolidate the work started with the implementation of the Round 6 grant.

In the Philippines, HIV preval...ence is low – below 0.1 percent – and less than one percent of sex workers are infected with HIV. The primary mode of transmission is through unprotected sex, and about one-third of diagnosed infections have occurred in returning overseas workers, primarily seafarers and domestic workers. The program supported by this grant focuses on HIV prevention through scaling-up of voluntary counseling and testing services, ensuring blood safety and treatment, care and support for people living with HIV. It also aims to strengthen the health system to establish a supportive environment in the delivery of HIV services. The program targets the clientele of the public health facilities, which includes people most-at-risk and vulnerable to HIV infection.

Compared with other regions in... Africa, public awareness of HIV/AIDS is relatively low in Somalia. The available information on the disease prevalence and risk behavior has been limited. This has left communities vulnerable to the virus through lack of information. This Program aims to scale-up and improve the quality of existing services that integrate HIV prevention, treatment, care and support. In the context of Somali conflict and post-conflict settings, this scale-up will contribute to the achievement of national strategy outcomes, universal access targets, and Millennium Development Goal number 6 . In addition to their contribution to the scale-up, resources in this Program will be invested to enhance access to services in identified higher prevalence locations in the three Somali zones, and among key affected populations through community outreach and mobilization, and the promotion of health seeking behavior. The targeting of services will continue to be guided by accumulating biological and behavioral information on the epidemic generated throughout the life of the Program, and some flexibility is therefore maintained in the planned use of resources. As such, there will be a strong emphasis on information activities, to strengthen evidence-informed programming, but also on coordination activities, to ensure alignment between partners in a multi-sectoral HIV response. Recognizing the conflict and post-conflict contexts that apply to varying degrees in the three Somali zones, this Program also includes well developed dimensions on protection of vulnerable groups, on capacity-building of both community and health systems – including a cross-cutting HSS component and on mainstreaming of HIV response activities into wider emergency, recovery and development plans.

The Western Cape is one of the... nine provinces in South Africa and has a population of 5.3 million, or 10.9% of the population of the country (Statistics South Africa, 2009). The Province has adopted a comprehensive response to the HIV/AIDS epidemic that aims to provide prevention, treatment and care and includes life skills training in schools, condom distribution, management of sexually transmitted infections, voluntary counseling and testing, prevention of mother-to-child transmission (PMTCT), treatment of opportunistic infections, provision of antiretroviral treatment, and provision of home-based and hospice care. This Program started in 2004. In April 2010, the Global Fund Board approved additional six years of funding under the Rolling Continuation Channel (RCC). During the RCC term, the Program will maintain its four objectives: 1) To strengthen the treatment program through the expansion of the provision of antiretroviral treatment; 2) To strengthen the current prevention effort by expanding a peer education program among youth at secondary schools; 3) To strengthen palliative and step-down care services; and 4) To strengthen community-based responses to the epidemic through the development and implementation of local support projects within specified focus areas. The RCC Program will include two new components: (a) strengthening the PMTCT Program in the Province and (b) providing assistance with further development and roll-out of the eKapa patient registration and health information system to district health facilities. The RCC Program will continue to be managed by the Western Cape Provincial Health Department.

DFID funds will help in improved functioning of primary hospitals, child feeding centres, more doctors, nurses and medicines at the rural hospitals. Technical assistance will procure external expertise to help Government of Madhya Pradesh (GoMP) to improve planning, human resource management, improve monitoring and financial management and procurement systems. Illustrative areas of support for health and nutrition services under MPHSRP include:
• Quality improvement of maternal health services
• Promotion of Zn ORS use during childhood diarrhoea
• Family planning counselling and delivery of post partum family planning service
• Malaria prevention: distribution and use of Bed nets in malaria endemic districts and IRS spray in communities with more than 2 API (Annual parasitic index
• Setting up and functioning of state Nutrition Mission and district management and monitoring units
• Setting up of State and divisional Nutrition Resource Centres based in medical colleges or NGOs to help plan, implement and monitor the nutrition actions. The Centres will be the main resource for public health nutrition in the state and will be responsible for undertaking nutrition surveillance.

1. Background and Summary: T...he activities carried out under this Program take into consideration Tanzania's experience in scaling up care, treatment and support services. Experience from programs funded under previous Global Fund rounds highlights the need for greater coordination among relevant stakeholders in the HIV/AIDS sphere. Accordingly, this Program intends to support the scale up of HIV/AIDS services by strengthening capacity and coordination among key stakeholders in the public and private sectors, CSOs (Civil Society Organizations), FBOs (Faith Based Organizations) as well as PLHA (People Living with HIV/AIDS) groups. Taken together, support sought for these entities is intended to both form the institutional backbone to facilitate service delivery expansion as well as fill the gap that will emerge when World Bank TMAP funding, which has been catalytic in generating a multi-sectoral response to HIV/AIDS at the community level, comes to an end in September 2009. The absence of funding for securing and sustaining the national response to HIV/AIDS would not only undermine the progress already achieved, but also threaten to create a dangerous funding gap as momentum to tackle HIV/AIDS in Tanzania is increasing. The primary goal of this Program is to assist the Government of Tanzania to secure its response to the HIV/AIDS epidemic by expanding service delivery to the lower tiers of the health system. Two Principal Recipients will work on attaining this goal, namely the Ministry of Finance and AMREF. The Ministry of Finance grant will focus on distribution of drugs and consumables; while AMREF will focus on supporting activities relevant to maintaining and strengthening the system, such as refresher training for health care workers (HCWs), supervision, and mentoring. 2. Goal: Strengthen coordination and implementation capacity of key stakeholders. 3. Target Group/Beneficiaries: • PLHA who are eligible for care and treatment; • Patients on care and treatment; • Pregnant women and their partners; • Infants born to HIV positive mothers; • Sexually active population, especially youth; • Health care workers and PLHA accessing HIV/AIDS services; • Private sector and Individuals and groups served by FBOs, PLHA, CSOs; and • General population of Tanzania. 4. Strategies: • Facilitate the push of care and treatment services to the lower tiers of the health system; • Support the provision of home-based care services to facilities in targeted districts; • Support the provision of resources and supportive structures required to mitigate the adverse effects of chronic illness and opportunistic infection among PLHA; • Support the expansion of coverage of PMTCT services to targeted sites that provide Reproductive and Child Health Services (RCHS); • Strengthen supportive supervision and mentoring networks and the provision of service delivery tools; and • Build the human resources and organizational capacity of key public sector, private sector,

HIV prevalence in Timor-Leste ...is low, estimated at between 0.3-0.6 percent of the adult population. The program supported by this grant is addressing critical gaps in the development and scale-up of the national response to HIV and AIDS in Timor-Leste. The program's main objectives are to improve and increase prevention, diagnostic and treatment services for HIV and sexually transmitted infections; improve the quality of surveillance related to these diseases; promote a multi-sectoral response to HIV; and strengthen health systems that support the response to HIV and sexually transmitted infections. The primary strategies include targeted HIV prevention through condom distribution and peer outreach for prevention education among men who have sex with men and commercial sex workers; increased availability of voluntary counseling and testing; the provision of antiretroviral therapy for people living with HIV; the prevention of mother-to-child transmission of HIV; and partnership development between and among governmental and nongovernmental organizations as well as community-based and international organizations involved in the response. Training for health workers ranges from clinical management of antiretroviral therapy and treatment of opportunistic infections to monitoring, supervision and evaluation.

The program supported by this grant aims to reduce the incidence rate of HIV by 40 percent by 2012; mitigate the health effects of HIV and AIDS and improve the quality of life for persons infected with HIV; and strengthen the national capacity to coordinate and manage the multi-sectoral response to HIV and AIDS. The program targets pregnant women, discordant and concordant couples, commercial sex workers, people living with HIV and AIDS, and vulnerable and marginalized groups such as migrants and internally displaced persons.

A comprehensive review of tren...ds and projections and of the factors driving the HIV/AIDS epidemic in Uganda established the main issue as 'stabilization of the epidemic' at a high level of 6.7%. The Program therefore emphasizes the acceleration of prevention combined with a clearly balanced strategy for scale up and sustainability, integrating treatment into prevention and providing prevention, care and treatment interventions through a strengthened health system for service delivery. The Program covers key interventions for reducing sexual transmission of new HIV infections, strengthening and scaling up of counseling and testing, prevention of mother-to-child transmission of HIV (PMTCT), sexually transmitted infections (STI) control and management, as well as scaling up and sustaining provision of comprehensive HIV care including antiretroviral therapy (ART) to people living with HIV and AIDS (PLHAs). The Program also focuses on strategies to strengthen the health systems in both the public and private sectors. The goals of the Program are to reduce the incidence rate of HIV by 40% by 2012; to mitigate the health effects of HIV and AIDS and improve the quality of life of persons infected with HIV; and to strengthen the national capacity to coordinate and manage the multi-sectoral response to HIV and AIDS.

The main characteristics of the epidemic are: • the wide-spread of HIV among various population groups, particularly the most-at-risk groups of injecting drug-users (21.5% mean prevalence with over 40% prevalence in some Oblasts, 2011), female sex workers (7.6% prevalence, 2011), and men who have sex with men (6.4% prevalence, 2011). • the uneven spread of HIV infections in different regions of the country, with 77.1% of officially registered HIV cases in urban areas (Ukraine MOH, 2012). • the shift in dominant routes of HIV infection, from parenteral to sexual transmission (51.1% of persons infected as a result of unprotected sexual encounters vs. 28.6% from injecting drug use, 2012) • HIV affecting mainly the working age population. The Round 10 HIV grants aim to address the needs of MARPs and PLWHA and other people most affected by the HIV epidemic by scaling-up and ensuring equitable access to comprehensive HIV prevention, treatment, care and support; strengthening health systems for sustainable solutions and strengthening community systems that enable needs-based and cost-effective interventions. The Phase 2 proposal of the Round 10 grants takes into account the changing country context and epidemic trends and is aligned with the newly designed National HIV/AIDS Program 2014-2018 (NAP). Uruguay has a concentrated HIV... epidemic with prevalence rates among the most exposed and vulnerable populations exceeding five percent. The Program goals are: to promote universal access to evidence-based prevention, diagnosis and care of HIV, STIs and TB/HIV co-infection in the populations of gay men, other men who have sex with men (MSM) and Transgender (including sex workers and people deprived of liberty) in a framework of the right to health; strengthen the country's MSM and Trans organizations and constituencies to contribute to their social integration and to consolidate favorable conditions for controlling the epidemic; to promote a user-friendly environment to favor full social inclusion and exercise of citizenship by the MSM and Transgender populations to reduce their vulnerability to STIs and HIV; and to consolidate a timely, relevant and transparent national information system that may contribute to the national response with up-to-date evidence. The Program is being implemented by two Principle Recipients (PR) under dual-track financing. The current PR will focus on activities related to strengthening health care services and information systems to promote universal access to prevention, diagnosis and care of HIV, STIs, and TB/HIV co-infection for the target population.

Viet Nam is facing 2 major epi...demiological challenges: expansion of HIV epidemic among Most at Risk Populations (MARP) and rapid increase of people living with HIV/AIDS who are in need of care and treatment. The national coverage of HIV harm reduction activities as well as care and treatment including Anti-Retroviral Therapy (ART), remains far from sufficient even though these have been expanded during the last few years. In addition to Government and other donor resources, Global Fund's support will enable Viet Name to address substantial gaps in access to HIV prevention, treatment, care and support services in 53 to 64 provinces nationwide, through strengthening working partnerships between government and civil society. The current grant is a consolidation of the Round 8 and Round 9 HIV proposal under the Single Stream of Funding. The activities complement achievements of the national HIV/AIDS strategy goals by implementing core priorities, including harm reduction program for injecting drug users (IDUs), condom use promotion for female sex workers (FSW) and men who have sex with men (MSM), and care and support for people living with HIV/AIDS (PLWHA). The SSF program supports HIV treatment, care and support services and strengthens the organizational capacity of civil society organizations.

The program supported by this grant aims to address the following key strategic gaps in the HIV and AIDS response, based on the development of the National Strategic Plan on AIDS: reduction of HIV-associated stigma; containment of the spread of HIV infection; increased access to appropriate treatment; care and support for people living with and affected by HIV; and capacity building. The program targets populations most-at-risk, including injecting drug users and sex workers; vulnerable groups, including labor migrants, youth, women and prisoners; people living with HIV and their families; and sexually transmitted infection patients and blood recipients.

Afghanistan is still in the pr...ocess of building its knowledge about the HIV epidemic in the country. In the past three to five years, information on the population groups most affected by HIV and AIDS remained limited due to the absence of an appropriate infrastructure to monitor the disease. The number of officially registered HIV-positive persons is 184, but this is likely to be a gross underestimate. The program supported by this grant will strengthen the health system by establishing and upgrading urban centers devoted to HIV and other sexually transmitted infections and will strengthen collaborative dual diagnosis and treatment for TB/HIV co-infection. The program will also conduct a nationwide study on injecting drug use in Afghanistan and an exploratory survey on hidden populations, with specific attention to men having sex with men. In addition, the program will provide prevention, treatment, care and support services to people living with AIDS as well as those most at risk by establishing drop-in centers and HIV centers in prisons, providing HIV counseling and testing services and reaching out to young people with HIV and AIDS education.
