1. Background and Summary: Th...e primary goal of this Program is to reinforce the Government of Tanzania's response to the HIV/AIDS epidemic by expanding service delivery to the lower tiers of the health system. Attaining this goal requires financing recurring costs, such as Health Products and consumables, as well as supporting activities that maintain and strengthen the system in place, such as refresher training for community health workers, supervision and mentoring. Experience with previous Global Fund grants has highlighted the need for greater coordination among relevant stakeholders. Accordingly, this grant will support the scale up of HIV/AIDS services by strengthening capacity and coordination among key stakeholders in the public and private sectors, including: Civil Society Organizations (CSOs), Faith-based Organizations (FBOs), People Living with HIV/AIDS (PLHA) groups, as well as the Tanzania Commission for AIDS (TACAIDS), the national coordination body for Tanzania's multi-sectoral HIV/AIDS programming. Tanzania faces an urgent need for drugs and commodities for the delivery of HIV/AIDS services. The scale-up of the national anti-retroviral therapy (ART) program has resulted in a seven-fold increase in the number of patients enrolled in treatment, from 23,951 in 2005 to 189,494 in October 2008. Funding for commodities that are key to the delivery of key services such as HIV testing, Sexually Transmitted Infection (STI) management, Prevention of Mother to Child Transmission (PMTCT), ART, Opportunistic Infection (OI) treatment and prophylaxis, and early infant diagnosis will therefore form an integral part of the Program. The Program will additionally measure the outputs, outcomes and impact of HIV/AIDS interventions addressed by the Program. Specifically, this program's main objectives are to scale up through expansion of care and treatment services to primary health care facilities, increase coverage of key prevention services all over the country, ensure continuous quality service delivery and strengthen the national-level stakeholders and coordination mechanisms. Two Principal Recipients will work on attaining the goals of the program, namely the Ministry of Finance and African Medical Research Foundation (AMREF). 2. Goals: • Secure and sustain Tanzania's HIV/AIDS prevention, care, treatment and support services; and • Strengthen coordination and implementation capacity of key stakeholders. 3. Target Group/Beneficiaries: • PLHA who are eligible for care and treatment • PLHA suffering from chronic illness and/or malnourishment • Patients on care and treatment • In- and out-patients at health facilities • Pregnant women and their partners • Infants born to HIV positive mothers • Sexually active population, especially youth • Health care workers and PLHA accessing HIV/AIDS services • Private sector and Individuals and groups served by FBOs, PLHA, CSOs • General population of Tanzania 4. Zanzibar is a country with a l...ow prevalence of HIV and AIDS. However, its proximity to and frequent contacts with the Tanzania mainland, where the HIV and AIDS prevalence rate is high, places Zanzibar at a significant risk of a dramatic increase of infection in the near future. The program supported by this grant focuses on accelerating access to HIV and AIDS prevention, care, treatment and support in Zanzibar with special focus on most-at-risk populations including drug users, commercial sex workers and men who have sex with men. Under the program, community and health workers are being sensitized on HIV and AIDS issues through community outreach, media programs and training to change attitudes and behaviors.

HIV prevalence in Timor-Leste ...is low, estimated at between 0.3-0.6 percent of the adult population. The program supported by this grant is addressing critical gaps in the development and scale-up of the national response to HIV and AIDS in Timor-Leste. The program's main objectives are to improve and increase prevention, diagnostic and treatment services for HIV and sexually transmitted infections; improve the quality of surveillance related to these diseases; promote a multi-sectoral response to HIV; and strengthen health systems that support the response to HIV and sexually transmitted infections. The primary strategies include targeted HIV prevention through condom distribution and peer outreach for prevention education among men who have sex with men and commercial sex workers; increased availability of voluntary counseling and testing; the provision of antiretroviral therapy for people living with HIV; the prevention of mother-to-child transmission of HIV; and partnership development between and among governmental and nongovernmental organizations as well as community-based and international organizations involved in the response. Training for health workers ranges from clinical management of antiretroviral therapy and treatment of opportunistic infections to monitoring, supervision and evaluation.

Uganda has had considerable su...ccess in reducing HIV prevalence rates, but is still confronted with a serious HIV and AIDS epidemic, including rising numbers of people needing care and support, and orphans. The program supported by this grant aims to reduce the incidence rate of HIV by 40 percent by 2012; mitigate the health effects of HIV and AIDS and improve the quality of life for persons infected with HIV; and strengthen the national capacity to coordinate and manage the multi-sectoral response to HIV and AIDS. The program targets pregnant women, discordant and concordant couples, commercial sex workers, people living with HIV and AIDS, and vulnerable and marginalized groups such as migrants and internally displaced persons.

Benin is a country with a gene...ral HIV epidemic. At the end of 2007, UNAIDS estimated HIV prevalence in adult population at 1.2 percent with a significant gender disparity of 1.5 percent among women and 0.8 among men. These estimates did not consider the appearance of new determinants which favor the propagation of infection such as sex workers and their clients, men who have sex with men (MSM) and injecting drug users (IDUs). The Program was developed based on the lessons learned from Round 2 and Round 5 of Global Fund grants and aims to ensure that objectives of the National Strategic Framework for the fight against AIDS are met by 2015. It encompasses achieving universal access to the prevention, treatment, care and support of persons infected and affected by HIV, through strengthening health and community systems. The Program consolidates Round 5 and Round 9 HIV grants and is implemented by three Principal Recipients.

Malaria is endemic in large ar...eas of Afghanistan below 2,000 meters above sea level and is highly prevalent in river valleys used for growing rice. In 2007, a total of 433,412 malaria cases were reported. The program supported by this grant aims to contribute to improving the health status in Afghanistan through the reduction of malaria-associated illness by 50 percent and a reduction of malaria-related deaths of 80 percent. Program strategies include prompt and reliable diagnosis and effective treatment; application of effective preventive measures such as long-lasting insecticidal nets; timely detection and control of malaria epidemics; and strengthening of the health system and malaria control. The program targets the population living in the 14 provinces of Afghanistan with high risk of malaria transmission, with particular attention paid to pregnant women, children under age 5, internally displaced populations and Afghan returnees to malaria endemic areas from low or non-malaria endemic countries. The grant has been consolidated with AFG-509-G06-M.

Malaria is a major health prob...lem in Namibia, the leading cause of illness and death among children under age five and the third leading cause of death in adults. The program supported by this grant aims to reduce the burden of malaria to such an extent that it will neither be the major contributor to illness and death, nor of significant socio-economic impediment by the year 2030. The program targets nine regions with 22 health districts endemic to high malaria prevalence, pregnant women and children under age five. Activities broadly focus on vector control, personal protection, case management, surveillance and epidemic preparedness, detection and response and operational research.
