Afghanistan is still in the pr...ocess of building its knowledge about the HIV epidemic in the country. In the past three to five years, information on the population groups most affected by HIV and AIDS remained limited due to the absence of an appropriate infrastructure to monitor the disease. The number of officially registered HIV-positive persons is 184, but this is likely to be a gross underestimate. The program supported by this grant will strengthen the health system by establishing and upgrading urban centers devoted to HIV and other sexually transmitted infections and will strengthen collaborative dual diagnosis and treatment for TB/HIV co-infection. The program will also conduct a nationwide study on injecting drug use in Afghanistan and an exploratory survey on hidden populations, with specific attention to men having sex with men. In addition, the program will provide prevention, treatment, care and support services to people living with AIDS as well as those most at risk by establishing drop-in centers and HIV centers in prisons, providing HIV counseling and testing services and reaching out to young people with HIV and AIDS education.

Albania has a low prevalence o...f HIV and AIDS. However, data show an upward trend of new diagnoses and suggest a larger number of undiagnosed cases. As of June 2008, the National HIV/AIDS Program reported a total of 324 HIV infections, with 48 new cases midway through the year. The Institute of Public Health implements Albania's response to HIV and AIDS under a national strategy for the period 2004 to 2010. The program supported by this Round 5 grant aims to maintain low HIV prevalence among all vulnerable groups in Albania – including Roma,female sex workers, men who have sex with men and injecting drug users – and to provide care, support and treatment for people living with HIV and AIDS. The program focuses on harm reduction, drug substitution treatment and HIV and AIDS education for injecting drug users in prisons.

This program implemented by the Ministry of Health aims to accelerate the reduction of the HIV epidemic in the country by providing antiretroviral treatment for those who need it, universal access to counseling and testing among pregnant women and general population, HIV prevention activities among prisoners and mobile populations, methadone substitution treatment, HIV education in school settings, strengthening of national information, monitoring and evaluation, laboratory and procurement and supply management systems.

Azerbaijan is a lower-middle i...ncome country with a concentrated HIV epidemic. HIV prevalence in adults is estimated at 0.2 percent (UNAIDS, 2008 Report on the Global AIDS Epidemic). Although the national burden of HIV appears comparatively low, it is probably underestimated - due, in particular, to difficulties in accessing most-at-risk population groups. The predominant HIV transmission mode is sharing of injecting equipment among people who inject drugs. The government of Azerbaijan is committed to fighting the HIV epidemic and allocates increasing amounts of financial, human and infrastructural resources for this purpose, particularly to cover the substantial costs of staff, medical interventions and facility expenses. However, substantial financial and programmatic gaps exist, especially with regards to the complex and costly interventions in HIV prevention and treatment activities among most-at-risk population groups. The program supported by this grant aims to ensure the continuation and scaling-up of the activities initiated under the Round 4-supported program, which ended in June 2010. This program will initiate new activities, such as expansion of voluntary counseling and testing, ensuring universal access to prevention of mother-to-child transmission of HIV and establishing community-based care and support.

The Program was developed based on the lessons learned from Round 2 and Round 5 of Global Fund grants and aims to ensure that objectives of the National Strategic Framework for the fight against AIDS are met by 2015. It encompasses achieving universal access to the prevention, treatment, care and support of persons infected and affected by HIV, through strengthening health and community systems. The Program consolidates Round 5 and Round 9 HIV grants and is implemented by three Principal Recipients.

UNAIDS estimates Bhutan has so...me 500 HIV-positive people, putting prevalence of the disease at less than 0.1 percent. However, 35 percent of its population is youth aged 10 to 24 years, which constitutes a particularly vulnerable group and risky behavior among the general population is widespread. The program supported by this grant will address key gaps in the current disease response to reduce the risk of HIV transmission among youth and other vulnerable population groups by building on the activities and achievements of a World Bank-funded project. The program's three main objectives are to increase access to prevention services for the target groups, which include substance abusers, sex workers, mobile populations, monks, uniformed personnel and their families; to ensure a continued supply of antiretroviral drugs and opportunistic infection drugs for the treatment and care of people living with HIV; and to increase the national capacity to plan, implement, coordinate, and monitor and evaluate HIV and AIDS programs.

The HIV and AIDS epidemic in B...olivia is concentrated within at-risk populations. HIV prevalence in the general population is 0.1 percent, although it is estimated that the epidemic is severely underreported. Half of all new HIV cases in the country are reported in the state of Santa Cruz, which neighbors Brazil and is characterized by its commercial activity and migration flows. Sexual transmission accounts for 84 percent of new infections in the country, with half reported as heterosexual. This grant funds a program which targets vulnerable communities – including pregnant women – in nine provinces to receive integrated services, including antiretroviral therapy and monitoring to promote adherence; laboratory and psychological support; and treatment of opportunistic illnesses. Media campaigns, peer training and improving surveillance systems are some of the additional activities carried out under the program. One of the primary objectives of the program is to reduce the incidence of sexually transmitted infections and to improve surveillance systems. Another important program component includes advocacy and promotion of human rights for people living with HIV.

The goal of the program supported by the Round 9 grant is to reduce new HIV infections and HIV and AIDS morbidity and mortality rates in line with the national multisectoral strategic plan. Specific objectives include timely HIV prevention in high-risk population groups (such as lesbians, gays, bisexuals, transgender and intersex people) and other vulnerable populations (such as sex workers, prisoners and pretrial detainees, and the homeless); the prevention of mother-to-child transmission of HIV; universal access to integral health care for people living with HIV; and strengthening the national information system and public policies on HIV. Target groups include men who sleep with men, transgender people, sex workers, pregnant women and young people. The Principal Recipient for the program financed by this grant has also implemented a Round 3 program (BOL-307-G07-H), which is currently in the close-out stage.

HIV infection rates among vulnerable groups such as commercial sex workers or prisoners are especially alarming. The program supported by this grant aims to improve sexually transmitted infection and AIDS services, including voluntary counseling and testing services; improve accessibility, quality therapeutic treatment and care, and follow-up of people living with HIV and AIDS, including those co-infected with TB; and improve and broaden pediatric care.

At 1.9 percent among adults fr...om 15 to 49 years of age, Cambodia has the highest HIV prevalence in East Asia and Pacific and one of the highest rates in Asia as a whole. The program supported by this grant aims to increase the survival of people living with HIV in Cambodia; reduce the vulnerability and mitigate the impacts of HIV and AIDS on orphans and other vulnerable children through targeted interventions; decrease the risk of transmission by blood transfusion in Cambodia; reduce the prevalence rate of HIV among the general population; and reduce HIV transmission through targeted intervention with high-risk groups. To achieve its goals, the program uses television and radio to communicate with its target audiences about HIV and AIDS; promotes condom use through condom distribution and social marketing; provides psychosocial support to orphans and other vulnerable children and people living with HIV, including support aimed at reducing stigma and discrimination against these groups; and procures, distributes and monitors the usage of antiretroviral drugs.

Cameroon has a general HIV pre...valence of 5.1%. The Program will contribute to a reduction in morbidity and mortality related to HIV, thanks to a decrease in new infections in the most at risk populations (MARPs) and in the general population, and an improvement of the quality of care for people living with HIV/AIDS (PLWHA). The goal of this Program is to reduce new HIV infections in newborns by preventing mother-child transmission; reduce new HIV infections among sex workers, MSM, truck drivers and their partners; reduce morbidity and mortality related to HIV as well as the related socio-economic impact by reinforcing the global care of adult and child PLWHA and the support to OVC by 2015; and strengthen coordination, monitoring and evaluation, and partnership with civil society in the context of the implementation of the proposal. The Program is implemented by two Principal Recipients (PR) under the dual-track financing scheme.

Chad ranks among the least dev...eloped countries. A 2003 epidemiological study estimated HIV prevalence at around 8 percent of the adult population, with 200,000 individuals infected with HIV. The program supported by this grant aims to decentralize services through coordination with the National AIDS Committee A 'minimum activity package' for prevention, health care and treatment will also be set up in 57 health care districts according to criteria based on the characteristics of the epidemic, the health care chart, the possibility of progression for the health care system, the demographic and cultural context, and the geographic disparities of Chad. The program targets women living in vulnerable situations; people living with HIV; young people; orphans and other vulnerable children; people who sell sex; and mobile groups and nomads. It will focus on behavior change communication and community outreach and on strengthening managerial and organizational capacities of community-based organizations and networks of people living with HIV.

Comoros has a low HIV and AIDS... prevalence. The principal aim of the program supported by this grant is to keep the prevalence rate low. The program focuses on preventing HIV and AIDS through integrating the teaching of sexually transmitted infections and HIV and AIDS into academic programs in primary and secondary schools; reaching youth outside of the education system through anti-AIDS clubs; and establishing psychosocial and clinical care services at health centers and facilities to allow care of all youth affected by HIV and AIDS. In addition to youth, the program targets commercial sex workers, health service providers and political and religious leaders.

The program supported by this grant seeks to promote safer sexual behavior among teenagers not attending school; facilitate access to counseling and testing services for adults; extend coverage of comprehensive HIV services to 100 percent of the children identified as living with HIV; carry out HIV-related monitoring and evaluation and strengthen coordination of HIV efforts. The program is supported by two Global Fund grants implemented by a government and civil society Principal Recipient, respectively, working in partnership. This grant focuses on activities related to psychological care and support and to strengthening national civil society organizations. The program will cover the entire country and benefit from joint funding by the Congo government, which will primarily support the purchase of medicines for children.

The overall goal of the program supported by this grant is to reduce the prevalence, sickness and death caused by HIV and AIDS. The program targets the general population; vulnerable groups such as youth, pregnant women, commercial sex workers and mobile populations; and people infected with or affected by HIV and AIDS. The program is using grant funds to strengthen capabilities related to prevention, the overall care of people living with HIV and AIDS, community mobilization and awareness. The program also is strengthening capabilities in the field of mother-to-child prevention; promoting the prevention of the sexual transmission of the disease and of other sexually transmitted infections; and providing medical and psychosocial support for people living with HIV and AIDS.

There are between 50,000 to 70...,000 people living with HIV in the Dominican Republic. Heterosexual transmission constitutes 75 percent of all cases, affecting more men than women. However, prevalence is growing among female sex workers. The program supported by this grant aims to scale up existing HIV prevention and treatment services throughout the country, with a special focus on at-risk and vulnerable populations, including sex workers, men who have sex with men, youth, pregnant women, migrant populations and members of the armed forces. Funded activities include behavior change communication geared toward vulnerable groups, increased provision of facilities for voluntary counseling and testing, and scaling up the number of sites that provide antiretroviral treatment, including for pregnant women to prevent mother-to-child transmission of HIV. The program also provides training to health care personnel in the clinical management of HIV and AIDS.

Although significant advances ...have been made in the HIV and AIDS epidemic in El Salvador, they still face many challenges. This Program is a consolidation of the Round 7 and Round 2 HIV grants and is being implemented by 2 Principal Recipients (PR) under dual-track financing. This PR is responsible for the training of health care staff in HIV and AIDS care, treatment and prevention; strengthening of prevention activities focusing on mobile populations and persons deprived of their freedom; training of persons working for municipal councils in the response towards HIV and AIDS; implementing and expanding the mass media campaign; and expansion of voluntary counselling, testing, diagnosis and treatment services. The goal of this Program is to reduce the incidence of HIV and AIDS among populations living in highly vulnerable conditions as well as the social and economic impact of the HIV and AIDS epidemic.
