MRC/NMHB award to improve unde...rstanding of stigmatisation against people with mental illness, and then to develop and test new methods to reduce such stigma in economically developing countries. The overall aims of the Indigo Partnership are to establish a strong research collaboration to provide the infrastructure for harmonised metrics and to develop stigma reduction interventions, and to carry out platform activities to strengthen the scientific understanding of mechanisms of action of stigma processes against people with mental illness. The longer term aim is to reduce the mental health treatment gap. Specific objectives of the Indigo Partnership are to: 1. Create, and put on a sustainable footing, an actively managed consortium which acts as a highly collaborative network of research staff in 6 low and middle income country (LMICs) field sites: Ethiopia, Tunisia, China (2 sites), India (2 sites), and in partner sites in Germany, Switzerland [WHO], UK and USA. Milestones: collaborative North-South and South-South capacity building activities; publication protocols; collaborative publications; skill sharing and capacity building procedures; closely managed budget agreement; ethics toolkit, and collaborative grant submissions. 2. Conduct formative qualitative situational analyses and ethnographic research to identify stigmatizing language, behaviours, and institutional practices and their underlying mechanisms of action in stigmatisation processes across cultural contexts. Milestones: systematic framework with documentation of cultural factors for cross-cultural adaptation of tools and interventions. 3. Establish a harmonised online evaluation toolkit which is a set of instruments specifically designed for use in future stigma reduction intervention studies. Milestone: culturally adapted, acceptable, feasible instruments with strong psychometric properties to assess implicit biases, self-reported attitudes and experiences, and observed behaviours. 4. Develop and pilot test inter-personal contact intervention packages in 4 LMICs for stigma reduction in community, primary care and specialist care settings, based on mechanisms of action identified through evidence-based literature and formative cross-cultural research. Milestone: packages of care including manuals and training materials for community, primary care, and specialist care settings adapted for 4 LMICs. 5. Build research capacity in the 4 LMICs to strengthen their capability to become centres of excellence for future multi-site intervention studies related to mental health stigma reduction. Milestones: publications led by LMIC-based investigators. 6. Use the results of this research to develop protocols and funding applications for multi-site randomized controlled trials (RCTs) for three intervention packages (community, primary care, specialist care) to ultimately inform a stigma-reduction module for the WHO Mental Health GAP (mhGAP) package. Milestone: protocols and funding proposals

High blood pressure (hypertens...ion) is extremely common within the general population in the UK and worldwide and is a major cause of heart disease, kidney disease and stroke. It is recognised to be the single biggest contributor to the global burden of disease. In most people with hypertension, a healthy lifestyle alone is not enough to control blood pressure, and drug treatment is required. There are a wide variety of drugs available and although these are effective and safe, it is often necessary to try different types of drugs and often to use a combination of two or more drugs. Delay in choosing the right kind of tablet or combination of tablets through trial and error is a major problem and, in a large proportion of people with hypertension, blood pressure is not adequately controlled. Relatively little is known about why some people respond better to one kind of tablet or combination of tablets than others. It is know that response to treatment differs in different ethnic groups in the UK and may differ between populations in Europe, Asia and Africa. The objective of this research proposal is to determine if we can use genetic markers of ancestry (which predict the proportion of a person's ancestors from Europe, Asia and Africa) and a detailed measure of chemical metabolites circulating in the blood that characterise the biochemical processes in each person to predict the best type of drug or combination of drugs for that person. If a combination of drugs is required we can use a new technology to put these into a single tablet. We hope that this will provide a more effective simpler approach to single tablet treatment of hypertension both in the UK and in other countries.
