Shrimp aquaculture is playing ...a pivotal role in ensuring food and nutritional security of the growing population, in most of the south Asian countries. However, disease outbreaks among cultured animals put significant constraint and affect the economic development. Vibrios belonging to the Harveyi clade are pathogenic marine bacteria affecting both vertebrates and invertebrates causing devastating luminescent Vibriosis. The virulence of vibrios is controlled by different regulatory systems, including quorum sensing (QS-bacterial cell to cell communication through signal molecules). We have in a previous study seen that in the presence of microalgae there is a down regulation of virulence genes expression in Harveyi clade vibrios with known QS regulation. In the present study, by a series of experiments, we will investigate the QS response of pathogenic Harveyi clade Vibrios when grown together with microalgae in the presence of the gnotobiotic Artemia as a crustacean model organism, and simultaneously determine the effect on survival of Artemia infected with pathogenic Harveyi clade vibrios. Our intention is to work and develop a small-scale gnotobiotic model system that is easy to control. At the latter part of the project period we will test the system on Asian tiger shrimp, which is a marine crustacean widely reared for food, and the most important cultured marine product in Sri Lanka and in India.

Why do citizens pay taxes? Thi...s question is surprisingly underexplored. Understanding when citizens pay taxes and why they find it relevant and purposeful is central for answering this question. Tax compliance is a major concern in developing countries and particularly in Sub-Saharan Africa, in order to reduce dependency of foreign aid and increase state revenues for providing adequate health, educational and infrastructural services for their populations. In a nutshell, taxes are needed to build capable states that can provide for their citizens. Remarkably, taxation has not figured centrally on the agenda among scholars working on statebuilding in Africa or the African voter. Determinants of willingness to pay taxes in developing countries need to be scrutinized using an insiders' perspective of what it means to be a tax payer on an everyday basis across different socioeconomic groups. This project expands on current explanations through focusing on the social practice of taxes; when, how and where taxation occurs and how this practice forms part of the everyday for ordinary citizens. This is studied through an ethnographic study of the practices surrounding tax payment in three cases in Sub-Saharan Africa (Lesotho, Tanzania and Zambia), where the willingness to pay taxes and the relative tax burden vary. A comparative micro-study of citizens' experience with taxes, using interviews and observations, will reveal the room for building a tax paying culture.

Research on the health and hea...lth care of prisoners in low-income countries is scant. The overall objective of present research collaboration is to improve the mental health of young inmates and to strengthen the health care system within the prisons in Cambodia. This will be done through the development of a collaborative partnership between researchers at the Caritas-Center for Child and Adolescent Mental Health (CCAMH) in Phnom Penh and the unit of Epidemiology and Global Health (EpiGH) at Umeå University. This will be achieved through the combination of five interlinked research and capacity building specific aims: Research aimsTo assess prevalence and pattern of mental health problems and life skills among young prisoners,To evaluate the impact of the Life Skills Education intervention to promote mental health among young prisoners,To develop a plan to strengthen the mental health care system in the selected prisons. Capacity building and collaboration aimsTo identify conceptual and methodological approaches to promote mental health among youth prisoners and the strengthening of mental health care system in prisons,To build individual and institutional capacity in both research institutions in the areas above highlighted.The proposed research collaboration can highlight the health situation of youth inmates, give voice to their needs and create a space of dialogue with prison authorities aiming to improve the mental health and health care in the prisons nationwide.

Content of study Groin hernia... is the most commonly performed procedure in general surgery. The most effective surgical method uses a synthetic mesh. We have also shown that a low-cost mosquito mesh can be used for this purpose. In sub-Saharan Africa, most surgical procedures are performed by non-surgeons, a practice called task-sharing. In this study, we will implement a training program for surgeons and non-surgeons in groin hernia repair using low-cost mesh. We hypothesize that these can be performed by surgeons and non-surgeons with similar outcomes. Project organisation, time frame and methods This is a single-blinded prospective cohort study at four sites in Uganda, Sierra Leone, Ghana, and Tanzania. Surgical providers will be trained in mesh inguinal hernia repair. Adult men with reducible, primary inguinal hernia will be included (n=228 per site). The patients will be followed up after 2 weeks and after one year. Primary outcome is recurrence and secondary outcomes are complications and patient satisfaction. Results between study sites and types of providers will be compared. Enrollment of patients will start in late 2016 in Ghana and during 2017 and 2018 in the remaining countries. Follow up will be completed during 2018 and 2019. Significance Results will determine the safety and efficacy of task-sharing in mesh inguinal hernia repair. Findings will guide implementation of mesh hernia repair and other surgical education programs in low-income contexts.

The average age of our society... is increasing drastically. This demographic shift poses a difficult task for health- and social care systems. A plethora of natural products are tested for their ability to counteract aging and associated diseases. Here, we will isolate and purify compounds from bee products and test them for their cytoprotective function during aging. Thereby, already known as well as newly identified compounds will be analysed for their ability to counteract oxidative stress, age-associated decay and the functional decline of mitochondrial function and autophagy. Large-scale screens in yeast, which is a well-established model system for aging, will identify compounds with anti-aging activity, which will in turn be (i) studied in more detail regarding their underlying mechanisms, and (ii) subjected to structure-function elucidation via the synthesis and subsequent analysis of structural analogues. To test for phylogenetic conservation, most promising compounds will be synthesized in larger amounts and tested in Drosophila for their effect on life span, motor function and learning and memory capacity. In aggregate, proposed research might identify cytoprotective compounds of bee products and pave the way for future applications of these compounds as food supplements and nutraceuticals.

For over 40 years, natural pro...ducts have served us well in combating cancer. The main sources of these successful compounds are microbes and plants from the nature. A major contribution comes from plant alkaloids, taxoids and podophyllotoxins. A vast array of biological metabolites can be obtained from the bee-honey natural products, which can be used for effective cancer treatment. The search for novel drugs is still a priority goal for cancer therapy, due to the rapid development of resistance to chemotherapeutic drugs. In addition, the high toxicity usually associated with some cancer chemotherapy drugs. Hepatocellular carcinoma is one of the leading causes of death in Egypt and globally the numbers of cases of this cancer are increasing gradually. There are several medicines available in the market to treat this type of cancer and they are found to be not fully effective and/or safe. However natural products have proved effective and safe in the treatment and management of some other types of cancer. Here an attempt will be made to pave the way of using natural products isolated from bee venom and their small and large molecules analogues to combat hepatocellular carcinoma.

Within the collaboration we will conduct a prospective multi center implementation study on the training and introduction of groin hernia mesh repair under local anaesthesia in Ghana, Sierra Leone, Uganda and Tanzania. We will investigate the feasibility of task sharing to increase accessibility to surgical services for groin hernia in the involved sub-Saharan settings of this study. Patients (adult men with reducible, primary groin hernia) will be included at the respective study sites (n=228 per site). They will be operated under local anaesthesia using the anterior mesh repair with a low-cost synthetic mesh. They will be followed up after two weeks and after one year. The patients will be included into the study between 2016 and 2018 and follow up will be completed by 2019. Through the collaboration, we will pool knowledge and experience and we will increase operative capacity of the study team. It enables us to undertake large studies in several sites within a limited time frame. The results from this study will guide large scale implementation and introduction of the anterior mesh repair in the study countries and beyond. This is urgent as the unmet need for groin hernia surgey amounts to many millions of patients worldwide.

Mental illness is contributed ...to by adversity yet psycho-social assets in young people can be strengthened to moderate the impacts of adversity. This projects' overall purpose is to adapt and implement an intervention to strengthen resilience and mental health for disadvantaged young people in urban North India using theory driven evaluation to guide the process. In 2016 this team evaluated an intervention to build youth resilience as promising. We believe it can become more acceptable, effective, and sustainable by adding components that address family environments and the different needs of young men. The key steps to the research activities in this collaboration are: 1 (Months 0 – 15) Eliciting the programme theory with the community and research team, 2 (Months 16 – 24) Testing the programme theory by implementing the adapted intervention among disadvantaged 12 – 16-year-olds in urban North India using qualitative (in-depth interviews, focus group discussions) and quantitative data (psycho-metric scales) to assess resilience outcomes, 3 (Months 0 -36) Identify how, why, for whom and under what circumstances the intervention works, The collaboration will facilitate a long-term partnership. The EHA team in India will contribute expertise in implementing community mental health programmes while members from Umea and Melbourne will contribute their expertise in theory-driven evaluation. The collaboration will be supported by joint field work, workshops and study visits.

Poverty and disability typical...ly operate in a cycle, with one reinforcing the other, often exacerbated for women and older adults where social safety nets are weak: a reality in Myanmar. Evidence- informed programmatic/policy action, which is currently lacking in Myanmar, is needed to break the cycle. The aim of this project is to explore determinants that influence and shape ageing and older adult lives – from their life choices, to their local environment, to the services they access – and how these come together to affect their wellbeing, care needs and utilization. This research project will employ a mixed methods design using a follow-up quantitative study and qualitative study to understand the intersection of ageing, poverty, gender and functioning in Myanmar. We will follow-up individuals who participated in the 2012 Myanmar Aging Survey and conduct panel data analysis to analyse how health and its determinants of a large group of older people have changed over time. Focus-group discussions and interviews will be conducted to further explore local, contextual perceptions of ageing and how the health system is responding to care needs. The results of this project can be used to understand what drives health and health inequalities in different groups of older people and build on this scientific knowledge to inform the National Health Plan and National NCD Strategic Action Plan (2017-21) in order to improve older people health in this resource-constrained setting.

Although nasopharyngeal carcin...oma (NPC) is a common cancer in Sudan, little is known about its etiology and there is currently no existing screening program for early detection there. The overall aim of this project is to understand etiology of NPC and to develop a useful screening tool for early detection of NPC in Sudan. Specifically we aim at: 1) to determine the genetic and environmental risk factors for NPC, 2) to develop a non-invasive and cost-effective method for early detection and diagnosis of NPC. To achieve aim 1, we will initiate a case-control study in 2 largest hospitals in the capital of Sudan, where NPC diagnosis and treatment is centralized. We aim to recruit 500 incident NPC cases and 1000 matched controls. Each study subject will be interviewed by questionnaire and blood samples collected. Conditional logistic regression model will be used to examine association between genetic and environmental risk factors with NPC risk. For aim 2, we will further use serum samples collected in the case-control study to build an optimal algorithm by testing various serology assays for EBV antibodies and EBV load. The developed screening tool will then be tested in high-risk groups, including relatives of NPC patients, and a high-incidence tribe. This unique collaborative project will hopefully broaden our understanding of etiological factors of NPC, and the results will eventually lead to the development of primary and secondary prevention strategies for NPC in Sudan.

The extent of pesticide exposure and associated health effects among populations in LMICs is not established. The aim of this project is to evaluate how climate change and intensified use of pesticides affect pesticide exposure and health in humans, including susceptible groups. We will create a new international research collaboration bringing together studies of pesticide exposure and health outcomes in populations from LMICs around the world affected by climate change. The populations include mother-child pairs from Bangladesh and Costa Rica, children from South Africa and agricultural workers from Uganda, El Salvador and Nicaragua. We will use the expertise in biomarker analyses at Occupational and Environmental Medicine (OEM) in Lund to monitoring pesticide exposure in the populations. Exposure will be analyzed in relation to health effects relevant to pesticide exposure (e.g. respiratory effect, neurodevelopment and chronic kidney disease) by research groups responsible for the individual study populations. Negative health effects in relation to pesticide exposure will highlight the importance of controlling pesticide usage in LMICs and to increase the emphasis on more sustainable means to adapt to climate change.

Exposure to toxic chemicals in... food and water is a high-priority public health problem (UN Sustainable Development goals). The proposed study is a competing renewal of the VR grant Dnr 2016-05799 (ToxBol), in which Bolivian, Swedish, and French universities collaborated to evaluate environmental problems in Bolivia and their impact on human health. We identified areas in Bolivia with elevated metal and pesticide exposure and showed that part of the population is exposed to these chemicals at worrying levels. We also identified susceptibility factors, including genetics. However, many areas in Bolivia still have uncharacterised exposures, and we know little about exposure in children, a particularly vulnerable group. Furthermore, the effects of these exposures on the gut microbiome, a susceptibility marker for diseases that are increasing in South America, remains unclear. We therefore propose to add 2 new partners to ToxBol (new acronym MicroToxBol), enabling us to 1) characterise children's metal and pesticide exposures, 2) investigate the gut microbiome in relation to metals and pesticides, and determine possible dysbiosis, 3) perform pilot studies in new high-risk areas in Bolivia (with high salinity, mining, and extensive use of specific pesticides), and 4) conduct risk assessment and 5) workshops for dissemination of the results. The MicroToxBol Network will increase knowledge about environmental problems in Bolivia, providing key information for improving public health.

The question why some individu...als are poor and have difficulty escaping poverty is a fundamental question that lies at the heart of the sustainable development goals (SDGs). This project will offer new insight into this question by examining whether poverty harms individuals psychological wellbeing which in turn may have large implications for their decision making and consequently their ability to escape poverty. The aim of this project is three fold: 1) Understand whether poverty impacts the psychological outcomes of individuals in developing countries. 2) Investigate the impact of psychological outcomes on production and consumption decisions. 3) Study the impact of decision making on poverty. We will measure psychological outcomes using novel lab in the field experiments and surveys conducted in areas with an exogenous variation in poverty. This information will be matched with agricultural and consumption decisions. The field studies will be complimented with an analysis of the impact of poverty reduction policies on psychological outcomes. This will be the first research to empirically investigate the connection between poverty, psychological outcomes and production and consumption decision making. Thus, this project will improve our understanding of why people are poor, which, in turn provides the foundations needed to reduce poverty and achieve the SDGs. This project will commence in 2019 and include researchers in Sweden, the UK and Austria.

Alzheimer's disease is one of ...the most disastrous neurodegenerative disorders and is connected with the deposition of amyloid beta plaques and neurofibrillary tangles (NFTs) composed of tau proteins in the brain. Currently, there are no drugs available for the treatment of this disease except those which treat the symptoms. With increased welfare and aging population Alzheimer's disease shows a pandemic development also in the third world. There is an urgent need for efficient and cheap diagnostic tools that can be used in the field, outside main hospitals, to screen large populations. Positron Emission Tomography (PET) for brain imaging is currently the most commonly used diagnostic technique for Alzheimer's disease, which, however, is very expensive and involves the exposure of the body to high energy radiation. It is also possible to diagnose the disease and monitor its progress by measuring the loads of amyloid beta plaques and NFTs in collected samples of the cerebrospinal fluid (CSF). Therefore, as an alternative to PET, one can use optical imaging biomarkers on collected samples of CSFs to estimate the population of amyloid andtau fibrils that are associated with AD. This requires only a cheap hand-held optical equipment. In this proposal, we aim to optimize optical fluorescent probes with high affinity and specificity for amyloid and tau fibrils and so to contribute to a high-throughput Alzheimer's diagnostic tool for the developing countries.

The long-term goal of the project team is to determine what the effects of psychotherapy as practiced in regular clinical practice in Uganda and Kenya are, which subgroups of patients do now achieve good outcome, and how could treatment outcome for these groups be improved? In the short-term, the network grant would facilitate building connections, understanding mental health system needs and functioning on ground.Collaboration: The network will consist of regular meetings between Swedish, Kenyan and Ugandan researchers. In addition, professor Pim Cuijpers at Vrije Universiteit Amsterdam will function as a project consultant.Organisation, time plan and collaboration formats: Pilot work has been done at two sites in Nairobi, Kenya. The experiences from this work will be built upon to plan for a broader project involving sites in both Kenya and Uganda. During 2019 two meetings will be held, one in Africa and one in Europe, to draw the plans for how the project should be implemented. Poverty and disability typical...ly operate in a cycle, with one reinforcing the other, often exacerbated for women and older adults where social safety nets are weak: a reality in Myanmar. Evidence- informed programmatic/policy action, which is currently lacking in Myanmar, is needed to break the cycle. The aim of this project is to explore determinants that influence and shape ageing and older adult lives – from their life choices, to their local environment, to the services they access – and how these come together to affect their wellbeing, care needs and utilization. This research project will employ a mixed methods design using a follow-up quantitative study and qualitative study to understand the intersection of ageing, poverty, gender and functioning in Myanmar. We will follow-up individuals who participated in the 2012 Myanmar Aging Survey and conduct panel data analysis to analyse how health and its determinants of a large group of older people have changed over time. Focus-group discussions and interviews will be conducted to further explore local, contextual perceptions of ageing and how the health system is responding to care needs. The results of this project can be used to understand what drives health and health inequalities in different groups of older people and build on this scientific knowledge to inform the National Health Plan and National NCD Strategic Action Plan (2017-21) in order to improve older people health in this resource-constrained setting.

Stroke is a growing societal c...hallenge and rehabilitation is needed. Information Communication Technology (ICT) provides clients with access to information, self-evaluation, and self-management. ICT-supported programs in persons' home environment are recommended when there are long distances to health care. Our previous feasibility study suggested beneficial effects on self-efficacy and occupational performance and satisfaction of a mobile phone supported and family-centred rehabilitation intervention (F@ce) after stroke in Uganda. The purpose of present proposal is to implement and evaluate F@ce on functioning in ADL and participation in everyday life among persons with stroke and their families. With recommendations on evaluation of complex interventions both outcomes and processes will be studied and hence both quantitative and qualitative methods will be applied. In a randomized controlled trial we will evaluate F@ce compared to ordinary rehabilitation in urban/rural Uganda regarding, self-efficacy, perceived performance and participation in everyday activities, independence in ADL, health care utilization and the families' perceived participation in everyday activities. Qualitative data will explore experiences of people with stroke and family members of participating in F@ce. This research program has a multidisciplinary perspective for sustainable rehabilitation, a prerequisite for better living conditions for people with stroke in low/middle income countries.

Non-communicable diseases (NCD...s) are on the rise in low-income settings in Africa. Using a new population-based cohort the Africa Medical and Behavioral Sciences Organization (AMBSO) population health surveillance in urban Uganda, we aim to analyse and prevent the occurrence and determinants of NCDs, specifically hypertension and diabetes, causing lifelong disability and premature death in young adults. NCD research in low-income settings is still in its infancy, but we have a unique opportunity in time to identify effective policy strategies for prevention and early detection, since these chronic conditions are costly and resource-demanding to treat. The NCD rise in urban areas is due to longer life expectancy (reduced child mortality and access to HIV treatment), but also changing dietary, smoking, drinking, transport and work habits. Though poorly known so far, these traditional determinants likely interact with preterm birth/malnourishment, chronic infections (e.g. HIV), antimicrobials, air pollution and stressors related to poverty and gender-based violence. Through a new collaboration between KI and AMBSO, our team is highly qualified to deliver results contributing to future health policy and several of the SDGs (1,3,4,5,11) in Uganda and beyond, directly promoting sustainable development and lifestyles, human rights, gender equality and cultural diversity through education through mutual learning, professional exchange and capacity building (SDG 4.7).

The trauma life support traini...ng programs Advanced Trauma Life Support (ATLS) and Primary Trauma Care (PTC) are widely used to improve hospital trauma care but no robust evidence exists to show that these programs have any effect on patient outcomes.The purpose of this network of researchers in Sweden and India is therefore to develop a new research project to assess the effects of ATLS and PTC training on patient outcomes compared to standard care. We intend to focus on the following research questions: 1. Is it feasible to conduct a cluster randomised controlled trial comparing ATLS and PTC with standard care? 2. What outcomes are perceived as most important and relevant by trauma patients and community members? 3. What is the potential effect of ATLS and PTC compared to standard care on patient outcomes? 4. How do the magnitude and direction of this effect differ across clinically important subgroups?We will arrange seminars to plan a joint research project. To inform this planning we will conduct community consultations and a pilot study. We will arrange a stakeholder meeting to gather input from and shortlist prospective sites for participation in the large scale project.The planned joint research project would be the first to provide high-quality evidence of the effect of ATLS and PTC on patient outcomes. The findings may thus have substantial impact in both low resource settings like India and high resource settings like Sweden.

Biomass smoke (BMS) exposure i...s a greater global risk factor for chronic obstructive pulmonary disease (COPD) compared to tobacco smoking (TBS). Women living in the rural areas of Low-and Middle-Income countries are disproportionately affected by BMS induced COPD as they cook for 4-6h/day using biomass fuel. Earlier lung was considered as sterile. With emerging evidence, the presence of normal lung microbiome and its role in maintaining homeostasis as well as in disease conditions is being realized. Alteration of normal lung microbiome may predispose individuals to chronic lung diseases. Changes in microbial diversity associated with asthma, cystic fibrosis, COPD, have been shown. However, limited information is available on lung microbial dysbiosis in TBS induced COPD patients and almost no information on lung microbiome of BMS induced COPD subjects is available at present. Hence, with a multi-national team, which includes researchers, clinicians and microbiologist we aim to compare the lung microbiome from induced sputum among BMS exposed women with COPD, BMS exposed women without COPD and healthy non-BMS exposed women (n= 50/group: 2021-early 2022). Molecular microbiological techniques such as high throughput amplicon sequencing of 16S rRNA genes will be used to analyze the data (Fall 2021-Fall 2022). We will initiate two-year continuous collaborative work between Sweden, India and Germany (2021-2022) with the aim of developing joint long-term research network and project.

Piloting multisectoral work ar...ound sexual and reproductive health, child health and non-communicable diseases in Uganda: The aim of the proposed collaboration is to develop a common research project examining the critical health challenges within the overall framework of the Sustainable Development Goals (SDGs) between institutions in Sweden and Uganda. Specifically, a multisectoral network for research will be created and three associated research questions on critical health priorities–which are believed to have the potential to enable synergetic progress across many SDG–swill be explored by teams of researchers from Karolinska Institutet and Makerere University:How can sexual and reproductive health and rights for the most vulnerable be effectively improved?Which strategies are successful in enabling a healthy start for a healthy childhood and minimising the risks for NCDs later in life?What factors characterise the risk for NCDs in low resource settings?The project-activities, which are planned to be implemented during 2021–2022, will utilise a range of state-of-the-art research theories and methods applied to concrete health challenges facing Uganda and the surrounding region. The collaborative research project at hand is seen as a unique cross-cutting initiative that builds on the synergies of the SDGs and addresses critical gaps in their delivery. By putting the health needs of the most vulnerable populations in the centre of our activities, this project is believed to contribute to a just, equitable, tolerant, open and socially inclusive world.

1) Distribute 6.48 million bed-nets to pregnant women for malaria
2) Treat 522,400 pregnant women for malaria
3) Provide 662,000 women with family planning
4) Distribute 173.5 million condoms
This will avert more than 36,000 malaria deaths, 16,765 HIV infections and 770,800 unwanted pregnancies
