Els antibiòtics han estat l'a...vanç més important de la medicina de tota la història. Actualment, la resistència dels bacteris en front els antibiòtics és una de les amenaces més greus per la Salut Pública a nivell mundial. Un dels motius que potencien i acceleren l'aparició i propagació de bacteris resistents a antibiòtics és el mal ús d'aquests per part dels humans, especialment els ramaders, els veterinaris i els metges. El present projecte determinarà la presència de bacteris resistents a antibiòtics a éssers humans, animals domèstics i a la fauna salvatge a zones rurals d'Uganda. Els bacteris analitzats seran tres agents que afecten greument la salut de les persones com son Salmonella, E. coli i Campylobacter. S'analitzarà la seva presència a animals salvatges, domèstics i persones de zones geogràfiques amb característiques ecològiques diferents on interaccionen entre ells. Paral·lelament es realitzaran enquestes a ramaders i veterinaris de les zones d'estudi per determinar els antibiòtics i les pautes que fan servir. Amb la informació generada al laboratori i a les enquestes, es definirà i impartirà un pla de formació per ramaders i veterinaris locals per millorar l'ús dels antibiòtics a aquestes zones.

La caza de animales salvajes p...ara el comercio ilegal de carne amenaza la biodiversidad y aumenta el riesgo de transmisión de enfermedades zoonóticas. En Uganda hay información limitada sobre las especies objeto de la caza furtiva, los patrones de contacto con las comunidades que practican la caza y con los puntos de venta de esta carne. El objetivo principal de estos proyectos es investigar las especies de animales salvajes que son cazadas y son puestas a la venta ilegalmente, con el riesgo potencial de transmisión de enfermedades. Por eso se seleccionarán diversos puntos de muestreo alrededor de las zonas protegidas (Parques Nacionales) y se obtendrá muestras de carne de diferentes establecimientos y carreteras donde se proporciona este tipo de comida a los viajeros que pasan. Mediante el estudio de ADN se determinará la especie animal a la que pertenece una muestra de carne. Además, se realizará un estudio serológico de las enfermedades de la fauna salvaje a partir del banco de muestras de suero de la Uganda Wildlife Authority (UWA) para identificar los riesgos de exposición de la población a diversos agentes infecciosos.

Enterotoxigenic Escherichia co...li (ETEC) and Helicobacter pylori (Hp) are common infections in low- and middle-income countries (LMIC). ETEC gives rise to watery diarrhea, causing 400 000 deaths/year, primarily in young children. LMIC children are also often chronically infected with Hp and in 15-20% of cases, the infection causes symptoms later in life such as peptic ulcers or gastric cancer. Our recent results suggest that CD4+ T helper cells, particularly those that secrete the cytokine IL-17A (Th17), play important roles in promoting protective immune mechanisms in both infections. The overall aim of this project is to advance the knowledge we have recently gained on Th17 responses in different model systems to human ETEC and Hp infection. This includes (I) to characterize T helper cell responses to these infections in humans and (II) to study the functional capacity of novel mucosal vaccine adjuvants to enhance human CD4+ T helper cell responses. This collaborative project, performed at the University of Gothenburg and the International Center for Diarrhoeal Disease Research, Bangladesh, will benefit from the combined expertise of the applicants, including broad knowledge in techniques for analyzing T cell responses and extensive experience of studies of mucosal immunity in LMIC. The project also aims to establish a framework for mutual learning and training of PhD students and younger scientists at both sites, forming a basis for extended future collaboration.

Waterborne diarrheal diseases ...remain a major threat to the youngest population in low- and middle-income countries. Recently we have seen how crowded refugee camps, flooding and earth quakes may result in rapid spreading of diarrhoea and other water borne infectious diseases. Hence we need to understand how water borne diarrhoeal pathogens can emerge and spread so rapidly. The aim of this project is to understand how pathogenic bacteria survive in water and how water transmission affects virulence and acquired antibiotic resistance. The hypothesis is that bacteria in water transiently actively regulate genes that aid in infection of new hosts but that long term water incubation select for antibiotic resistant dormant bacteria that still may be infectious. Determination of the transcriptome in enterotoxigenic Escherichia coli (ETEC) in water by Illumina RNA-seq analyses revealed active gene regulation and indicated adaptation towards antibiotic resistant so called persister cells We aim to verify essential genes required for survival by advanced sequencing methods as well as determine if water is a inducer for uptake of naturally occurring antibiotic resistance genes as well as increased infecitvity. The project is a collaboration with Bangladesh and Bolivia and includes local capacity building by PhD training of Bolivian students. The aim is to identify means to predict and prevent epidemic outbreaks of gastrointestinal infections transmitted via water.

In Brazil, there are 600,000 c...ases of malaria each year. An approach for fighting malaria is to transform mosquito symbiotic bacteria to produce effectors against the malaria parasite, a.k.a. paratransgenesis. We have previously isolated a new family of bacteria, Thorsellia, which is uniquely associated with malaria mosquitoes, and by DNA sequencing shown its existence in Brazil. To know more about the biology of this mosquito symbiont and its potential for paratransgenic approaches against malaria, we aim to isolate Thorsellia spp. from Brazil and investigate its relative abundance in mosquitoes and their breeding waters. In addition, we will investigate the survival and stability of Thorsellia in the Anopheles mosquitoes. A deeper understanding of Thorsellia and its relation to the mosquitoes is a prerequisite for fruitful paratransgenesis. In Brazil this system is optimal since the most important malaria mosquito cannot mate in captivity for a transgenic approach and thus is restricted to paratransgenesis. In the project, researchers from Sweden will go to Manaus for on-site development of sampling techniques. Students from Brazil will go to Sweden for development of transformed Thorsellia bacteria that can be followed in the malaria mosquitoes and to learn more about techniques in microbiology. For both research teams the current proposal would serve as a starting point with a common goal, the development of paratransgenic malaria mosquitoes.

Malaria, caused by parasites o...f the genus Plasmodium, is a global health concern, killing half a million humans yearly. Malaria puts a heavy burden on the economic progress of the afflicted countries, especially in the low-income strata of those societies. Despite the severity of the problem, global efforts to combat malaria remain minor, allegedly since this disease mostly affects developing countries, with limited economic power. No efficient malaria vaccine has been developed, and resistance existing antimalarial drugs is emerging. However, with the recent support of international charities, this may change. This project aims at identifying novel promising lead antimalarial compounds, with a special view to combat P. vivax, the major malaria-causing parasite in Latin America. Frontline techniques in yeast genomics will be used to identify the targets of molecules with antiplasmodial activity, and to improve the selectivity for the parasite ortholog of the target over the human counterpart. The Brazilian partners contribute knowledge about growth and genetic engineering of the malaria parasite, as well as of its pathogenesis and malaria epidemiology. The Swedish partners contribute high-throughput yeast genetic screening methodology and medicinal chemistry for redesign and development of leads. We intend to establish a long-lasting international collaboration in this field of great global importance.

A global research goal is to f...ind ways to treat TB effectively, as the long treatment (6-9) months leads to multi-drug resistance. Antibiotic efficacy displays a biphasic course in TB, with a rapid reduction in bacterial load followed by a steady-state phase due to antibiotic-tolerant bacteria. Our data show that macrophage immunity causes Mtb to reduce replication, making it tolerant towards antibiotics. We will investigate our idea that immunosuppressive (IS) treatment given to macrophages prevents them from triggering Mtb-tolerance. Hypotheses: 1) Latent TB infection is maintained by macrophages causing increased tolerance towards antibiotics. 2) IL-1â and TNF-á stimulate antimycobacterial functions of the macrophage, including autophagy antimicrobial peptides and NO. 3) IS of macrophages results in antibiotics-sensitive Mtb, thus a combination of IS and antibiotics accelerates microbial clearance. We will define how Mtb alters when exposed to macrophage immunity and what host factors trigger phenotypic shift. We will use our unique TB-models with state-of-the art techniques to evaluate how macrophages are optimally modulated to kill bacteria in combination with antibiotics. Identified candidate drugs will be further explored in other models. The unique goal of the present bench-to-bedside project is to identify a strategy for combination therapy based on existing drugs that reduces the time required for TB treatment, thus minimizing drug resistance.

Campylobacter is one of the mo...st common bacterial zoonotic pathogens worldwide. In Egypt, previous research reported an incidence of Campylobacter diarrhea of 0.6 episodes per person-year, which was concluded as the highest incidence rate ever reported from a developing country. One complication associated with Campylobacter diarrhea is Guillain-Barre' Syndrome (GBS), an acute, symmetric, ascending paralysis. Recent Egyptian data suggested that GBS cases were primarily in the young aged and cases and many controls had a history of infection to a variety of Campylobacter serotypes. In hyperendemic settings such as in Egypt, the socio-economic burden of illnesses associated with Campylobacter is substantial. In this project we aim to integrate microbiological surveillance data, sequence-based subtyping, and probabilistic risk assessment modeling tools together to investigate the molecular epidemiology of Campylobacter in the broiler meat, the most important source of human campylobacteriosis, and to quantitatively assess the public health burden of human campylobacteriosis arising from consumption of broiler meat in Egypt. The research approach of this project could be adopted as a model for investigating other zoonotic pathogens in developing countries. The project fund will be directed toward enhancing personnel capacities and institutional collaboration between food safety researchers from Alexandria University in Egypt and the Swedish Agricultural University in Sweden.

Health care associated infecti...on (HCAI) is a major public health problem associated with increased hospital stay, cost of therapy and mortality. Transmission of HCAIs from patients, visitors and staff may lead to outbreaks and spread to the community. Treatment of HCAI is complicated due to high prevalence of antimicrobial resistant pathogen and health-care facilities can act as amplifiers of the outbreaks. Previous studies in Ethiopia indicated up to 59% of patients attending the surgical ward at Tikur Anbessa University Hospital (TAH) acquired HCAI. Antimicrobial resistance and HCAIs is a major problem in Ethiopia. The present study aims to perform the first controlled intervention testing in the country through capacity building & knowledge transfer for prevention of HCAIs and antimicrobial resistance. A three-phase prospective interventional study will be conducted at TAH, Addis Ababa, Ethiopia. First baseline survey will be done to identify prevalence, severity, etiology, spread pattern, risk factors for HCAIs, antibiotic use &, resistance, knowledge and attitudes of healthcare staff. In the 2nd phase, educational interventions will be introduced based on the gap and problems identified. In the 3rd phase we will evaluate the impact of intervention. The study will provide awareness, competence, quality control strategy and evidence based recommendations to establish better guidance for early detection of HCAIs of outbreaks and preparedness to handle threats of epidemics

Pneumococcal infections are th...e leading cause of death among children below 5 year worldwide.Ethiopia is among the 10 countries with the highest disease burden. A pneumococcal conjugate vaccine (PCV) has given to all children in Ethiopia since 2011, but the effect on the disease is not known. The good response seen in USA and Europa may not be repeated because of diferences in serotype distribution and virulence among strains of some serotypes. We plan to investigate the serotype distribution and resistance among pneumococci from children with clinical infections and asymptomatic carriers. Clinical data and level of antibiotic resistance will be related to the serotypes in available vaccines. Indoor air pollution has been found to be a leading cause of morbidity where the food is cooked on open fire with insufficient ventilation. The children in the houses are exposed to smoke, which is likely to be a risk factor for respiratory infections. The air quality will be determined by measuring the carbon monoxide (CO) concentration since CO is a known tracer of incomplete combustion. However, the relation between carbon monoxide levels, airway inflammation and bacteriology in children has not been studied. 100 children living in homes where the food is cooked indoors on open fire without chimney will be compared with 100 children from homes with other forms of cooking. We also plan to study the effect of an intervention with a sun-powered fan.
