Recent outbreaks of the highly... infectious and dangerous Ebola virus in West and Central Africa underscore the importance of rapid diagnostics and surveillance infrastructure, evidence-driven health communications and community engagement activities, and an effective and well-coordinated emergency response to strengthen the resilience of vulnerable populations in the face of emerging pandemic threats. While vaccines and medical therapies have become important tools for the treatment and control of infectious diseases, greater efforts are needed in West and Central Africa to strengthen gaps in the technical and logistical capacities of researchers, surveillance agents, diagnosticians, first-line responders, and public health systems at large to quickly and effectively work with communities to identify, halt, and prevent new pandemics. The aim of this initiative is to implement an integrated research and capacity-strengthening program tailored for Central and West Africa. This initiative will support research to identify and assess the role of socio-cultural, animal health, and public health interventions to control and prevent emerging pandemic threats in the region. Research findings will help to inform the design and implementation of the capacity strengthening activity that will focus on enhancing and adapting the practices, protocols, and policies of public health actors in conducting surveillance, public health outreach and communications, and outbreak management of emerging pandemic threats. The research team will collaborate with African public health and research institutes to build their existing expertise in surveillance, research, and public health response and will provide a foundation for greater inter-regional scientific collaboration on pandemic threats that pose significant threats to the health and livelihoods of millions of people in Central and West Africa.

The 2013-2015 outbreak of Ebol...a virus disease (EVD) in West Africa (Guinea, Liberia, and Sierra Leone) was unprecedented, resulting in more than 11,000 human deaths with an estimated total cost of US$4.3 billion. The outbreak highlighted the need for a greater understanding of the social, economic, cultural, and physical environment factors that are essential to improving EVD response capacities and decreasing the risk of further outbreaks. This project aims to improve understanding of social and environmental factors as precursors for increased vulnerability to EVD outbreaks in susceptible communities. It also seeks to apply this knowledge to mitigate the outbreak potential of EVD and to improve response strategies to contain viral transmission and the spread of disease using community-based initiatives. This research is expected to generate new information on vulnerabilities associated with gender, age, and cultural association that will help to improve prevention and response capacities to EVD at the community, local, country, and international levels.This project is funded by the Rapid Research Fund for Ebola Virus Disease Outbreaks, a CA$1.5 million initiative that was mobilized and co-funded by IDRC, the Canadian Institutes of Health Research, and the Social Sciences and Humanities Research Council of Canada. The Fund supports four collaborative projects jointly led by Canadian and African researchers.

This initiative supports phase...s 2 and 3 clinical trials of an experimental Ebola vaccine. The experimental vaccine is based on an attenuated recombinant Vesicular Stomatitis Virus vector (VSV-EBOV). The Public Health Agency of Canada developed the vaccine and licensed it to NewLink Genetics and Merck. Early vaccine trials During the multicentre phase 1 trials, researchers demonstrated the vaccine's safety and its ability to provoke an immune response (immunogenicity). The next trials will determine the expanded safety, protective immune response, and efficacy of the vaccine with at-risk populations in Africa. The trials will allow African teams to strengthen their clinical research capacities. The initiative is supported through a partnership of Foreign Affairs, Trade and Development Canada, the Canadian Institutes of Health Research, IDRC, and the Public Health Agency of Canada. Phases 2 and 3 clinical trials The phase 3 clinical trial of the VSV-EBOV vaccine will take place in Guinea, one of the main Ebola-affected countries in West Africa. An international research consortium headed by the Norwegian Institute of Public Health is leading the trials. The World Health Organization is coordinating the consortium. The clinical trial will determine the safety, protective immune response, and efficacy of the vaccine to prevent infection and the spread of the disease. Working with colleagues from Mali and Senegal, Canada will provide support for field monitoring, trial assessment, and developing Guinean clinical research capacities. The Centre hospitalier universitaire Sainte-Justine will host and chair the Data Safety Monitoring Board, an essential validating and transparency function for the vaccine trials. Canada will co-chair the Scientific Advisory Group for the Guinea Ebola vaccine trials. The partnership is discussing support for additional phase 2 clinical trials of the VSV-EBOV vaccine. These would take place in Canada and one at-risk African country not affected by the current epidemic. These trials would be performed in partnership with other donors. This will help create strong evidence of the vaccine's safety and protective immune response in broader populations, including children and HIV-positive people. Results from the project activities will be shared with the international community as part of the global response to the Ebola crisis. The results will inform the approval of the VSV-EBOV vaccine for future use to prevent Ebola transmission.

August 1, 2018 - A 10th outbre...ak of the Ebola virus disease was declared in North Kivu and Ituri provinces of the Democratic Republic of Congo (DRC). North Kivu and Ituri are among the most populated provinces in the DRC. These provinces have been experiencing intense insecurity and a worsening humanitarian crisis, and over one million internally displaced people. The continuous movement of refugees to neighbouring countries, including Burundi, Tanzania and Uganda, represents a risk of spreading the outbreak to other provinces and neighbouring countries. With GAC's support, the International Federation of the Red Cross and Red Crescent Societies (IFRC) Appeal works to support Red Cross Societies' staff and volunteers to carry out relief operations focussed on epidemic control through: community-based surveillance and response mechanisms; safe and dignified burials; risk communication; social mobilization and community engagement; infection prevention and control; support to health facilities and communities; and psychosocial support. The project includes the DRC response, and preparedness activities in Burundi, Rwanda, South Sudan and Uganda. Project activities include: (1) establishing and maintaining infection prevention and control measures in health facilities; (2) providing psychosocial support to Ebola patients and their families; (3) conducting advocacy and community outreach on Ebola prevention, detection and control; and (4) advocating for access by humanitarian personnel to affected and at-risk communities. /1er aout 2018 - Un 10e foyer du virus Ébola a été déclaré dans les provinces du Nord-Kivu et de l'Ituri de la République démocratique du Congo (RDC). Le Nord-Kivu et l'Ituri sont parmi les provinces les plus peuplées de la RDC. Le niveau d'insécurité est très élevé dans ces régions, qui sont difficiles d'accès, alors que la crise humanitaire s'aggrave et que l'on compte plus d'un million de personnes déplacées à l'intérieur du pays. Le mouvement continu de réfugiés vers les pays voisins, notamment l'Ouganda, le Burundi et la Tanzanie, représente un risque de propagation de l'épidémie à d'autres provinces et pays voisins. The provinces have been experiencing intense insecurity and a worsening humanitarian crisis, with limited access and over one million internally displaced people. The continuous movement of refugees to neighbouring countries, including Uganda, Burundi and Tanzania, represents a risk of spreading the outbreak to other provinces and neighbouring countries. With GAC's support, Doctors Without Borders/Médecins Sans Frontières (MSF) is helping to meet the immediate medical needs of persons infected by or at risk of contracting Ebola in the affected health zones. MSF specializes in emergency health services and is the lead on Ebola treatment in the DRC. Project activities include: (1) increasing access to quality primary healthcare and isolation centres for women, girls, men and boys living in the Ebola affected health zones; (2) increasing awareness about prevention measures and availability of treatment, through affected communities; (3) improving surveillance, case management and infection management and control; and (4) raising awareness and immunization of at-risk groups. /Octobre 2019 - Un 10e foyer de la maladie à virus Ebola a été déclaré dans les provinces du Nord-Kivu et de l'Ituri en République démocratique du Congo (RDC) le 1er août 2018. The initiative will contribute... to Canada's coordinated response to the Ebola outbreak in eastern DRC. The project will help strengthen or rebuild the weak health system in 22 zones at risk for the Ebola, not only to prepare them for the possible spread of the disease, but to increase supply, demand and quality of health services for vulnerable populations, including girls and women, living in this conflict zone. This development project will work in coordination with humanitarian efforts in the DRC. / L'initiative contribuera à la réponse coordonnée du Canada à l'épidémie d'Ebola dans l'est de la RDC. This project proposes suppleme...nting the response to Ebola through stabilization and peacebuilding efforts. The project aims to help contain the spread of Ebola, and reduce the community's violent resistance and mistrust of the Ebola response in North Kivu, Democratic Republic of Congo (DRC). The project also aims to build trust between communities and non-medical state actors, such as elements of the police, loyalist members of the DRC armed forces, administrative officials and media leaders who play a key role in response activities. Interventions such as community dialogues and participatory theatre work to bring together key stakeholders, including women and youth, and reduce the security dilemma in favour of local government, thereby enhancing existing stabilization interventions. These activities take place in Butembo and Beni, North Kivu, where most of the Ebola crisis has spread and where community engagement can strategically support the Ebola response. / Ce projet propose de compléter la réponse à Ebola par des efforts de stabilisation et de consolidation de la paix. Le projet vise à aider à contenir la propagation d'Ebola et à réduire la résistance violente de la communauté et sa méfiance à l'égard de la réponse à Ebola dans le Nord-Kivu, en République démocratique du Congo (RDC). Le projet vise également à établir la confiance entre les communautés et les acteurs étatiques non médicaux, tels que les éléments de la police, les membres loyalistes des forces armées de la RDC, les fonctionnaires administratifs et les responsables des médias qui jouent un rôle clé dans les activités de réponse. Les interventions telles que les dialogues communautaires et le théâtre participatif visent à rassembler les principales parties prenantes, notamment les femmes et les jeunes, et à réduire le dilemme de la sécurité en faveur du gouvernement local, renforçant ainsi les interventions de stabilisation existantes. February 2020 - The crisis in ...the Democratic Republic of the Congo (DRC) is characterized by inter-ethnic conflict and widespread human rights abuses, including sexual violence. In 2020, 15.6 million people are in need of humanitarian assistance in the DRC, a 22% increase from 2019, and 2020 expects additional population movement, creating significant needs in protection, livelihoods, and food security. Currently, the DRC is home to the second largest food crisis, with almost 16 million people severely food insecure. In addition, it is home to the largest internally-displaced persons (IDPs) population in Africa with 5.01 million displaced people. These needs are exacerbated by the ongoing Ebola outbreak, increased violence, and the forced return of hundreds of thousands of Congolese refugees. With the support of GAC and other donors, the World Health Organization's emergency operations aim to rapidly assess the threat of infectious disease outbreaks and initiate urgent activities to prevent and reduce morbidity and mortality caused by infectious disease outbreaks, and to prevent escalation of outbreaks. Project activities include: (1) deploying experts to rapidly assess outbreaks, (2) delivering essential medications, medical supplies and equipment, (3) providing training to health personnel, (4) coordinating and implementing vaccination campaigns, and (5) strengthening health sector coordination and health information systems. / Février 2020 - La crise en République démocratique du Congo (RDC) est caractérisée par un conflit interethnique et des violations généralisées des droits de la personne, y compris des violences sexuelles. En 2020, 15,6 millions de personnes ont besoin d'aide humanitaire en RDC, soit une augmentation de 22% par rapport à 2019, et 2020 prévoit des mouvements de population supplémentaires, créant des besoins importants en matière de protection, de moyens de subsistance et de sécurité alimentaire. An international research consortium headed by the Norwegian Institute of Public Health is leading the trials. The World Health Organization is coordinating the consortium. This initiative supports phase...s 2 and 3 clinical trials of an experimental Ebola vaccine. The experimental vaccine is based on an attenuated recombinant Vesicular Stomatitis Virus vector (VSV-EBOV). The Public Health Agency of Canada developed the vaccine and licensed it to NewLink Genetics and Merck. Early vaccine trials During the multicentre phase 1 trials, researchers demonstrated the vaccine's safety and its ability to provoke an immune response (immunogenicity). The next trials will determine the expanded safety, protective immune response, and efficacy of the vaccine with at-risk populations in Africa. The trials will allow African teams to strengthen their clinical research capacities. The initiative is supported through a partnership of Foreign Affairs, Trade and Development Canada, the Canadian Institutes of Health Research, IDRC, and the Public Health Agency of Canada. Phases 2 and 3 clinical trials The phase 3 clinical trial of the VSV-EBOV vaccine will take place in Guinea, one of the main Ebola-affected countries in West Africa.
