The number of COVID-19 cases is on the rise again, with South Africa nearing half of all confirmed cases in the WHO African Region. Threats of new variants loom and low vaccination coverage raises questions on the future of the response to COVID-19. Prevention remains the key strategy in most sub-Sa...haran countries. Five National Centres (NCs) from the African Health Observatory Platform on Health Systems and Policies (AHOP), based in Ethiopia, Kenya, Nigeria, Rwanda and Senegal, reflect on lessons to be learnt from their containment responses in the initial phases. They construct timelines to highlight the policies and challenges associated with introducing a range of public health containment measures and
discuss the extent to which these measures continue to be valuable given the ever-changing nature of the pandemic.

RESULTS: Between 76 and 97% of the PHCS offered RMCAH services before the lockdown. Except in antenatal, delivery and adolescent care, there was a decline of between 2 and 6% in all the services during the lockdown and up to 10% decline after the lockdown with variation across and within States. Dur...ing the lockdown. Full-service delivery was reported by 75.2% whereas 24.8% delivered partial services. There was a significant reduction in clients' utilization of the services during the lockdown, and the difference between States before the pandemic, during, and after the lockdown. Reported difficulties during the lockdown included stock-out of drugs (25.7%), stock-out of contraceptives (25.1%), harassment by the law enforcement agents (76.9%), and transportation difficulties (55.8%). Only 2% of the PHCs reported the availability of gowns, 18% had gloves, 90.1% had hand sanitizers, and a temperature checker was available in 94.1%. Slightly above 10% identified clients with symptoms of COVID-19.
CONCLUSIONS: The large proportion of PHCs who provided RMCAH services despite the lockdown demonstrates resilience. Considering the several difficulties reported, and the limited provision of primary protective equipment more effort by the government and non-governmental agencies is recommended to strengthen delivery of sexual and reproductive health in primary health centres in Nigeria during the pandemic.

Community health worker teams are potential game-changers in ensuring access to care in vulnerable communities. Who are they? What do they actually do? Can they help South Africa realize universal health coverage? As the proactive arm of the health services, community health workers teams provide ho...usehold and community education, early screening, tracing and referrals for a range of health and social services. There is little local or global evidence on the household services provided by such teams, beyond specific disease-oriented activities such as for HIV and TB. This paper seeks to address this gap.

Avec l’augmentation soudaine du nombre d’hospitalisations causées par la pandémie de COVID-19 en Amérique latine et dans les Caraïbes, et étant donné la rareté des ressources humaines et matérielles telles que les équipements médicaux et les gaz médicaux, il est devenu nécessaire de ...repenser les modèles de soins dans la Région dans le but d’optimiser l’utilisation de ce qui est disponible et de s’assurer qu’un plus grand nombre de patients recevront l’oxygène dont ils ont besoin en quantité et en qualité. L’oxygène est inclus dans la liste des médicaments essentiels de l’Organisation mondiale de la Santé et sert à soigner les patients à tous les niveaux des réseaux de services de santé intégrés. L’efficacité de l’utilisation de l’oxygène est démontrée dans le traitement de patients atteints de maladies respiratoires causées par la COVID-19, et cette efficacité pourra être grandement améliorée si on encourage le recours rationnel, durable et sûr à cette pratique.

La pandemia de covid-19 ha afectado la detección y tratamiento de pacientes con TBC en Paraguay que lejos de ser una enfermedad erradicada, es motivo de preocupación sanitaria y sigue afectando a miles de ...personas, si bien se inician actividades de campo con apoyo del PNCT para la búsqueda activa, seguimiento de casos y control de contactos en comunidades indígenas y asentamientos, estas tareas no son continuas debido al aumento de los casos de covid-19 en el país, que favorece el temor a acudir a los hospitales para el diagnóstico, a sus controles e incluso se ve afectado el retiro de los medicamentos. En el marco de la pandemia, es probable que aumenten los fracasos terapéuticos con el consiguiente desenlace fatal, por lo que debe hacerse una revisión de las estrategias dirigidas a poblaciones vulnerables con un compromiso verdadero del sector político, sumado a una fuerte colaboración multisectorial e intercultural para lograr el fin de la TBC
