Health Sector Reform Project
|
The Health Sector Reform Proje...ct, will increase access to sustainable quality of health care services, particularly in the areas of rehabilitation, and prevention, within the framework of the health program in Lesotho. The components will: 1) improve financial management, and information systems, through training in accounting procedures, formulation of reporting formats, and responsiveness to information requirements in the Ministry of Finance on health planning, and statistics, and, includes the procurement of supportive information technology. Regional training will be provided, to attain a restructured, and comprehensive integrated budget, within a medium-term expenditure framework; 2) establish procedures, responsibilities, and skills for procurement planning, and implementation within the Ministry of Health and Social Welfare, and, provide technical assistance through training, addressing monitoring, and evaluation of financial, programmatic, and impact indicators; 3) develop a strategic human resources plan, to rationalize staff deployment, and recruitment/retention of health workers; 4) coordinate donor support to acquire expertise regarding annual work-plans, and budgets among developing partners; 5) include analytical activities in planning for Phase II implementation plan; and, 6) support HIV/AIDS prevention, and control policy.

Roads and Bridges Management a...nd Maintenance Project

The Roads and Bridges Manageme...nt and Maintenance Program Project will stimulate economic growth and contribute to poverty reduction through improved road infrastructure, better sector policies, and enhanced roads sector management. The Specific objectives of this project will improve the coverage and condition of roads and bridges; strengthen the country's capacity to manage and administer the road sector effectively and transparently, with efficient and sustainable institutional arrangements; establish finance mechanisms to ensure sufficiently, timely, stable, and secure flow of funds for roads maintenance, affordable at the macroeconomic level and for the road users; and improve road transport safety. The project consists of three components: 1) The civil works component will support roads routine and perodic maintenance, roads rehabilitation, rural roads civil works, emergency works, road safety, and engineering services for road works. 2) The second component will establish an institutional framework for the sector to ensure transparent roads management and administration and support policy reform. 3) The third component will support strategy and preparatory activities for Phase II, which involve revising and updating of the national integrated road sector strategy and preparing environmental impact assessment and social impact assessments for Phase II.

1) Capacity Development (Appra...isal: US$30.56 million; Appraisal + AF: US$50.05 million; Actual: US$70.3 million):
This component aimed to support the newly established National and State HIV /AIDS Program Teams, which acted
as the implementing arms of the National Action Committee on HIV /AIDS and the State Action Committees on
HIV/AIDS. (At the national level, the Committee was responsible for setting priorities for action and financing,
approving action plans for federal Ministries and national civil society organizations, and monitoring the overall
HIV/AIDS situation, while the State Committees performed the same functions for state -level entities.)
Responsibilities of the national and state teams included evaluating specific action plans proposed by sector
ministries and NGOs; monitoring implementation of the HIV/AIDS Emergency Action Plan and the overall HIV/AIDS
situation in the country; and acting as knowledge manager of HIV /AIDS information. Activities included provision of
staff and technical support to teams; training for committees; and financing of consultant contracts for M&E and
knowledge activities.
2) Expanding the Public Sector Response (Appraisal: US$31.25 million; Appraisal + AF: US$39.0 million; Actual:
US$11.45 million): This component aimed to support HIV/AIDS activities to be carried out by sector ministries and
government agencies. Three ministries were to be funded in the first year of the project - Health, Education, Defense
– with at least ten funded by the third year including Agriculture and Rural Development, Information, Internal Affairs,
and Women’s Affairs. Higher funding amounts were set for the Ministries of Education, Defense, Internal Affairs,
and Health. The Ministries of Health, Education and Defense had already prepared action plans, which included
activities targeted at both staff and clients (including high risk populations.)
3) The HIV/AIDS Fund (Appraisal: US$33.87 million; Appraisal + AF: US$50.6 million; Actual: US$30.35 million):
This component aimed to support the preparation and implementation of NGO, community -based organization
(CBO), private sector, and community HIV/AIDS programs. Activities included technical assistance, training, and
program implementation. The implementing groups were to focus on targeting high risk groups (such as truck

Based on the development objec...tive of the HPDP-2, the three components delineated for
the project. are: i) Expanding the Public Sector Response; ii) Expanding the Civil and
Private Sector Engagement and Response Through the HIV/ADS Fund (HAF); and iii)
Strengthening Mechanisms For Project Coordination And Management at all levels. In
recognition of the mixed epidemic in Nigeria, a mixed response approached is being
used to address and scale up prevention, care and impact mitigation among high risk
groups and the general population that will be identified through National and Statespecific
high risk mapping and epidemiological analysis.
The main essence of HPDP-2 is to progress towards a more strategic response, based
on the commissioning of new epidemiological evidence, that will be used to further
underpin strategic direction of programming within all components. To ensure that the
response is strategic and evidence-based, the development objective and project
components were derived from experiences and lessons from HPDP-1; and based on

prevention); (ii) childhood essential medicines and increasing treatment of
important childhood diseases (Community

supplementation); (iv) immunization (Routine

objective is to dramatically improve the coverage of these interventions that currently suffer

Introduction of Nutrition Serv...ices. It will support introduction of preventive nutrition
services in all 36 districts by scaling up the provision of micronutrients and deworming (see
details in Annex 2) and expanding the delivery of behavior change communication, focusing
on the prevention of malnutrition during pregnancy and in the first two years of life. In 12
districts and urban slums in 9 cities with the highest prevalence of acute malnutrition, therapeutic nutrition services will be delivered, including commodity support micronutrients
and Ready-to-Use Therapeutic Foods. The project will improve the capacity of health service
providers to deliver nutrition interventions at the facility and community level and
coordinating structures to implement multi-sectoral response to address nutrition.
iii) Expanded Coverage of HIV/AIDS Preventive Services. The component will support
expanding coverage of preventive, treatment, and care services for population subgroups
vulnerable to HIV infection [IDUs and Men Who Have Sex with Men (MSM)] in targeted
cities; and support provincial integrated biological and behavioral surveillance. Service
provision will be contracted out through a competitive process complementing the Global
