Component 1: District Service ...Delivery (Total: US$35 million: US$20 million IDA Additional Financing
(AF) and HRITF US$15 million). In the 26 districts currently covered by the Project and the eight
additional departments where PBF will be scaled-up, this component will provide PBF payments: (i) to
health facilities in the targeted regions conditional on the quantity and quality of services delivered via inclinic
activities and/or via health-outreach activities, and (ii) to community health workers for providing
selected basic health services as well as ensuring community organization to support positive health
behavior. Contracted health facilities will use PBF payments to (i) increase the quality and the quantity of (a) health
services provided at the facility level and (b) health services provided at the community level to women and
children (in collaboration with community health workers) through outreach campaigns; and to (ii) provide
financial incentives to health facility staff based on performance achieved.
Community health workers who are sub-contracted by health facilities with PBF contracts, will use PBF
payments to (i) increase the quantity and the quality of (a) selected basic health services for common
illnesses such as malaria, acute respiratory infections and diarrheal illnesses, (b) outreach activities in
support to facility-based health workers, and (c) referrals of community members to health facilities for
illness episodes that cannot be treated at the community level; and to (ii) keep a certain percentage of the
payment as a bonus which will be based on their performance.
Out of the US$35 million for Component 1, US$4 million will finance national procurement and ex-post
verification activities (conducted by an independent third party, i.e. an external evaluation agency).
Component 2: Institutional strengthening (Total: HRITF US$5 million). The component supports
institutional strengthening at national, regional, and district levels, particularly focusing on PBF contract
design and management and establishing a unified health management and geographic information system
to generate up to date, reliable, financial and programmatic data. The Component also supports project

The Population and AIDS Contro...l Project will assist the Government in implementing its long-term strategy in population and family planning (FP) and its medium term plan 1995-1999 for AIDS control. Its overall objectives are to advance the onset of fertility decline by increasing the use of modern methods of contraception, and slow the spread of HIV infection by promoting behavioral change. To attain these objectives, the project will support policies and investments designed to: 1) reinforce the national capacity to implement the national population policy by a) strengthening the national capacity for undertaking population-related activities; b) implementing a community awareness program and dissemination of the population policy; and c) supporting population-related research; 2) strengthening the national capacity to contain the spread of HIV/AIDS/STDs by: a) institutional strengthening of the Ministry of Public Health; and b) supporting epidemiological, operational, and socio-economic research; 3) putting in place a social marketing program for the promotion of condoms; and 4) promoting the participation of the private sector and NGOs in population, FP, and HIV/AIDS/STD programs by setting up a social fund.

Components (or Key Conditions ...in the case of DPLs, as appropriate):
(a) Community-based school rehabilitation and development program (CSRDP) (US$7.70 million appraisal;
US$7.68 million actual) to carry out a community-driven school rehabilitation and development program through : (i)
provision of grants to Community Development Committees (CDCs) to finance the construction, rehabilitation and
equipment of school infrastructure under sub -projects identified and proposed by Community -Based Groups; (ii)
provision of specialized training, including in project cycle management to CDCs, prefectoral and communal
administrations; (iii) provision of technical advisory services to CDCs in the preparation of the education components
of Community Development Plans; (iv) carrying out of M&E activities involving the beneficiaries; and (v) the Forum
for African Women Educationists (FAWE) girls' secondary school was added during the restructuring in June 2006.
(b) School quality improvement program (US$16.11 million appraisal; US$13.09 million actual) to raise the quality of
education training through (i) improving the quality of teacher training by providing pedagogical support, staffing,
facility rehabilitation, equipment and completion of construction of science faculty at the Kigali Institute of Education;
(ii) capacity building in curriculum development; (iii) improving the availability of textbooks and pedagogical material;
(iv) strengthening the examination/assessment system; and (v) improving the quality teaching in science and
technology.
(c) HIV/AIDS (US$3.29 million appraisal; US$1.81 million actual) to promote behavioral changes required for
effective HIV/AIDS prevention through: (i) sensitization activities among ministry officials, teachers and students; (ii)
curriculum development and teacher training drawing from the experience of other countries with HIV /AIDS; (iii)
development of dissemination strategy; (iv) strengthening HIV/AIDS program management at the Ministry of
Education (MINEDUC); and (v) counseling, caring and testing targeted to teachers and students .
Activities (ii) - (iv) were cancelled and funds reallocated during the restructuring in June 2006.
(d) Capacity building program, studies and development of a Sector Wide Approach (US$6.56 million appraisal;
US$9.66 million actual) including (i) strengthening capacities for educational management and planning; (ii)
reinforcing the system of decentralization and general inspectorate with logistics, training, office equipment and
transport; (iii) focused training and staff development in key tertiary education institutions; and (iv) studies to support
the development of the SWAp in collaboration with DFID .

The Ninth Health Project for C...hina aims to 1) reduce maternal and child mortality and morbidity and improve child survival and development in the poorest areas of China; and 2) improve prevention and control of the Human Immunodeficiency Virus (HIV), Acquired Immune Deficiency Syndrome (AIDS), sexually transmitted diseases (STDs), and other blood-borne infections. There are three components. The first has five subcomponents: 1) It funds development and dissemination of clinical protocols and procedures, equipment, incremental costs, and health facility repair to help focus on providing systematic prenatal, obstetric, labor and delivery, and post-natal care; on improving well-child and new-born care; and on integrating sick child care for priority childhood illnesses. 2) It promotes pre-marital counseling and family health education materials. 3) It improves supervisory and monitoring support for maternal and child health services by funding management procedures and information systems. 4) It funds training including materials and equipment. 5) It provides a variety of poverty relief funds. The second component comprises four components, which improve and expand the policy environment to prevent and control STDs, improve interventions targeted at changing behavior and informing and educating the general public, improve the disease surveillance system, and improve management of blood transfusion services. The third component consists of project coordination and support.

The objectives of the Integrat...ed Health Services Development Project, within the framework of the government's National Health Development Plan, are to: 1) expand access to health services and improve their utilization; 2) make health services available on a sustainable basis; and 3) improve the ability of the Ministry of Public Health (MSP) to analyze health problems, and to formulate, monitor and evaluate policies, strategies and programs. The project components are as follows: 1) the minimum package of health services (PMA); 2) reproductive health; 3) health planning and management; and 4) information and health. The first two components - service delivery - include the following: (a) investment and start-up costs for PMA introduction in about 14 districts, including nutrition, pharmaceuticals, training, reproductive health, and information, education and communication services; and (b) an accelerated program of reproductive health services, including family planning and sexually transmitted diseases. The last two components - institutional strengthening activities - cover the following areas: (a) organization development of the MSP; 2) information systems, including epidemiological surveillance and continuous monitoring and feedback of user perspectives on services; 3) training of prescribers in use of generic drugs and strengthening of quality control laboratory services; 4) health care financing; 5) financial management; and 6) review of training programs and strengthening of personnel management.

The objectives of the Particip...atory Health, Population, and Nutrition Project for the Gambia are to improve quality in reproductive health services, in infant and child health services, in nutrition services for women of childbearing age, infants, and children; and in management and implementation of a family health program. There are four components. The first helps improve maternal health services, prevents unwanted pregnancies, and prevents and controls STDs/HIV (sexually transmitted diseases and the human immunodeficiency virus). The second component aids in integrating vertical programs to combat childhood diseases through introducing and implementing the integrated management of childhood illnesses (IMCI) approach. The third component supports nutrition policy formulation and institutional strengthening; as well as community and micronutrient approaches to improve the nutritional status of women and young children. The fourth component supports the management and implementation of a family health program by capacity building and policy development, upgrading and maintaining health infrastructure--rebuilding selected primary health care facilities, establishing a blood supply system, procuring equipment, maintenance support, a local fund initiative fund, and project management. Capacity will be built in the areas of cost recovery; information, education, and communication; monitoring and evaluation; and research application.

The third component, health sector responses to Sexually Transmitted Infections (STIs) and HIV/AIDs management, will provide resources to the sector for management manuals and other activities, including enhancement of the blood supply system and acquiring access to antiretroviral (ARV) therapy as well as general prevention and care. The three project components, ...with appraisal and final costs, were :
1. Improve Awareness of Population Issues and Promote Adequate Reproductive Health Practices (US$4.5
million/US$1.9 million). (i) Increased awareness of population and development issues through training
workshops and dissemination of messages; (ii) Increased promotion of positive and safer behavior with
information, education and communication activities for youth, and support for behavior change of young
parents through training programs; and (iii) Improved equity and access to reproductive health services for
vulnerable groups including behavior change toward female genital mutilation, using a flexible funding
mechanism for NGOs (Population Support Fund) to undertake grass roots activities .
2. Improve Quality and Utilization of Priority Reproductive and Child Health Programs (US$6.0 million/US$8.0
million) (i) Rationalize management of support for reproductive and child health services, providing
management training for national programs to execute decentralized priority reproductive health programs plus
immunization. Included construction and equipping of two regional pharmaceutical warehouses, contraceptive
supplies, and rehabilitation of three prefectoral directorates of health . (ii) Provision of contraceptives,
STD/HIV/AIDS and family planning services that are culturally acceptable and medically safe, through
constructing and equipping two model clinics and ten health posts, and rehabilitating 15 health centers together
with training, supplies, and rehabilitation of the national referral laboratory for HIV /AIDS, plus equipment for
regional laboratories for screening of HIV /AIDS; and (iii) Support for safe motherhood and child health services
through training of obstetricians and their teams, with construction of a maternity hospital, rehabilitation of
operating theaters, intensive care units and post operative wards in eight prefectures, and 10 maternity waiting
shelters, together with vehicles, drugs, and obstetrical kits . During implementation it was agreed to reduce the
number of health posts from ten to four; construct three additional health clinics for US$ 1.5 million; and
purchase 28 vehicles instead of 14.
3. Strengthening Institutional Capacity to Manage and Coordinate Population and Reproductive Health Programs (US$1.5 million/US$2.5 million). (i) Strengthen institutional capacities to better manage population
programs through data collection and dissemination at policy workshops; and (ii) National coordination of

The project has two components. The first delivers cost-effective interventions against HIV/AIDS by 1) targeting interventions for groups at high risk, specifically commercial sex workers, injecting drug users, migrant workers, men having sex with men, and STI (sexually transmitted infection) clinic attendees; 2) providing locally appropriate information, communication, and awareness campaigns; voluntary testing and counseling; and a strengthened infrastructure for blood safety and hospital and occupational precaution programs; and 3) establishing new sources of support for AIDS care in partnership with nongovernmental organizations. The second component finances institutional strengthening by enhancing planning, management, and implementation capacity at the national, state, and local levels; strengthens the leadership capacity of the health and family welfare ministry; conducts training; builds capacity for ongoing monitoring and supervision; supports operational research and research and development; and supports broad social mobilization and cooperation and information.

The Second National HIV/AIDS (...Human Immunodeficiency Virus and Acquired Immune Deficiency Syndrome) Control Project aims to implement selected parts of the second phase of India's national program, namely to reduce the rate of growth of HIV infection in India; and to strengthen India's capacity to respond to HIV/AIDS. The project has two components. The first delivers cost-effective interventions against HIV/AIDS by 1) targeting interventions for groups at high risk, specifically commercial sex workers, injecting drug users, migrant workers, men having sex with men, and STI (sexually transmitted infection) clinic attendees; 2) providing locally appropriate information, communication, and awareness campaigns; voluntary testing and counseling; and a strengthened infrastructure for blood safety and hospital and occupational precaution programs; and 3) establishing new sources of support for AIDS care in partnership with nongovernmental organizations. The second component finances institutional strengthening by enhancing planning, management, and implementation capacity at the national, state, and local levels; strengthens the leadership capacity of the health and family welfare ministry; conducts training; builds capacity for ongoing monitoring and supervision; supports operational research and research and development; and supports broad social mobilization and cooperation and information.

The project aims to support th...e GOI in achieving its goal of halting and reversing the HIV/AIDS
epidemic by 2011 through integration of prevention and care, support and treatment programs. This will
be achieved through four main strategies of the program:
Preventing new infections in high risk groups and vulnerable populations;
Increasing the proportion of persons living withHIV/AIDS receiving care, support and treatment;
Strengthening the infrastructure, systems and human resources in prevention and treatment
programs at district, state and national levels;
Establishing a nation-wide strategic planning, program management, monitoring and evaluation

The development objective stat...ed above would be achieved through Central assistance for States
and Union Territories which would support the following components:
i) provision of TB treatment through the general health services of states and union
territories and developing collaborative systems for provision of TB services to those
accessing the HIV/AIDS program to ensure equitable access to all.
ii) communications campaign to create demand for TB services, sponsor community
mobilization, reduce stigma, support institutional reform and advocate for TB control.
iii) private sector participation to increase early case detection and extend the reach of
TB control services while widening patient choice; and providing TB services in

The objectives of the Sexually... Transmitted Infections Project are to: strengtehen institutional capacity at the national and district levels to design, implement, monitor and evaluate interventions; to promote preventive measures to reduce the risks of sexually transmitted infection (STI) transmission; and to enhance both health sector and community provision of physical and psychological care and develop strategies to mitigate the socioeconomic consequences. The medium term plan identifies the following strategies and components to address these objectives: 1) prevention of sexual transmission of HIV through syndromic case management and the provision of standardized drug kits, promoting safer sexual behavior and promoting and providing condoms through primary level health facilities; 2) prevention of transmission through blood and blood products; 3) mitigation of socioeconomic impact by the treatment of turberculosis, providing drugs for opportunistic infections and protective supplies for government and non-government district health facilities and training health provider staff, and supporting district based nongovernmental and Community Based Organizations (CBO) home-based health care and counseling for HIV/AIDS patients; 4) epidemiology and surveillance; 5) coordination of research; and 6) management and coordination of a multi-sectoral AIDS control program.
