Health facilities in the Region of the Americas frequently suffer the effects of health emergencies and disasters, which jeopardize their ability to provide services to the population. The STAR-H methodology helps staff responsible for health emergency and disaster risk management to identify and as...sess risks as part of strategic planning to improve facility preparedness. It is intended to help them develop, with a multi-hazard approach, a response framework with operating procedures to deal with hazards of any type, scale, or frequency; determine roles and responsibilities; facilitate the effective use of resources; undertake strategic planning exercises, and improve the preparedness of facilities to effectively respond to and recover from impacts. This methodology is designed for use in health facilities of any size and capacity, and makes it possible to generate historical reports and national or subnational risk profiles. This information can be used to develop an effective health emergency and disaster risk management program.

Chagas disease (American trypanosomiasis) is endemic in 21 countries of the Americas, where control is largely focused on elimination of the domestic insect vectors (Triatominae) coupled with measures to extend and improve the screening of blood donors in order to avoid tranfusional transmission. Th...rough national programmes and multinational initiatives coordinated by WHO-PAHO, much has been accomplished in these domains in terms of reducing transmission. Attention now turns to consolidating the successes in interrupting transmission, and improved treatment for those already infected and those who may become affected in the future. This article, based on technical discussions at the "Epidemiological and Sociological Determinants of Chagas Disease, Basic Information to Establish a Surveillance and Control Policy " meeting in Rio de Janeiro, is designed to open the debate on appropriate strategies for continuation of the successful initiatives against Chagas disease.

A framework to implement the Agenda for the Americas on Health, Environment, and Climate Change 2021–2030
Introdução: Em março de 2020, a Organização Mundial da Saúde (OMS) declarou que o surto de COVID-19, doença causada por um novo coronavírus, passava a constituir uma pandemia, dada a velocidade e escala de transmissão da doença. A Região das Américas é caracterizada por uma riqueza mult...iétnica e multicultural. No entanto, povos indígenas, afrodescendentes e outros grupos étnicos, muitas vezes, sofrem discriminação e exclusão, e isso leva a desigualdades de saúde. A COVID-19 pode ter um maior impacto em certos grupos da população, como povos indígenas e afrodescendentes. Em 2017, os Estados Membros da Organização Pan-Americana da Saúde (OPAS) aprovaram a primeira Política de Etnia e Saúde (documento CSP29/7, Rev.1), baseada no reconhecimento de diferenças entre grupos étnicos, bem como em seus respectivos desafios, necessidades e contextos históricos. A política também reforça a necessidade de uma abordagem intercultural, fundamentada na igualdade e no respeito mútuo, para melhorar os desfechos de saúde e avançar em direção à saúde universal. A OPAS prioriza a etnia como uma questão de caráter transversal no gerenciamento de emergências e desastres. Isso está refletido em uma série de regulamentos, como o Plano de Ação 2016-2021 para a Redução do Risco de Desastres e outros manuais, diretrizes e iniciativas.

En este manual se explica cómo ejecutar las medidas con vistas a logra...r la eliminación de la malaria y prevenir su restablecimiento, mediante el aumento de la intensidad y la calidad de las medidas, la reorientación de las iniciativas, la reducción de los retrasos que favorecen la transmisión y un monitoreo adecuado que permita ajustar las intervenciones. En este sentido, se propone como primer paso estratificar el territorio —de modo que se puedan planificar y priorizar las intervenciones y grupos de población— para, a continuación, aplicar una medida de diagnóstico, tratamiento, investigación y respuesta como estrategia clave dirigida a hacer efectiva la eliminación de la enfermedad y a prevenir su restablecimiento en todos los estratos. Además, el documento trata de hacer operativo el concepto de vigilancia, como intervención promovida por la Organización Mundial de la Salud a través de la Estrategia Técnica Mundial contra la Malaria 2016-2030, y destaca la importancia de que las comunidades tengan acceso al diagnóstico y el tratamiento tempranos. Por último, en el manual también se incluyen aspectos de gestión y monitoreo que deben tenerse en cuenta para la eliminación de los focos de transmisión.

All malaria-endemic countries in the Region of the Americas have taken on the challenge to eliminate the disease and to put in place measures to orient their health programs and strategies in that direction. This manual explains how to implement measures to achieve malaria elimination and prevent it...s reestablishment by increasing the intensity and quality of interventions, reorienting initiatives, reducing delays that favor transmission, and ensuring adequate monitoring to adjust interventions. In this regard, the first suggested step is to stratify the territory - so that interventions and population groups can be planned and prioritized - and then implement diagnosis, treatment, investigation and response (DTI-R) actions as a key strategy aimed at effectively eliminating the disease and preventing its reestablishment in all strata. In addition, the document seeks to operationalize the concept of "surveillance as an intervention" promoted by the World Health Organization through the Global Technical Strategy for Malaria 2016-2030 and highlights the importance of communities having access to early diagnosis and treatment. Finally, the manual also includes management and monitoring aspects that must be taken into account to eliminate the foci with transmission.

Chagas disease affects 8-10 million people worldwide, mainly in Latin America. The current therapy for Chagas disease is limited to nifurtimox and benznidazole, which are effective in treating only the acute phase of the disease but with severe side effects. Recovery from COVID-19 has been challenging in Guatemala. As a result of the prolonged socio-economic impact of the pandemic, the average poverty rate nationwide has increased by almost 5%. This rise in the poverty level further exacerbates preexisting vulnerabilities and erodes the limited safety n...ets available to vulnerable populations. Year after year, recurrent disasters and humanitarian crises aggravate the historic social gaps that result in high levels of vulnerability, multidimensional poverty, and overall deprivation of essential services among hundreds of thousands of Guatemalans. According to the World Risk Report 2020, Guatemala is the tenth country with the highest level of exposure to disaster worldwide. Globally, it ranks 28th regarding vulnerability according to the 2021 INFORM’s risk index and 62nd in the Global Climate Risk Index 2021.
In 2020, Guatemala faced a record-breaking and devastating hurricane season with extreme rainfall, catastrophic winds, and deadly landslides, from which the country has not yet recovered. Unfortunately, recurrent extreme weather events, such as Hurricane Julia that hit Central America in early October 2022, progressively but deeply eroded a weak health infrastructure and local health systems.

Faced with the largest population outflow in Latin America of recent years, 95 organizations covering 16 countries have been working together to establish a comprehensive response to the urgent needs of millions of refugees and migrants from Venezuela, and host communities.
It is estimated that, in Latin America alone, between 6 and 8 million people are infected with the parasite, and 99% of these are not receiving treatment, either because they are unaware of their infection or because they do not know that treatment exists. The potential spread of the disease is also... a cause for considerable concern: 65 million people live in areas of exposure and are at risk of contracting Chagas disease, and 28,000 new cases occur every year.
