The African Network for improv...ed Diagnostics, Epidemiology and Management of Common Infectious Agents – ANDEMIA represents a transnational strategy to research and combat acute respiratory tract and gastrointestinal infections, acute febrile disease of unknown cause and the emergence of antimicrobial resistance in sub-Saharan Africa (SSA).
Although many international networks give support to programs related to the diagnosis and management of tuberculosis, malaria and Human Immunodeficiency Virus in Africa, rarely have international consortia focused on developing and strengthening capacity for the comprehensive surveillance of common infectious diseases, although recent studies demonstrate that they are an important cause of morbidity and mortality in SSA, mostly in children.
ANDEMIA will establish a sentinel surveillance system through which relevant data will be gathered from not only urban, but also remote rural sentinel clinics within the African partner countries Côte d’Ivoire, Burkina Faso, Democratic Republic of the Congo und Republic South Africa. We employ a One Health approach by also investigating zoonotic and environmental pathogen reservoirs.

The East African Community mee...ting on orientation and planning on the operationalization of the Regional Network of Public Health Reference Laboratories for Communicable Diseases (RNPHRL) is underway at the EAC headquarters in Arusha, Tanzania.
The purpose of the RNPHRL meeting is to strengthen capacities in all five EAC Partner States to respond to pathogens of biosafety level 3 and 4 nature and other outbreaks of infectious diseases, prone to cause cross-border epidemics, by rapid diagnostic capacities that enable timely interventions.
The establishment of the RNPHRL has been necessitated by the growing challenge of epidemics and pandemics including viral haemorrhagic fevers (e.g. Ebola) with devastating impacts on health outcomes and the economy amidst limited infrastructure and human resource capacity of the National and Regional Reference Public Health Laboratory since most of the laboratories are at biosafety level 2.

Only few countries in sub-Saha...ran Africa have implemented a antimicrobial surveillance system, therefore there are important data gaps with regard to public health relevant bacteria.The project will support partner countries to establish blood culture diagnostic systems and to establish necessary laboratory diagnosis in selected health facilities. Furthermore, clinical and socio-demographical data are collected and summarized in a national surveillance system. Regular trainings are planned in partner countries. These activities will be based on the already established contacts within the ANDEMIA project.

Investigation in outbreak situ...ations of public health relevance (i.e. encephalitis and hemorrhagic fevers) to allow for rapid diagnostic. In the first collaboration with the Sri Lankan institutions, the prevalence of Henipaviruses in the human population and reservoir hosts will be determined by an One Health approach. Henipaviruses (BSL-4) are causing often fatal encephalitis in humans. Our Sri Lankan partners report a high number of undiagnosed deaths by encephalitis. In this project we will investigate these cases for the prevalence of pathogens. In the neighboring countries of Sri Lanka, Bangladesh and India, Henipaviruses have already been detected in Pteropus bats (potential reservoir host). It is described that these bats are abundant in Sri Lanka and migrate seasonally to Bangladesh and India. We are working collaboratively with Ecologists and Medical doctors on risk assessment, characterization and relevant differential diagnostic of these highly pathogenic viruses.

Health risks experienced by co...nsumers and producers related to Staphylococcus (S.) aureus in the dairy food chain in Zambia should be reduced as a prerequisite to improve food security in the country. Objectives are risk assessment of milk/milk products as source of enterotoxigenic and/or methicillin-resistant S. aureus (MRSA) and adopting methods of monitoring zoonotic and antimicrobial resistant S. aureus in the dairy value chain to the Zambian situation. Purposes are to understand to which extend the dairy value chain contributes to the burden of S. aureus induced foodborne disease, to identify whether the dairy value chain has to be considered as a pathway through which antimicrobial resistant bacteria, i.e. MRSA can be transmitted to humans, to identify targets for possible interventions to reduce transmission, and to foster sustainable research and capacity addressing the impact and control of S. aureus caused foodborne-diseases and antimicrobial resistance (AMR) in humans.

In many low and middle income ...countries, bacterial pathogens such as Streptococcus pneumoniae or Salmonella enterica species constitute still a considerable public health problem despite the availability of vaccines and antibiotics. Data on the burden of disease and degree of antimicrobial resistance (AMR) are often limited. Standardized surveillance which captures epidemiological data and the genetic sequences from clinical organisms is necessary to further define the global AMR threat and understand the origins and transmission dynamics of AMR in specific organisms. Data from surveillance networks uploaded to real-time whole-genome sequencing databases currently enables researchers to follow the evolution of resistant pathogens, resistance genes, and determine high-risk areas.

The African Network for improv...ed Diagnostics, Epidemiology and Management of Common Infectious Agents – ANDEMIA represents a transnational strategy to research and combat acute respiratory tract and gastrointestinal infections, acute febrile disease of unknown cause and the emergence of antimicrobial resistance in sub-Saharan Africa (SSA).
Although many international networks give support to programs related to the diagnosis and management of tuberculosis, malaria and Human Immunodeficiency Virus in Africa, rarely have international consortia focused on developing and strengthening capacity for the comprehensive surveillance of common infectious diseases, although recent studies demonstrate that they are an important cause of morbidity and mortality in SSA, mostly in children.
ANDEMIA will establish a sentinel surveillance system through which relevant data will be gathered from not only urban, but also remote rural sentinel clinics within the African partner countries Côte d’Ivoire, Burkina Faso, Democratic Republic of the Congo und Republic South Africa. We employ a One Health approach by also investigating zoonotic and environmental pathogen reservoirs.

Few countries in sub-Saharan A...frica have implemented an antimicrobial surveillance system, there are important data gaps with regard to public health relevant bacteria. Project will support partner countries to establish blood culture diagnostic systems and necessary laboratory diagnosis in selected health facilities. Clinical and socio-demographical data are collected and summarized in a national surveillance system. Trainings are planned in partner countries. Activities will be based on already established contacts within the ANDEMIA project. Depending on the resources available, primary focus will be on the neighboring countries Ivory Coast and Burkina Faso. Project could be extended to the Congo. South Africa will be a partner for South-South cooperations, a partially active national surveillance system exists. WHO-documents serve as templates in the development of surveillance systems.

(I) Prevalence of Henipaviruses in the human population and reservoir hosts by PCR, serology and high-throughput sequencing. (II) Investigation of the viral and bacterial fauna of potential reservoir hosts. Selected sentinel viruses will be used to study the seasonality of pathogen shedding. (III) Extension of the already existing range of diagnostic methods to allow for fast and accurate diagnostics and differential diagnostics of viruses. Progress and quality will be assessed in regular cycles by EQA tests. (IV) Investigation of yet undiagnosed cases of encephalitis and hemorrhagic fevers in collaboration with the Colombo North Teaching Hospital to identify yet unknown pathogens and adapt the range of diagnostic methods.

Health risks experienced by co...nsumers and producers related to Staphylococcus (S.) aureus in the dairy food chain in Zambia should be reduced as a prerequisite to improve food security in the country. Objectives are risk assessment of milk/milk products as source of enterotoxigenic and/or methicillin-resistant S. aureus (MRSA) and adopting methods of monitoring zoonotic and antimicrobial resistant S. aureus in the dairy value chain to the Zambian situation. Purposes are to understand to which extend the dairy value chain contributes to the burden of S. aureus induced foodborne disease, to identify whether the dairy value chain has to be considered as a pathway through which antimicrobial resistant bacteria, i.e. MRSA can be transmitted to humans, to identify targets for possible interventions to reduce transmission, and to foster sustainable research and capacity addressing the impact and control of S. aureus caused foodborne-diseases and antimicrobial resistance (AMR) in humans.

In many low and middle income ...countries, bacterial pathogens such as Streptococcus pneumoniae or Salmonella enterica species constitute still a considerable public health problem despite the availability of vaccines and antibiotics. Data on the burden of disease and degree of antimicrobial resistance (AMR) are often limited in these countries. Standardized surveillance which captures epidemiological data and the genetic sequences from clinical organisms is necessary to further define the global AMR threat and understand the origins and transmission dynamics of AMR in specific organisms. Data from surveillance networks uploaded to real-time whole-genome sequencing databases currently enables researchers to follow the evolution of resistant pathogens, resistance genes, and determine high-risk areas.

The African Network for improv...ed Diagnostics, Epidemiology and Management of Common Infectious Agents – ANDEMIA represents a transnational strategy to research and combat acute respiratory tract and gastrointestinal infections, acute febrile disease of unknown cause and the emergence of antimicrobial resistance in sub-Saharan Africa (SSA).
Although many international networks give support to programs related to the diagnosis and management of tuberculosis, malaria and Human Immunodeficiency Virus in Africa, rarely have international consortia focused on developing and strengthening capacity for the comprehensive surveillance of common infectious diseases, although recent studies demonstrate that they are an important cause of morbidity and mortality in SSA, mostly in children.
ANDEMIA will establish a sentinel surveillance system through which relevant data will be gathered from not only urban, but also remote rural sentinel clinics within the African partner countries Côte d’Ivoire, Burkina Faso, Democratic Republic of the Congo und Republic South Africa. We employ a One Health approach by also investigating zoonotic and environmental pathogen reservoirs.

Few countries in sub-Saharan A...frica have implemented an antimicrobial surveillance system, there are important data gaps with regard to public health relevant bacteria. Project will support partner countries to establish blood culture diagnostic systems and necessary laboratory diagnosis in selected health facilities. Clinical and socio-demographical data are collected and summarized in a national surveillance system. Trainings are conducted in partner countries. Activities will be based on already established contacts within the ANDEMIA project. Depending on the resources available, primary focus will be on the countries Cote d'Ivoire and Burkina Faso. Project could be extended to the Congo and Nigeria. South Africa will be a partner for South-South cooperations, a partially active national surveillance system exists. WHO-documents serve as templates in the development of surveillance systems.
