Tuberculosis (TB) re-emerged a...s an important public health problem after Armenia gained its independence from the Soviet Union in 1991, and the burden of the disease remains high. Resistance to anti-TB drugs represents a serious obstacle to effective control of the TB epidemic in Armenia. The Program has two main objectives: to strengthen the National TB Control Program to provide better diagnostics and treatment, including the scale-up of the management of drug-resistant TB (DR-TB) in the country by enrolling over a thousand multidrug-resistant TB (MDR-TB) patients over five years from the civilian and penitentiary sectors, and to improve the supportive environment for effective TB control. It aims to strengthen the National TB Control Program in the following areas: procurement and supply management, human resources, MDR-TB and TB diagnostics and case management, social support to TB patients, TB/HIV co-infection, infection control, monitoring and evaluation, advocacy, communication and social mobilization. Its goals are to reduce TB incidence & mortality and prevent the further development of drug-resistant TB by targeting all TB and DR-TB patients. This is a consolidation of Round 10 and Round 8 TB grants.

A drug-resistant survey, condu...cted in 2007, showed multidrug-resistant TB (MDR-TB) prevalence of 22 percent among new TB cases and 56 percent among previously treated cases. Over 14 percent of all MDR-TB cases are extremely drug-resistant TB (XDR-TB). Azerbaijan is among the 25 priority MDR-TB and XDR-TB countries identified by the World Health Organization based on estimated MDR-TB burden (more than 10 percent among both new and retreatment cases combined). The consoidated program encompasses the activities financed under Phase 2 of the Round 5 TB grant and the Round 7 TB grant. The program aims to reduce the burden of TB in Azerbaijan in high-risk population groups (including people living with HIV), prisoners, labor migrants, refugees and internally displaced persons) by strengthening and expanding the DOTS framework, scaling up the management of drug-resistant TB and preventing the further spread of drug-resistant TB including XDR-TB. This consoidated program also aims to improving the detection, diagnosis and quality treatment of TB cases (including MDR-TB) and contributing to the universal access of the population to diagnosis and treatment. These objectives are achieved by strengthening the management and coordination of the National TB Control Program, involving the primary health care network in TB control, introducing and scaling up drug-resistant TB management, sustaining drug resistance surveillance throughout the country, strengthening DOTS in penitentiaries and upgrading the laboratory services.

TB, which once was in regressi...on, is once again proving a major health problem in Benin, with poverty and HIV and AIDS as aggravating factors. The program supported by this grant combines funding from Rounds 6 and 9 and aims to reduce TB-related sickness and death in Benin. The program will first focus on the early diagnosis and appropriate treatment of smear-positive cases, which are the most contagious. However, greater attention will be paid to smear-negative and nonpulmonary forms to TB, which may have very little influence on transmission, but can lead to death and relapses on an individual level. To reach the goal, the program will increase the case detection rate to 70 percent by 2011, increase access to appropriate care for TB patients co-infected with HIV to 95 percent by 2011, maintain the therapeutic success rate of 87 percent, provide appropriate treatment to 60 percent of TB patients with multidrug-resistant TB and strengthen the capacity of the country's National Tuberculosis Control Program to provide coordination and support to integrated and decentralized anti-TB structures. Additionally, a new screening guide will be developed, personnel will be trained or retrained and diagnosis by radiology will be strengthened.

Over the years, Bhutan has ach...ieved a significant breakthrough in scaling down the TB infection rate due to the establishment of the Bhutan National Tuberculosis Control Program in 1976. However, DOTS coverage of certain population groups is still incomplete — in particular, women, youth under the age of 18 and people living in rural areas. The government is strongly committed to eradicating the TB threat, directing its efforts at the most vulnerable groups — the poor and remote populations. The national program has been pursuing a pro-poor policy by treating 70 percent of TB patients free of charge. The program supported by this grant aims at building capacity for responding to the disease at national, regional and village levels. The program will intensify advocacy and awareness activities through media campaigns. It will also increase case detection and cure rates through the establishment of DOTS committees and the provision of training for village health workers as well as medical and technical staff. The program has made significant progress toward its targets.

In addition to DOTS, the strategy includes management of multidrug-resistant TB, including building capacity for drug-susceptibility testing and HIV sentinel testing in all TB patients during three months per year. The strategy is being implemented through all health care providers in the country.

The TB burden in Bosnia and He...rzegovina is moderate in comparison with other countries in the World Health Organization European Region. The goal of the program supported by this grant is to cost effectively reduce the TB burden, which has remained steady in recent years, and further consolidate and fortify National Tuberculosis Programme efforts aimed at its sustainable reduction. The program will continue and further develop ongoing activities initiated under the Round 6 grant. It is also aligned with national objectives for strengthening the governance and coordination of health systems and for the further development of primary health care and family medicine, among other reform efforts. The main beneficiaries of this grant will be TB and multidrug-resistant TB patients, ethnic minorities and Roma populations, prisoners and migrants, and other vulnerable populations living in poor conditions. The population at large will also benefit from a decrease in TB and from strengthened health systems.

TB is a significant health pro...blem in Cambodia, primarily affecting adults of working age. The program supported by this grant will strengthen the technical and managerial capacity of health staff at all levels to deliver high quality DOTS services; ensure uninterrupted supply of drugs for TB treatment; conduct a national TB prevalence survey; increase access to DOTS among the urban poor; and increase access of TB patients to voluntary counseling and testing for HIV and relevant prevention and treatment programs, including antiretroviral treatment and care. TB is a significant health pro...blem in Cambodia, primarily affecting adults of working age. The program supported by this grant aims to reduce TB-related illness and death in line with the Millennium Development Goals and the Stop TB Partnership strategy.. The program targets people suspected of having TB and those that have been confirmed as having TB; TB patients with limited access to DOTS; TB/HIV co-infected patients; and multidrug-resistant TB (MDR-TB) patients. Grant funds are being used to ensure high quality TB services nationwide by strengthening laboratory services, providing quality assured anti-TB drugs and implementing activities for advocacy, communication and social mobilization; expand close-to-patient DOTS services through community DOTS and public-private mix services; address TB/HIV co-infection by building capacity of staff to plan and implement TB/HIV collaborative activities; and to address MDR-TB by building the capacity of the National TB Program to provide MDR-TB services in three selected sites in country and to provide MDR-TB treatment to a growing number of patients.

Despite the government's effor...ts towards providing free treatment, statistics from medical officials show that the rate of TB infection is not dropping. The overall goal of the program supported by this grant is to reduce the burden of TB and the spread of the disease through proper case management, by detecting at least 70 percent of infectious cases and curing at least 85 percent of cases detected. To reach this goal, the program will expand the number of TB diagnosis and treatment centers; improve the management of TB patients; and establish voluntary HIV screening services in TB centers. The program will also develop public awareness campaigns aimed at the general population to reduce stigma and provide information on treatment possibilities.

Tuberculosis (TB) is a major p...ublic health problem in Central African Republic (CAR). According to WHO 2009 report, the prevalence reached 424 cases per 100,000 habitants for all forms with an incidence of 328 cases per 100,000 inhabitants for new smear-positive cases. Based on a study carried out by the Institut Pasteur in Bangui in 2009, the rate for multi-drug resistance TB (MDR-TB) constituted 14 percent. The Round 9 proposal supports the Strategic Plan to Stop TB 2008-2012 and aims to contribute to a reduction in TB morbidity and mortality in CAR. Building on the Round 4 grant, the current grant seeks to improve the monitoring of patients receiving treatment to achieve a treatment success rate of 85 percent. The activities focus on expanding and enhancing of high-quality DOTS, fighting HIV/TB co-infection and MDR-TB, enhancing community involvement, advocacy, awareness-raising and social mobilization against TB, and strengthening program coordination and monitoring and evaluation.

The program supported by this grant aims to reduce TB-related illness in the country by 50 percent and TB-related deaths by 45 percent. The program will train health professionals, laboratory technicians and community health workers; provide quality control of microscopy laboratories; prepare, produce and distribute information, education and communication campaigns to the media and supply anti-TB drugs and laboratory consumables to health delegations. In addition, the program plans to hold a workshop to review training and monitoring support needs for 750 health workers.

In Colombia, Tuberculosis (TB)... affects different sectors of the population in the entire territory. The higher burden of the disease is concentrated in the major urban areas; however, the higher incidences are registered in disperse, rural regions of difficult access, with high indexes of unsatisfactory basic needs among the population. This Program focuses on 46 high-priority municipalities located in hard-to-reach areas of the Pacific coast. It aims to implement cost-effective actions in geographic areas with limited access to health services due to the great dispersion and rural location and is structured under five objectives: strengthen DOTS quality in 46 priority municipalities, deal with TB/HIV co-infection and MDR-TB, bring together diagnosis and timely treatment activities through community DOTS, favour the local response for TB control through advocacy, communication and social mobilization in 46 priority municipalities, and contribute to the improvement of programmatic management by conducting operational studies. The Program is implemented by two Principal Recipients (PR) under the dual-track financing scheme. The current PR will focus on improvement of diagnosis, detection, infection control and management of the TB/HIV co-infection activities.

Congo is one of the 22 tubercu...losis (TB) high-burden countries as defined by the World Health Organization. With 403 new TB cases (all forms) per 100,000 population reported in 2007, Congo is experiencing a severe TB epidemic that is unequally distributed across the country with the highest incidence (85 percent) detected in Brazzaville and Pointe-Noire. Coverage with DOTS, the basic package that underpins the Stop TB Strategy, extends to only 60 percent of the population. The goal of the program – the first in Congo to receive Global Fund support to fight TB – is to halve TB morbidity and mortality by strengthening and promoting DOTS, and expanding multidrug-resistant and extensively drug-resistant TB control measures. The program targets TB patients and patients co-infected with TB and HIV, particularly focusing on women, young girls and prisoners.

Based on a national survey conducted in 2006, the rate of Multi-Drug Resistant TB (MDR-TB) is 2.5 percent for new cases, and 8.6 percent for cases in re-treatment. The Program based on the Round 9 proposal and is implemented by two Principal Recipients (PR) through the dual-track financing scheme. It continues to strengthen the extension of Directly Observed Treatment Short course (DOTS) strategy by increasing and de-centralizing the available care, by improving the management of MDR-TB cases, and by making better use of mass media for communication about TB. The activities implemented by this PR include training of private sector care workers, health community workers, and peer educators, communication on stigma reduction, Knowledge Attitude Practice (KAP) survey, and community support.

However, for a number of reasons, the incidence of TB in Korea (DPR) then increased rapidly and became one of the highest in the region, reaching 344 per 100,000 people in 2008. The National TB Control Program in Korea (DPR) implemented DOTS following the standard World Health Organization guidelines in 1998 and achieved nationwide coverage by the end of 2003. The program supported by this grant will expand DOTS and implement newer interventions in support of the country's strategic plan.

According to the World Health ...Organization (WHO), the estimated incidence rate of sputum positive pulmonary tuberculosis (TB) in the Dominican Republic fell from 54 to 40 per 100,000 inhabitants between 2003 and 2006, and the mortality rate from 17 to 15 per 100,000 inhabitants (WHO Global Tuberculosis Control 2005 and 2008 respectively). This downtrend in the incidence and mortality rate is a result of the marked improvement in the country's national TB control response. The main objective of the Rolling Continuation Channel program is to contribute to the reduction of TB-related morbidity and mortality by sustaining and expanding the actions implemented in the program supported by the Round 3 grant among vulnerable populations in 22 provinces of the Dominican Republic. In order to meet this goal, most of the project interventions are related to the need for strategic National Tuberculosis Control Program (NTCP) guidelines within the context of the health sector reform process taking place in the country, as well as a review and update of the tools and planning and operational systems of the NTCP. In addition, the program focuses on the need to promote the closure of detection gaps and to implement a number of necessary studies for the reorganization of NTCP interventions.

In the last 6 years, progress ...has been made in Dominican Republic to control Tuberculosis (TB). The Secretariat of Public Health and Social Welfare (SESPAS) has implemented varies TB control measures, with support from international partners such as USAID and World Foundation. The overall goal of the program is to lower morbidity and mortality due to TB through the implementation of the Stop TB strategy. The program activities target populations higher risk of becoming infected with TB such as TB patients and people living with HIV/AIDS (PLWHA) and their families. The current grant is expected to widen and sustain the achievements of the Directly Observed Treatment Short-course (DOTS) strategy, bridge the managerial, operational and epidemiological gaps identified in order to achieve the objectives of 'Stop TB Strategy' and implement activities based on stratified analysis at national, regional and local levels. The Principal Recipient has changed from Asociacion Dominicana Pro-Bienestar de la Familia (PROFAMILIA) to the Ministry of Public Health. The program is designed to strengthen the National Health System in key areas such as management capacities of the National TB Control Programme (PNCT), management of human resources and information systems.

World Health Organization rank...s Ecuador as fourth among the highest number of Tuberculosis (TB) cases in the Americas, with a prevalence rate of 140, an incidence rate of 101 and a mortality rate of 23 per 100,000 persons. The program is designed to strengthen the TB Control Program at local level, the inclusion of institutions of the health sector, participation of the population through those affected by TB, and the participation of indigenous and grass roots organizations. The program is being carried out in ten prioritized provinces in Ecuador with the highest proportion of poor and vulnerable populations, which collectively comprise 78 percent of the population, 82 percent of TB and TB/HIV cases, and 94 percent of cases of Multi-Drug Resistant Tuberculosis (MDR-TB). The program is based on Round 9 Global Fund TB Program and will be implemented by two Principal Recipients (PR), through Dual Track Financing scheme. This PR will focus on indigenous populations in marginal urban areas, and persons deprived of liberty and implement activities to strengthen actions for the control of TB, TB/HIV and MDR-TB within the framework of the overall Stop TB Strategy.

The goal of the program supported by this grant is to reduce the burden of TB in Egypt, particularly among the poor and vulnerable population, in line with the Millennium Development Goals and the Stop TB targets, ensuring that all the poor patients in rural areas and urban slums and refugees with TB have access to effective therapy. The program seeks to upgrade health facilities for multidrug-resistant TB; provide DOTS training to health care providers, prison workers and community workers; train laboratory technician to diagnose TB correctly; conduct awareness activities for targeted groups; and enhance the follow-up and counseling of TB patients and their families.

In El Salvador, tuberculosis (...TB) is still a major public health issue. Despite 100 percent DOTS coverage in public health institutions, the Salvadoran Social Security Institute and incarceration centers, the case detection rate remains too low. The program financed by this Round 9 grant aims to provide comprehensive TB prevention, treatment and care services for vulnerable populations in the 26 prioritized municipalities through five key strategies: (1) implementation of a differentiated strategy, based on the characteristics of each population group, to control TB in precarious urban settlements; (2) improving TB control in 18 prisons; (3) enhancing the timely diagnosis, management and prevention of TB in people living with HIV; (4) improving management of the risk of infection in people exposed to TB patients; and (5) prevention of multidrug-resistant TB by employment of a differentiated approach to handling resistant cases and groups at high risk of developing resistance. This program will be implemented by two Principal Recipients. The United Nations Development Programme (UNDP) will be responsible for the procurement of medicines and health equipment, and for community-based services. The Ministry of Health and Social Welfare will be mainly responsible for TB diagnosis, treatment and training medical personnel.

The Fiji Islands have a popula...tion of 827,900 people. The Ministry of Health in Fiji introduced the DOTS strategy for the treatment of TB in 1997. Even though the notification rate of all TB cases has declined at a rate of 3.9 percent per year over the past 15 years, the notification rate of new smear-positive cases has been stable. The program supported by this grant aims to reduce the burden of TB in Fiji; to achieve improved TB and HIV and AIDS outcomes through strengthening the capacity of the health system to deliver services; and to strengthen the health system by means of improving the production, management and use of information. The program targets 650 TB patients to be treated over five years; 750 children in contact with infectious patients who will be screened and will receive TB prophylaxis; people living with HIV; populations in remote areas or outer islands; and prisoners, who will benefit from improved access to diagnosis and treatment services for TB.

Although the incidence and pre...valence of TB in the Gambia are unknown, it is a growing public health problem as indicated by the rising number of cases. The program supported by this grant will expand the TB diagnostic and treatment network together with an extension of laboratory services. To integrate DOTS services into the existing health system, the program will provide training and support this integration through a new manual for TB management. The program will further strengthen HIV/TB collaborative activities to reduce the burden of TB in people living with HIV and AIDS and to decrease the burden of HIV in TB patients. In addition to the general population of the Gambia, the program targets the following vulnerable groups: children under five years of age, people living with HIV and AIDS and adults who are prisoners and refugees.

The annual burden of all forms... of tuberculosis (TB) in The Gambia is estimated to be 4,415 cases (TB incidence of 257 per 100,000 population), including 1,893 (113 per 100,000 population) smear-positive TB cases. The goal of the Program is aligned with the National Strategic Plan for Tuberculosis Control (2008-2012) i.e. to reduce transmission, morbidity and mortality of TB so that it is no longer a public health problem. It aims to increase access to TB diagnosis and treatment, to achieve the minimum 70% percent case detection rate and at least 85 percent treatment success rate. It is expected that the Program will build upon the achievements of the Round 5 Global Fund funded grant, which is expected to end in 2011, but also tackle several challenges, including a relatively high TB mortality rate, a weak referral system, and areas with spatial clustering of TB cases. The Program will be implemented through dual-track financing arrangement, which will be implemented in the country by two Principal Recipients. The current grant will focus on the identification/treatment of infectious TB cases, increasing access to HIV counselling and testing for TB patients, and on the introduction of the Practical Approach to Lung Health Strategy.

Due to the economic downturn, ...political instability, armed conflicts and population displacements, Tuberculosis (TB) re-emerged as a major public health threat in Georgia. Despite remarkable progress achieved since 1995 under the National TB Control Program (NTP), serious challenges remain related to resistance to TB drugs. In 2008 to 2009, MDR-TB in Georgia was found in 10.6 percent to 11.3 percent of new cases and 32.5 percent to 40.3 percent of previously treated cases. The Country Coordinating Mechanism (CCM) submitted a TB proposal in Round 10 and agreed to consolidate the existing Round 4 Rolling Continuation Channel (RCC) TB grant with Round 10 grant to align activities and reporting. In sustaining universal access to multi/extensively-drug resistant (M/X-DR) TB, the program targets to enrol 3, 730 DR-TB patients in treatment over the next 5 years, ensuring coverage of not less than 90 percent of the needs. The NTP Central Unit through the network of public health care institutions and providers will be responsible for implementation of the majority of the planned activities. The current grant will support provision of technical assistance to the NTP in priority areas of TB control and provide IT support.
