Dracunculiasis (Guinea worm disease), caused by the parasite Dracunculus medinensis, is traditionally acquired by drinking water containing copepods (water fleas) infected with D. medinensis larvae, but in recent years also appears increasingly to be transmitted by eating fish or other aquatic anima...ls. Sixty-four mayors in nine regions across Senegal are now budgeting locally for family planning—a transformation in prioritizing access to contraceptives for women there. Mayors only recently obtained the authority to budget for family planning due to a change in national law in 2014. Advocates act...ed quickly and strategically to capitalize on this new authority: from 2014 to 2017, 64 mayors budgeted a total of 73.8 million West African CFA francs (XOF), about US $125,000. Although many are seemingly small amounts, typically about $2,000, these commitments reflect important, first-time contributions from these local leaders—and a strong indicator of growing local ownership for family planning.

Severe Acute Malnutrition (SAM) is one of the greatest child survival challenges in the world today and
reportedly affects more than 16.2 million children each year1. High impact, proven treatment interventions exist
yet sadly approximately only 3.2 million children with SAM have access to treatme...nt each year2. Thus, there
is a need to scale up interventions to improve coverage and access across high burden countries. While efforts
are currently underway to expand services in many countries, obstacles remain.
One critical barrier to expanding SAM treatment services is the acceptance, accessibility and utilisation of
ready-to-use therapeutic food (RUTF). In some countries and contexts, RUTF is still not fully accepted by
community members; while other countries face problems with procurement, storage and supply chain
management which impact on availability and use3. Reports from Ghana and Zambia highlighted that stock-
outs and logistical challenges are often noted as key contributors to high default rates in outpatient treatment
centres4.

Trypanosoma brucei rhodesiense (T.b. rhodesiense), ranging from chronic disease patterns in southern countries of East Africa to an increase in virulence towards the north. However, only limited d...ata on the clinical presentation of T.b. rhodesiense HAT is available. From 2006-2009 we conducted the first clinical trial program (I MPAMEL III) in T.b. rhodesiense endemic areas of
Tanzania and Uganda in accordance with international standards (ICH-GCP). The primary and secondary outcome measures were safety and efficacy of an abridged melarsoprol schedule for treatment of second stage disease. Based on diagnostic findings and clinical examinations at baseline we describe the clinical presentation of T.b. rhodesiense HAT in second stage patients from two distinct geographical settings in East Africa.

L’absence de services WASH dans les établissements de santé compromet la sécurité et la dignité des patients, tout en menaçant d’exacerber la propagation d’infections résistantes aux agents antimicrobiens et en fragilisant les efforts déployés pour améliorer la santé des enfants et des mères.
