The report summarizes key global health expenditure patterns and trends, and illustrates the potential of the new database to inform thinking about financing reforms to progress towards UHC, and also raises issues for further research. It analyses the following areas:
The results of the report clearly show that in 2020, a year dominated by the emergence of COVID-19 and its associated health and economic crises, governments around the world rose to the challenge. Sharp increases in government spending on health at all country income levels underpinned the rise in ...health spending to a new high of US $9 trillion (approximately 11% of global GDP). Government health spending generally increased and offset declines in out-of-pocket spending. Importantly, the rise in government health spending was part of a much broader fiscal response to the pandemic. In high income and upper-middle income countries social protection spending also increased sharply in as governments attempted to cushion populations from the economic impacts of COVID-19. In contrast to health and social protection, growth in education spending was relatively subdued. Countries face the further challenge of sustaining increased public spending on health and other social sectors in the face of deteriorating macroeconomic conditions and rising debt servicing. This also includes the challenge of sustaining external support for low income countries, which is essential for reducing ensuring poverty, ensuring access to health services and strengthening pandemic preparedness.

It adversely affects mostly the poor and vulnerable populations in countries, which are already d...eprived of proper health facilities and conducive environmental conditions. The disease spreads through oro-fecal transmission by the ingestion of contaminated food or water or by person-to-person contact. It has a short incubation period of 2 hours to 5 days and the number of affected cases can rapidly increase across large regions. Cholera is a significant threat to global public health leading to an estimated 3-5 million cases per year worldwide, with an annual toll of 100,000 deaths. The disease was first reported in 1817 from the Ganges Delta of India and since then the ongoing 7th pandemic has emerged from Indonesia, reached Africa in 1970 and Somalia happens to be one of the early affected countries. In 2006, the Special Session of African Union Health Ministers adopted the Maputo Plan of Action for implementing the Continental Policy Framework on sexual and reproductive health and rights (SRHR), which expired at the end of 2015. The goal was for all stakeholders and partners to join forces and ...re-double efforts, so that together, the effective implementation of the Continental Policy framework including universal access to sexual and reproductive health by 2015 in all countries in Africa can be achieved. The Revised Maputo Plan of Action (MPoA) 2016 – 2030 was subsequently endorsed by the African Union Heads of State at the 27th AU Summit in July 2016 in Kigali, Rwanda. The plan reinforces the call for universal access to comprehensive sexual and reproductive health services in Africa and lays foundation to the Sustainable Development Goals, particularly Goal 3 and 5, as well as the African Union Agenda 2063.

Substantial progress in the fight against HIV has been made over the past decade. Advances in HIV prevention, testing, and treatment have been matched by declines in HIV incidence and HIV-related deaths. The success of Botswana reaching the 95-95-95 targets, despite resource limitations, points to a... hopeful future. However, the recent publication of In Danger: UNAIDS Global AIDS Update 2022, the UNAIDS annual report, paints a sobering picture of the fragility of these gains.
Stagnating financing for the HIV response, alongside continued global inequities, and the impact of the COVID-19 pandemic, jeopardises progress. Current projections indicate that neither the UNAIDS 2025 95-95-95 targets nor the goal of ending the AIDS
epidemic by 2030 will be met.

In April 2020, Gavi and COVAX joined the Access to COVID-19 Tools Accelerator (ACT-A) to provide equitable global access to COVID-19 vaccines to tackle the pandemic. In June 2020, the Gavi COVAX Advance Market Commitment (AMC) was launched to finance equitable access in 92 lower-income countries. Si...nce then, US$ 10 billion has been raised for the AMC to procure vaccines and support delivery. Despite a challenging supply situation, COVAX has now shipped one billion doses to 144 countries, including over 870 million to AMC economies.

The World Health Organization (WHO) has been present in Niger since 1960, and acts as the Government's principal advisor on public health and lead of the health cluster. WHO covers all eight regions of the country with 113 staff members in Niamey and in 7 sub-offices (Agadez, Diffa, Zinder, Maradi,... Tillabéri, Dosso, Tahoua).
To strengthen its cooperation with Niger, WHO has recently developed a new Country Cooperation Strategy (CCS) for 2023-2027 period in collaboration with the Ministry of Public Health, Popula-tion and Social Affairs. The CPS is based on the WHO's 13th General Programme of Work (GPA) 2019-2025 and national priorities. It enables WHO to support Niger in the implementation of its national health policy and the 2022-2026 Health and Social Development Plan (HSSP).

Involuntary hospitalization and treatment, unsanitary living conditions and physical, psychological, and emotional abuse characterize many mental health services across the world.
