Environmental exposure to active pharmaceutical ingredients (APIs) can have negative effects on the health of ecosystems and humans. While numerous studies have monitored APIs in rivers, these employ different analytical methods, m...easure different APIs, and have ignored many of the countries of the world. This makes it difficult to quantify the scale of the problem from a global perspective. Furthermore, comparison of the existing data, generated for different studies/regions/continents, is challenging due to the vast differences between the analytical methodologies employed. Here, we present a global-scale study of API pollution in 258 of the world’s rivers, representing the environmental influence of 471.4 million people across 137 geographic regions. Samples were obtained from 1,052 locations in 104 countries (representing all continents and 36 countries not previously studied for API contamination) and analyzed for 61 APIs. Highest cumulative API concentrations were observed in sub-Saharan Africa, south Asia, and South America. The most contaminated sites were in low- to middle-income countries and were associated with areas with poor wastewater and waste management infrastructure and pharmaceutical manufacturing. The most frequently detected APIs were carbamazepine, metformin, and caffeine (a compound also arising from lifestyle use), which were detected at over half of the sites monitored. Concentrations of at least one API at 25.7% of the sampling sites were greater than concentrations considered safe for aquatic organisms, or which are of concern in terms of selection for antimicrobial resistance. Therefore, pharmaceutical pollution poses a global threat to environmental and human health, as well as to delivery of the United Nations Sustainable Development Goals.

Human strongyloidiasis is a chronic parasitic disease caused by infection with Strongyloides stercoralis, a soil-transmitted helminth that is estimated to infect 300–600 million people worldwide. This neglected tropical disease (NTD) is endemic globally, predominately in the South-East Asia, Afric...an and Western Pacific regions, and in South and Central America. Strongyloidiasis has a wide range of clinical presentations, including subclinical disease, symptomatic disease (often with diarrhoea, abdominal pain and urticaria) and a rare but deadly complication of hyperinfection with disseminated disease. The feared complication of disseminated strongyloidiasis can occur in the setting of immunocompromising conditions (e.g. human T-cell lymphotropic virus type 1 infection and malignancies) or immunosuppressive medications (e.g. steroids) and has an estimated case-fatality rate exceeding 60%. The standard treatment for chronic S. stercoralis infection is oral medication with ivermectin.

Affected regions are often remote and unstable, with limited resources for treating this disease. Doctors Without Borders calls leishmaniasis one of the most dangerous neglected tropical diseases. The organization also states this disease is second only to malaria in parasitic causes of death.

These infections affect the poorest and most deprived communities with poor access to clean water, sanitation and hygiene in tro...pical and subtropical areas, with the highest prevalence reported from sub-Saharan Africa, China, South America and Asia. They are transmitted by eggs present in human faeces, which in turn contaminate soil in areas where sanitation is poor. Over 260 million preschool-age children, 654 million school-age children,108 million adolescent girls and 138.8 million pregnant and lactating women live in areas where these parasites are intensively transmitted, and are in need of treatment and preventive interventions.

Fascioliasis is an animal parasitic disease (in cattle and sheep, but also pigs, donkeys, Llamas and alpacas that can be transmitted to humans. The infection is caught when aquatic or semi-aquatic plants with the larvae of the worm attached to their stem or leaves are ingested raw or poorly cooked. ...Once in the human body, the worm lodges itself in the main bile ducts and is responsible for severe morbidity. Endemic areas are distributed globally, but some of the most affected communities are found in the Andean highlands of South America. Triclabendazole is the only drug recommended for prevention and treatment and is provided free-of-charge to endemic countries through WHO. It can be distributed in mass interventions when clusters of cases are detected.

Human schistosomiasis otherwise called bilharzia, is a fresh- water snail transmitted intravascular debilitating disease resulting from infection by the parasitic dimorphic Schistosoma trematode worms, which lives in the bloodstream of humans. The World Health Organization (WHO) regards the disease ...as a neglected tropical disease, with an estimated 732 million persons being vulnerable to infection worldwide in renowned transmission areas. Steinmann and co-workers documented that over 200 million individuals from Africa, Asia, and South America are infected with this disease. The WHO further estimated that schistosome infections and geohelminths accounts for over 40% of the world tropical disease burden with the exclusion of malaria. Humans get infected with this disease when they make contact with water contaminated with the skin-penetrating cercariae. Prevalence of schistosomiasis, at present, is still high in sub-Saharan Africa. In 2008, 17.5 million people were treated globally for schistosomiasis, 11.7 million of those from sub-Saharan Africa only. Approximately 120 million individuals in sub-Saharan Africa have schistosomiasis-related symptoms while about 20 million undergo hardship as a result of chronic presentations of the disease.

The arrival and rapid spread of the mosquito-borne viral disease Chikungunya across the Americas is one of the most significant public health developments of recent years, preceding and mirroring the subsequent spread of Zika. Globalization in trade and travel can lead to the importation of these vi...ruses, but climatic conditions strongly affect the efficiency of transmission in local settings. In order to direct preparedness for future outbreaks, it is necessary to anticipate global regions that could become suitable for Chikungunya transmission. Here, we present global correlative niche models for autochthonous Chikungunya transmission. These models were used as the basis for projections under the representative concentration pathway (RCP) 4.5 and 8.5 climate change scenarios. In a further step, hazard maps, which account for population densities, were produced. The baseline models successfully delineate current areas of active Chikungunya transmission. Projections under the RCP 4.5 and 8.5 scenarios suggest the likelihood of expansion of transmission-suitable areas in many parts of the world, including China, sub-Saharan Africa, South America, the United States and continental Europe. The models presented here can be used to inform public health preparedness planning in a highly interconnected world.

While many of the countries hit by the COVID-19 in the first few months of the year are now beginning to relax lockdown measures as infection and death rates fall, in the regions most affected by HIV, TB and malaria, such as Africa, South Asia and Latin America, the pandemic continues to accelerate.... In lower resource settings, lockdowns are less effective and hard to sustain, and clinical care facilities are extremely limited. In such environments, the response to COVID-19 must focus on containing the pandemic’s spread as far as possible through testing, contact tracing and isolation, protecting the health workforce through training and the provision of personal protective equipment (PPE) and minimizing the knock-on impact on other diseases through shoring up fragile health systems, and adapting existing disease programs.
