                                                                The    international    community    sits    at    the    tipping    pointof    a    post-‐antibiotic    era,    where    common     bacterial     infections     are     no     longer     treatable     with     the     antibiotic    armamentarium    that    exists.    In    South    Africa,    t...he    identification    of    the    first    case    of    pan-‐resistant    Klebsiella     pneumoniae(Brink     et     al,     J     Clin     Microbiol. 2013;51(1):369-‐72)    marks     a     watershed     moment     and     highlights    ourtip     of     the     antibiotic     resistance    ‘iceberg’      in      this      country.      Multi-‐drug      resistant      (MDR)-‐bacterial      infections,    predominantly    in    Gram-‐negative    bacteria    such    as    Klebsiella    pneumoniae,    Escherichia    coli,    Pseudomonas     aeruginosaand    Acinetobacter     baumanniiare    now    commonplace    in    South    African    hospitals.    Whilst    a    number    of    expensive    new    antibiotics    for    Gram-‐positive     bacterial     infections     have     been     manufactured     recently     (some     of     which     are    licenced     for     usein     South     Africa),     no     new     antibiotics     active     against     Gram-‐negative    infections    are    expected    in    the    next    10-‐15years. Since the first State of the World’s Antibiotics report in 2015, antimicrobial resistance has leveled off in some high-income countries but continues to rise in many low- and middle-income countries (LMICs), where access to antibiotic...s has risen with increases in gross domestic product per capita. Per capita antibiotic consumption in LMICs is lower than in high-income countries, despite a higher infectious disease burden; however, consumption rates are rapidly converging. These trends reflect both better access to antibiotics for those who need them and increases in inappropriate antibiotic use.

Melarsoprol resistance emerged as early as the 1970s and was widespread by the late 1990s.
Melarsoprol resistance represents the only example of widespread drug resistance in HAT patients wher...e the genetic mechanism has been established.
The current goal of elimination of HAT as a public health problem by 2020 may be undermined by the emergence and spread of resistance to current or new drugs.
Insights into potential resistance mechanisms for current and new drugs will facilitate predictions of the likelihood of resistance and will also facilitate rational approaches to minimizing, monitoring, and tackling the future emergence of resistance.

The operational handbook provides practical information and tools that complement the recommendations in the guidelines. The strategies described in the operational handbook are based on the latest WHO recommendations which were formulated by Guideline Development Groups using the GRADE approach. In many cases however, the recommendations in their current form lacked sufficient clinical and programmatic detail, which is important for implementation. This operational handbook complements the guidelines with practical advice based on best practices and knowledge from the fields such as pharmacokinetics, pharmacodynamics, microbiology, pharmacovigilance and clinical and programmatic management.

IRC health programming across its portfolio, in terms of the size and breadth, responds to significant needs in crisis affected settings, improving health and wellbeing while reducing causes of ill-health.
This five-year Health Strategy sharpens our focus on where we can have the most impact. It guides our efforts in planning, technical assistance, business development, advocacy, and internal and external collaboration. Through this strategy, we will invest and grow in areas that will help us achieve high impact at scale for our clients. For the next five years these priorities will include: Nutrition; Immunization: Infectious Disease Prevention and Control; Last Mile Delivery of Primary Health Care: Clean Water.
Our strategy aligns with Strategy 100 (S100) and Strategy Action Plans (SAPs). It lays out how IRC, through health, nutrition, and Environmental Health (EH) programming, will advance the IRC’s S100 ambitions, respond to global trends, and capitalize on our value add. The strategy will be complemented by delivery plans that detail investments, actions, and roles and responsibilities to advance our priorities. At the end of FY24, we will take stock of the implementation of the strategy, measure progress towards achieving our goals, and review if it continues to be fit for purpose.

It  provides  clear  roles  for  health  workers  at  the  different  levels  of  our  devolved  system  which  will  ensure  a  harmonized  referral  system  with  basic  cardiovascular  diseases  treatment services available closest to the people while decongesting the county and national referral facilities.
