Die im Epidemiologischen Bulleti...n 32/2021 veröffentlichte Studie stellt die Validierung der Lolli-Methode und die Implementierung des Testkonzeptes in 32 Kölner Kitas im Zeitraum September 2020 bis März 2021 vor. Das Testkonzept Lolli-Methode basiert auf der Kombination einer einfachen Probenentnahme und anschließender PCR-Pooltestung. Es wird seit April 2021 an allen Kitas und Schulen der Stadt Köln und seit Mai 2021 an allen Grund- und Förderschulen in Nordrhein-Westfalen durchgeführt.

The toolkit is divided into two parts. Part I contains background information and a conceptual framework for creating dementia-incl...usive societies. Part II includes four practical modules, each featuring a series of practical steps and exercises. The four modules focus on: starting a new dementia-friendly initiative (DFI), integrating dementia into an existing initiative, monitoring and evaluation a DFI, and scaling a DFI. The modules can be used together or separately and offer guidance that can be adapted to suit local needs and settings.

The objective of the study was the validation and implementation of GeneXpert MTB/RIF for routine use in the rapid detection of tuberculosis and sensitivity to rifampicin in clinical samples; for this, 1592 respiratory samples were collected and analyzed in the laboratory of Instituto Nacional de In...vestigación en Salud Pública Guayaquil. The analysis of the results of GeneXpert in comparison with smear microscopy showed an initial sensitivity of 99.8% and specificity of 93.2%; The analysis of discrepancies using the results of the culture as a reference method showed that the GeneXpert results considered false negatives turned out to be true negatives, the same happens with the false positives that correspond to true positives. Recalculated the sensitivity and specificity of the GeneXpert was 99.8% and 100% correspondingly. The comparison with the drugs susceptibility test showed a sensitivity of 91.4% and a specificity of 95.5% for the GeneXpert MTB/RIF system. It is concluded that the implementation of the GeneXpert system allows solution to certain problems associated with the application of conventional diagnostic methodologies, decreasing the waiting times, and increasing the sensitivity and specificity in the diagnosis of drug-resistant tuberculosis, thus generating a valuable opportunity for early diagnosis.

Ces lignes directrices provisoires offrent des recommandations sur les utilisations prioritaires des tests de diagnostic rapide à détection d'antigène (TDR-Ag) dans des populations et des contextes spécifiques, notamment (i) pour la détection primaire des cas chez les personnes symptomatiques suspectées d'être infectées et les personnes asymptomatiques à haut risque de COVID-19, (ii) pour la recherche des contacts, (iii) pendant les enquêtes sur les épidémies et (iv) pour surveiller les tendances de l'incidence de la maladie dans les communautés. Les TDR-AG répondant aux exigences minimales de performance peuvent être utilisés en dehors des milieux cliniques et des laboratoires, y compris dans les communautés, par des opérateurs formés conformément aux instructions. governmental organizations (NGOs) and researchers
who seek a comprehensive presentation of the full range
of issues, including institutions and legal concepts with
which they may be unfamiliar. It is also designed to serve
as a factual resource for the three organizations’ technical
cooperation activities.
This update revises the information contained in that
insert in the light of more recent developments as of

Assessment and management of prolonged symptoms of COVID-19.

Apesar de inicialmente ter sido considerada uma infecção aguda do sistema respiratório,
atualmente sabe-se do comprometimento multissistêmico e protraído da COVID-19 em alguns indivíduos [1]. Segundo dados prelimi...nares de estudos observacionais, estima-se que aproximadamente 10% dos pacientes com quadros leves a moderados de COVID-19 apresentam sintomas prolongados, que duram 3 semanas ou mais [2]. Essa condição tem sido chamada de COVID “longa”, “pós-aguda” ou
“síndrome pós-COVID-19”. Entre os pacientes que necessitam internação, especialmente em UTI, estes sintomas residuais podem ser muito mais frequentes: mais que 80% dos pacientes reportaram ao menos 1 sintoma após 60 dias do início do quadro.

As pandemias virais costumam representar sérias ameaças ao equilíbrio dos sistemas de saúde, ao impor-lhes demandas extraor-dinárias e sustentadas, que podem ultrapassar a capacidade de atendimento, no que diz respeito tanto aos insumos e às tecnologias disponíveis, quanto aos recurs...os humanos/profissionais capacitados. Trazem o enorme desafio de equilibrar igualdade entre as pessoas e equidade na distribuição de riscos e benefícios entre elas.
Diante desse cenário de casos cada vez mais frequentes de COVID-19 na
população, surge a premente necessidade de se avaliarem as melhores práticas para a otimização da utilização dos meios e recursos disponíveis. Dessa forma, considerando o iminente risco de indisponibilidade de leitos de terapia intensiva e respiradores para todos os indivíduos, acometidos ou não pela doença, torna-se fundamental estabelecer critérios clínicos, técnicos e éticos, para a melhor utilização dos mesmos, para se conseguir otimizar resultado e gerar o maior benefício possível.
