Improvements in water sanitation and hygiene (WASH) and wastewater management in all sectors are critical elements of preventing infections and reducing the spread of antimicrobial resistance (AMR) as identified in the Global Action Plan to combat AMR. Yet, at present, WASH and wastewater management... actors and improvement actions are under-represented in AMR multi-stakeholder platforms and national action plans (NAPs). This WHO/FAO/OIE technical brief on WASH and wastewater management to reduce the spread of AMR provides a summary of evidence and rationale for WASH and wastewater actions within AMR NAPs and sector specific policy to combat AMR. Evidence and actions are presented in the domains of; coordination and leadership, households and communities, health care facilities, animal and plant production, manufacturing of antimicrobials, and surveillance and research.

While many of the countries hit by the COVID-19 in the first few months of the year are now beginning to relax lockdown measures as infection and death rates fall, in the regions most affected by HIV, TB and malaria, such as Africa, South Asia and Latin America, the pandemic continues to accelerate.... In lower resource settings, lockdowns are less effective and hard to sustain, and clinical care facilities are extremely limited. In such environments, the response to COVID-19 must focus on containing the pandemic’s spread as far as possible through testing, contact tracing and isolation, protecting the health workforce through training and the provision of personal protective equipment (PPE) and minimizing the knock-on impact on other diseases through shoring up fragile health systems, and adapting existing disease programs.

At a time when chronic disease challenges not only rich but now also poor countries, when infections like Ebola that for years were hidden in Africa pose a threat to the global situation, Christians have to reflect on the question of the proprium of Christian health care.

The Zambia Population Based HIV impact assessment of 2016, reported the prevalence of viral hepatitis in Zambia as ranging between 5.6% among adults aged 15 to 59% in the general population, and 7.1% among HIV infected individuals. It is estimated that the majority of persons with chronic hepatitis ...B and/ or hepatitis C are unware of their infection and do not benefit from promotive, preventive and curative services designed to reduce onward transmission. Zambia introduced hepatitis B virus vaccine to the routine Under 5 vaccination schedule in 2005. Preliminary results from the ZAMPHIA indicate that hundreds of infections have been abated in children since then. However, its also clear that we continue to miss key opportunities to prevent transmission, diagnose and treat infections, prevent serious disease, and in many cases cure people. In addition, high risk groups inter alia health care workers still have limited access to the vaccine.

The economic costs of NCDs are significant and are due principally to their impact on the non-health sector (reduced workforce and productivity). 2nd edition. Children with TB comprise about 10-12% of the total TB cases diagnosed in the country. This burden is likely to be higher given the challenges in diagnosing TB in children. The symptoms of TB in children mimic those of other childhood diseases. Children do not readily expectorate and th...ey have pauci-bacillary TB hence some will be missed using bacteriological tests. The government has however introduced GeneXpert molecular testing that is more sensitive than microscopy in detecting TB. Health care workers therefore need a reference guide to obtaining sputum from children for testing. Treatment of TB in children has been reviewed and now includes Ethambutol. There are now improved paediatric friendly TB medicines for treatment of TB in children and health care workers need a reference guide to enable them accurately dispense the TB medicine to children. Malnutrition is a common predisposing factor for TB in children. On the other hand, TB predisposes children to malnutrition or worsens an existing state of malnutrition. Nutrition care and support forms an integral part of treatment for a child with TB disease.

Geflüchtete Menschen in Aufnahmeeinrichtungen werden in bevölkerungsbezogenen Erhebungen, Routinedaten und amtlichen Statistiken bislang unzureichend berücksichtigt. Im Rahmen des Forschungs- und Entwicklungsvorhabens „Surveillance der Gesundheit und primärmedizinischen Versorgung von Asylsuch...enden in Aufnahmeeinrichtungen“ (PriCare) wurde daher ein Ansatz für ein Gesundheitsmonitoring durch Sekundärnutzung medizinischer Routinedaten in den Ambulanzen der Aufnahmeeinrichtungen für geflüchtete Menschen entwickelt. Hierzu wurde eine Dokumentationssoftware (Refugee Care Manager, RefCare©) zur Digitalisierung und Harmonisierung der Primärdokumentation entwickelt und in Aufnahmeeinrichtungen dreier Bundesländer implementiert. Der Ansatz des verteilten Rechnens in einem Surveillancenetzwerk ermöglicht durch dezentrale aber harmonisierte Analysen, die datenschutzkonforme Sekundärnutzung dieser medizinischen Routinedaten ohne zentrale Speicherung personenbezogener Informationen. Durch eine integrierte Monitoringfunktion können 64 Indikatoren zur Population, Morbidität sowie zu Versorgungsprozessen und -qualität routinemäßig und einrichtungsübergreifend ausgewertet werden. Der Beitrag beschreibt das konzeptionelle und praktische Vorgehen, das technische Verfahren sowie exemplarische Ergebnisse dieses Monitoringsystems.

To support the operationalization of quality CHW program design and implementation, USAID, UNICEF, the Community Health Impact Coalition, and Initiatives Inc. have updated and adapted the Community Health Worker Assessment and Improvement Matrix (CHW AIM) Program Functionality Matrix [12]. This tool can be used to identify design and implementation gaps in both small- and national-scale CHW programs, and close gaps in policy and practice.
